New study surveys data on operating room efficiency.

Abstract

The folks doing knee and hip replacement surgery at Bryn Mawr (PA) Hospital must be doing something right: For the second year in a row, the 330-bed hospital has been named Best Performer for operating room effectiveness and efficiency by OR Benchmarks, a Santa Fe, NM-based benchmarking company. The hospital was one of a dozen studied that provided data to OR Benchmarks on 51 cases of total knee replacements. Thus far, the company has collected data on 880 cases over the past five years. Data collected ranged from cost of supplies to use of clinical pathways.

Sports after hip resurfacing arthroplasty.

BACKGROUND: No information is available concerning patients' sports activity after hip resurfacing arthroplasty. HYPOTHESIS: Patients treated by hip resurfacing arthroplasty will be able to achieve a high level of sports activity. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: We surveyed 112 patients by postal questionnaire to determine their sporting activities at a mean of 23.5 (range, 9-40) months after hip resurfacing surgery. Data were also analyzed separately for older and younger patients and women and men, respectively. RESULTS: Of 112 patients, 110 participated in an average of 4.6 sport disciplines after surgery, compared with 105 patients pre-operatively engaged in an average of 4.8 disciplines. About 26% performed sports 4 times per week or more--almost 60% for longer than 60 min at each session. A high proportion of our patients were still engaged in downhill skiing (51%) and in high-impact disciplines such as tennis (12%) or contact sports (22%). More than 50% of the patients commenced sports within the first 3 months after surgery. During activities, 70% were completely pain-free. The group of older patients (mean age, 60.2 years) participated in more different disciplines with a higher weekly frequency and longer session length than younger patients (mean age, 46.4 years). CONCLUSION: The present study reports for the first time on the detailed sports activity and activity extent after hip resurfacing arthroplasty. Our patients could return to a high level of sports participation after surgery. Age-related differences were found that have not yet been published. Nevertheless, further follow-up is necessary to assess the influence of high-level activity on loosening and revision rates after hip resurfacing arthroplasty.

Mesenchymal stem cells regulate angiogenesis according to their mechanical environment.

In fracture and bone defect healing, MSCs largely drive tissue regeneration. MSCs have been shown to promote angiogenesis both in vivo and in vitro. Angiogenesis is a prerequisite to large tissue reconstitution. The present study investigated how mechanical loading of MSCs influences their proangiogenic capacity. The results show a significant enhancement of angiogenesis by conditioned media from mechanically stimulated compared with unstimulated MSCs in two-dimensional tube formation and three-dimensional spheroid sprouting assays. In particular, proliferation but not migration or adhesion of endothelial cells was elevated. Promotion of angiogenesis was dependent upon fibroblast growth factor receptor 1 (FGFR1) signaling. Moreover, stimulation of tube formation was inhibited by vascular endothelial growth factor receptor (VEGFR) tyrosine kinase blocking. Screening for the expression levels of different soluble regulators of angiogenesis revealed an enrichment of matrix metalloprotease 2, transforming growth factor beta1, and basic fibroblast growth factor but not of vascular endothelial growth factor in response to mechanical stimulation. In conclusion, mechanical loading of MSCs seems to result in a paracrine stimulation of angiogenesis, most likely by the regulation of a network of several angiogenic molecules. The underlying mechanism appears to be dependent on the FGFR and VEGFR signaling cascades and might be mediated by an additional cross-talk with other pathways.

Nonidentical and outlier duty cycles as factors accelerating UHMWPE wear in THA: a finite element exploration.

Wear rate and wear direction vary considerably within total hip arthroplasty (THA) patient cohorts. Third body effects and wide-ranging differences in patient activity levels are two factors suspected of contributing to wear variability. A sliding-distance-coupled contact finite element formulation was used to test the hypothesis that nonidentical duty cycles (differing activities, or change of third body challenge) produce accelerations in polyethylene wear. Effects of nonidentical duty cycles, time-variant femoral head roughening, and outlier gait inputs were investigated. Without femoral head roughening, combination walk/stair-climb wear simulations did not result in appreciably higher volumetric wear than a walk-only simulation, but when a roughened zone was included, walk/stair-climb volumetric wear increased by approximately 57% above that of a similarly roughened walk-only simulation. To investigate time-variant femoral head roughening, wear simulations were begun with femoral head roughening at one location on the femoral head, switching to another location halfway through the simulation. Results varied depending on roughening sites, but cases of substantial increase in wear involved a transient jump in wear rate shortly after the change of head roughening location. Outlier duty cycles were simulated by increasing or decreasing the joint contact force and range of motion inputs, to levels at the 97.5th and 2.5th percentiles of a population of normal subjects. The resulting wear showed an increase or decrease closely proportional to the percentage by which each input (force or range of motion) was changed.
