[Problems in the indication for hysterectomy post conization].

Abstract

The importance of histo-pathological diagnosis after conisation of the Cervix uteri in view of hysterectomia post conisation is discussed (94 Carcinomata in situ, 53 cases of dysplasia and 13 microcarcinomata). If there is a modern technique of coldknife-conisation, an intensive histo-pathological diagnostic regime and the possibility of cytological and colposcopical control, hysterectomia is not necessary in cases of complete removal of carcinoma in situ by conisation.

p53 over-expression is an early event in the development of human squamous-cell carcinoma of the larynx: genetic and prognostic implications.

The tumor-suppressor protein p53 is over-expressed in a large fraction of squamous-cell carcinomas of the larynx (LSCCs). p53 overexpression is dependent upon the synthesis of mutated versions of the protein and has been associated with the malignant progression of certain tumor types. In order to examine the prognostic value of p53 immunodetection in LSCCs, we performed a retrospective analysis on a selected series of tumors, using the PAb 1801 and CM1 antibodies. No significant difference in the frequency of p53 over-expression was observed between tumors from patients with early relapse (67%) and those who had been disease-free for more than 5 years (84%). The lack of correlation of p53 immunoreactivity with clinical stage and differentiation grade of LSCCs, together with the coordinated expression of p53 in primary tumors and the corresponding lymph-node metastases, indicate that p53 over-expression is probably unrelated to the biological aggressiveness of these tumors. In addition, the detection of p53 immunostaining in pre-invasive areas as well as in preneoplastic lesions suggests that p53 abnormalities probably constitute a very early event in LSCC development.

Synergistic effects of photoactivated tetra(4-sulfonatophenyl)porphine and nocodazole on microtubule assembly, accumulation of cells in mitosis and cell survival.

Human carcinoma cells of the line NHIK 3025 were incubated with meso-tetra(4-sulfonatophenyl)porphine (TPPS4) for 18 h and exposed to light in the absence or presence of nocodazole. Nocodazole (1 microgram ml-1) was applied to the cells 15 min prior to light exposure and washed off the cells immediately afterwards. The presence of nocodazole during photoactivation of TPPS4-loaded cells leads to a significantly reduced ability of tubulin to repolymerize after withdrawal of nocodazole, an increased accumulation of the cells in mitosis with a larger fraction in c-metaphase and a higher yield of photoactivated cells. A higher proportion of the cells accumulating in mitosis 6-12 h after exposure to light is unable to form colonies when exposed to light in the presence of nocodazole than in its absence. The present results are consistent with a specific TPPS4-induced photodamage to the unpolymerized form of the microtubule components.

Log-linear modelling of pairwise interobserver agreement on a categorical scale.

This article uses log-linear models to describe pairwise agreement among several raters who classify a sample on a subjective categorical scale. The models describe agreement structure simultaneously for second-order marginal tables of a multidimensional cross-classification of ratings. Practical difficulties arise in fitting the models, because models refer to pairwise marginal tables of a very large and sparse table. A standard analysis that treats the marginal tables as independent yields consistent estimates of model parameters, but not of the covariance matrix of the estimates. We estimate the covariance matrix using the jackknife. We apply the models to describe agreement between evaluations made by seven pathologists of carcinoma in situ of the uterine cervix, using a five-level ordinal scale. Previous analyses showed differences among the pathologists in their pairwise levels of agreement, but we observe near homogeneity in the dependence structure of their ratings.
