The difficult intubation kit.

Abstract

A kit for difficult intubation can be assembled quickly from vascular catheters and sheaths commonly available in surgical facilities. The kit provides for continuous oxygen administration throughout all phases of its application in difficult upper airway management. Such applications include stylet-guided endotracheal intubation, cricothyroid puncture, transtracheal ventilation, and translaryngeal catheter-guided retrograde tracheal intubation. A technical description of the Difficult Intubation Kit and guidelines for its use in difficult airway management are presented.

A conductive polymeric material used for nanospray needle and low-flow sheathless electrospray ionization applications.

A conductive polypropylene/graphite mixture is used for the production of polymeric nanospray needle emitters and as a coating on fused-silica capillaries that are used for sheathless electrospray ionization (ESI). The described production of these polymeric nanospray needle emitters and sheathless ESI contacts is exceptionally easy and at a very low cost. The described polymeric nanospray emitters have shown excellent features regarding their chemical inertness and spray performance. The long-term stability of the nanospray needles exceeds 24 h of continuous use. Furthermore, the resistance to electrical discharges, which is one of the factors that often limits the lifetime of metal coated tips, has proven to be outstanding. A voltage of up to 5 kV could be applied without loss of spray performance. The use of polypropylene emitters offers a number of desirable features, as compared to silica based emitters. Among these features are mechanical flexibility and simplified regeneration of the nanospray needle. Continuous nanospray of peptides and proteins in conjunction with orthogonal time-of-flight mass spectrometry are shown with signal relative standard deviations of 5%. In addition, the polypropylene/graphite mixture has also been applied as the conductive contact for sheathless ESI in fast capillary electrophoresis separations.

Rapid microfluidic mixing.

A preformed T-microchannel imprinted in polycarbonate was postmodified with a pulsed UV excimer laser (KrF, 248 nm) to create a series of slanted wells at the junction. The presence of the wells leads to a high degree of lateral transport within the channel and rapid mixing of two confluent streams undergoing electroosmotic flow. Several mixer designs were fabricated and investigated. All designs were relatively successful at low flow rates (0.06 cm/s, > or = 75% mixing), but had varying degrees of success at high flow rates (0.81 cm/s, 45-80% mixing). For example, one design operating at high flow rates was able to split an incoming fluorescent stream into two streams of varying concentrations depending on the number of slanted wells present. The final mixer design was able to overcome stream splitting at high flow rates, and it was shown that the two incoming streams were 80% mixed within 443 microm of the T-junction for a flow rate of 0.81 cm/s. Without the presence of the mixer and at the same high flow rate, a channel length of 2.3 cm would be required to achieve the same extent of mixing when relying upon molecular diffusion entirely, while 6.9 cm would be required for 99% mixing.

A useful modification of the Kerrison rongeur.

STUDY DESIGN: A useful modification of the Kerrison rongeur is reported. OBJECTIVES: To modify a rongeur of the Kerrison type to avert the problems associated with conventionally designed rongeurs. METHODS: A commercial rongeur of the Kerrison type was equipped with a special chip collector attached immediately behind the cutting edge. RESULTS: Conventionally designed rongeurs necessitate the continuous removal of resected bone chips. This time-consuming activity can be avoided easily by the use of the modified rongeur. CONCLUSIONS: The design of the chip collector fully met the expectations for the modification: no clogging of resected bone material, easy clinical use, and shorter operating times. The modified rongeur, available in two lengths (190 and 290 mm) and three sizes of the chip collector (diameter of 4, 6, and 8 mm) proved to be useful in clinical applications.
