Hand reconstruction with partial toe and multiple toe transplants.

Abstract

Microsurgical transplantation of toes to the hand can serve as an excellent method of reconstructing the severely traumatized hand. This article reviews the authors' experience with 188 great-toe and second-toe transplants. Detailed operative sequence and postoperative care are also discussed.

Glove perforation during oral surgical procedures.

OBJECTIVE: The aim of this study was to determine the incidence of glove perforation among undergraduates and residents performing maxillofacial surgery and identify procedures associated with the perforations. STUDY DESIGN: For this purpose, 200 pairs of surgical gloves were used. For inspecting the perforations, the gloves were filled with water, at least 500 ml. RESULTS: Sixteen gloves (8%) were perforated, 8 (4%) by residents and 8 (4%) by undergraduate students. Only the residents noticed that 2 gloves (1%) were perforated and identified the object causing the perforation. The left hand was the more affected with 9 perforations (4.5%). The finger pulp was the region most affected with 12 perforations (6%). The perforated forefinger appeared in 8 gloves (4%), representing the highest incidence of perforations. CONCLUSION: We concluded there was no difference in surgical glove perforation between undergraduate students and residents, the incidence of perforations was greater in the left hand (nondominant hand) and the fingers in which most perforations occurred were the forefinger and the thumb.

A new test for demonstrating the action of flexor digitorum superficialis (FDS) tendon.

The available tests for demonstrating the action of flexor digitorum superficialis (FDS) can be used on one finger at a time. A new test is proposed which can test all the four FDS tendons simultaneously. It was administered along with other available tests on 100 uninjured hands of 50 volunteers and on seven clinical cases with FDS injury. It correlated fully (100%) with the standard test on middle and ring fingers and with the pulp-to-pulp forced pinch test with the thumb on index fingers. It was more accurate than the standard test in detecting the presence of FDS action on the little finger. It was found to be in strong agreement with the modified standard test on little finger with a kappa coefficient of 0.98. It correlated fully for the absence of FDS tendon action in seven operatively proved cases. The present test is a new test which is faster and more versatile than the available tests.

Dynamic lumbrical muscle transfer for correction of posttraumatic extensor tendon subluxation.

Posttraumatic ulnar subluxation of the extensor tendon over the metacarpal head results from rupture of the radial sagittal fibers. The patient will complain of limited digital extension and pain. Various techniques have been described to correct the disorder by either reefing the sagittal fibers or using a slip of extensor tendon around the radial collateral ligament. Unfortunately, these techniques are either technically not feasible, reefing of the sagittal fibers, or result in significant stiffness, using a strip of extensor tendon. The author describes an extraarticular technique which uses a dynamic muscle transfer that is synergistic with metacarpophalangeal flexion.
