[Onchocerciasis].

Abstract

Onchocerciasis is an infestation caused by the nematode, Onchocerca volvulus, and characterized by eye manifestations, skin lesions and troublesome itching. Although partially controlled by international mass treatment programs, onchocerciasis remains a major health hazard in endemic areas in Africa, Arabia, and the Americas. Onchocerciasis is spread by bites from infested blackflies which transmit larvae that subsequently develop into adult filariae. Skin findings are commonly non-specific, and include severe pruritus, acute and chronic dermatitis, vitiligo-like hypopigmentation and atrophy. Onchocercal ocular disease has a large spectrum of manifestations and may even lead to blindness. Diagnosis is usually made by direct visualization of the larvae emerging from superficial skin biopsies, "skin snips". In some cases, the microfilariae can also be directly observed with a slit lamp when they migrate into the anterior chamber of the eye. Ivermectin is highly microfilaricidal, and is the current drug of choice for both skin and ocular manifestations.

Odorless benzenethiols in synthesis of thioglycosides and its application for glycosylation reactions.

p-Octyloxybenzenethiol (2) was synthesized as a new odorless benzenethiol. Moreover, preparation of thioglycosides using 2 and their application for glycosylation reactions were attempted. As a result, it was found that the thioglycosides were as excellent glycosyl donors as 4-dodecylphenyl 1-thio-glycosides, which were previously reported by our group, and more useful than the previous donors in terms of fine chemistry in glycosylation reaction activated with silver triflate and N-iodosuccinimide (NIS). In addition, this method was applicable to the sialylation with NIS and triflic acid. All procedures from the preparation of thioglycosides to the glycosylation reaction could be attained completely under conditions where no malodor was generated.

Determination of piperazine in pharmaceutical drug substances using capillary electrophoresis with indirect UV detection.

A fast, selective capillary electrophoresis (CE) method was developed for piperazine counter-ion analysis and applied to the analysis of an active pharmaceutical ingredient (API) that exists as a hemipiperazine salt. Due to the poor chromophore, the detection method chosen was indirect UV detection using benzylamine as the UV absorbing probe. Piperazine quantitation was performed using diethylamine as an internal standard and the method was validated for specificity, linearity, precision, and accuracy. The results indicate the method is suitable for piperazine counter-ion analysis in support of salt form characterization.

Instruction in behavior modification can significantly alter soil-transmitted helminth (STH) re-infection following therapeutic de-worming.

Five elementary ("prototypic") schools located in five districts in central Java were selected and the children examined for helminth infections (Ascaris, Trichuris, hookworm). They were de-wormed with a course of mebendazole and provided with 6-7 months of "behavioral remediation instruction" (BRI). In other ("control") schools, children were treated with mebendazole but were not provided BRI. The objective was to determine the effectiveness of BRI in minimizing infection/re-infection following deworming. After the 6-7 month course of BRI in the prototypic schools, all the children (in both the prototypic and control schools) were re-examined for geohelminth infection. The schools in two of the five districts were omitted from further analysis because the overall prevalence of infection was low (<10%) and the infections were dominated by hookworm which are only moderately susceptible to mebendazole. Comparisons of prototypic and control schools in the other three districts provided compelling evidence that BRI was quite effective in reducing both the frequency and intensity of infection with Ascaris and Trichuris. We suggest that instructing children and adults corrects personal habits which are conducive to infection and can be an effective and safe substitute for repeated deworming, reducing the opportunity for the emergence of drug-resistant helminthes, which should prolong the time benzimidazoles may be used for treatment of geohelminth infection.
