[Functional mobility measurement of removable partial dentures using newly-developed analyzing system in six degrees of freedom--effect of cusp inclination of molar artificial teeth on denture displacement in function].

Abstract

A measuring system to record three-dimensional displacement of removable partial dentures (RPDs) in six degrees of freedom has been developed. The system employed three pairs of photodiode and LED that were set up in a triangle. This system was capable of recording the in vivo RPD displacement with a recording sensitivity of 0.1 mm. In vivo displacement of upper RPDs with unilateral distal extension saddle was investigated using this measuring system to determine the effect of artificial cusp inclination of molar teeth on denture mobility. Two groups of different artificial dentition were switched to be attached to each of three experimental RPDs. The artificial teeth with cusp inclinations were calibrated to contact with opposing natural dentition during lateral grinding of the mandible, while the other artificial teeth were prepared to disclude with the mandibular teeth (flat-cusp group). Displacement range and average position of a reference point at the first molar of the RPDs during mastication were measured and analyzed as a function of the cusp inclination and the test food. As a result, the average displacement of the RPDs toward the edentulous ridge was smaller in the cusp group than that in the flat-cusp group, while the range of horizontal displacement in the buccal-palatal direction was greater in the former group than the latter. The denture wearers demonstrated typical grinding strokes at the end of the closing phase of the mandiblar movements when they chewed a peanut, which was attributed to a wide range of horizontal displacement of the dentures.

[A partially edentulous case, increased occlusal vertical dimension by prosthetic treatment].

PATIENT: A 53-year-old male presented with the chief complaint of masticatory disturbance caused by many teeth missing in the upper and lower jaws. There was extensive attrition on incisors and canines which only had occlusal contact, so the case was diagnosed as overclosure. To improve masticatory disturbance as the chief complaint, removable partial dentures were made after treating the abutment teeth. An adequate vertical dimension was planned in order to restore the worn teeth on the cast, and to arrange artificial teeth in the edentulous area. DISCUSSION: An anterior teeth core made as an inter-occlusal record was useful and the arrangement of artificial teeth in the edentulous area useful for estimating the adequate vertical dimension. CONCLUSION: It is important to diagnose an adequate vertical dimension by creating acast of the original tooth form in the case of prosthetic treatment of severely worn teeth.

[The establishment of 3D-FE model libraries for the elements of the RPD].

PURPOSE: To establish the 3D-FE model libraries for the elements of the RPD. METHODS: Based on the 3D geometric model libraries of the RPD and normal stomatognathic system, using the software, such as Powershape,Pro/E and Ansys5.5, transformed data, trimmed models, or rebuilt it, established many kinds of FE model elements, then combine these elements in the whole model, connected and trimmed, at last created several kinds of FE model libraries. RESULTS: This study established eight kinds of FE model libraries, including the occlusal rests, clasps, bases, artificial teeth, and connectors, etc, nearly two hundreds FE model elements. CONCLUSION: The established models have a satisfied geometric configuration; these elements combined well in the whole model, and can be easily call-in and edit.
