[Interface diffuse keratitis].

Abstract

Diffuse interface keratitis is a new clinical entity. It is becoming significantly more common due to the increasing of laser assisted in situ keratomileusis (LASIK) procedures. The knowledge we have about this uncommon but potentially sight-threatening complication is expanding. It appears in the early postoperatory LASIK, even in well-executed procedures. In this review we present an updated strategy for the diagnose and treatment of this syndrome, based on our own experience and an updated literature review. Early identification, evaluation, staging, and appropriate intervention, may help preventing visual loss associated with this entity.

Corneal confocal microscopy: a non-invasive surrogate of nerve fibre damage and repair in diabetic patients.

AIMS/HYPOTHESIS: The accurate detection, characterization and quantification of human diabetic neuropathy are important to define at risk patients, anticipate deterioration, and assess new therapies. Corneal confocal microscopy is a reiterative, rapid, non-invasive in vivo clinical examination technique capable of imaging corneal nerve fibres. The aim of this study was to define the ability of this technique to quantify the extent of degeneration and regeneration of corneal nerve fibres in diabetic patients with increasing neuropathic severity. METHODS: We scanned the cornea and collected images of Bowman's layer (containing a rich nerve plexus) from 18 diabetic patients and 18 age-matched control subjects. RESULTS: Corneal nerve fibre density (F(3)=9.6, p<0.0001), length (F(3)=23.8, p<0.0001), and branch density (F(3)=13.9, p<0.0001) were reduced in diabetic patients compared with control subjects, with a tendency for greater reduction in these measures with increasing severity of neuropathy. CONCLUSION/INTERPRETATION: Corneal confocal microscopy is a rapid, non-invasive in vivo clinical examination technique which accurately defines the extent of corneal nerve damage and repair and acts as a surrogate measure of somatic neuropathy in diabetic patients. It could represent an advance to define the severity of neuropathy and expedite assessment of therapeutic efficacy in clinical trials of human diabetic neuropathy.

Penetrating corneal transplant with inadvertent corneal button inversion.

PURPOSE: To report a penetrating corneal transplant in which there was inadvertent inversion of the corneal button. DESIGN: Interventional case report. METHODS: A 48-year-old man with lattice corneal dystrophy had a third penetrating keratoplasty in the right eye 3 years after the second procedure and 2 years following renal transplantation. RESULTS: Histologic examination of the corneal button from the second penetrating keratoplasty disclosed inadvertent corneal graft inversion. Survival epithelium from the donor in the anterior chamber may be explained by the ocular anterior chamber-associated immune deviation or by the patient's systemic cyclosporine A (CsA) treatment after renal transplantation. CONCLUSIONS: Histologically proven corneal button inversion is a rare cause of corneal graft failure.

Epidemic and sporadic cases of nontuberculous mycobacterial keratitis associated with laser in situ keratomileusis.

PURPOSE: To report national case-finding results for nontuberculous mycobacterial keratitis and describe its association with laser in situ keratomileusis (LASIK). DESIGN: Enhanced passive disease reporting. METHODS: In April 2001, we investigated a California cluster of Mycobacterium chelonae keratitis associated with hyperopic LASIK using a contact lens mask. To identify other possibly related cases, the American Academy of Ophthalmology e-mailed its members asking them to report recent cases of nontuberculous mycobacterial keratitis to the Centers for Disease Control and Prevention. RESULTS: Forty-three additional cases of keratitis were reported (onsets between August 2000 and June 2001). Of these, 31 occurred as part of two unrelated LASIK-associated outbreaks. The 12 other reported cases occurred in sporadic fashion. Of the latter cases, 4 were associated with LASIK surgery. None of the reported cases were related to the M. chelonae cluster in California. CONCLUSIONS: Laser in situ keratomileusis-associated keratitis with nontuberculous mycobacteria may be more common than previously known.
