Users' perspectives on epidemiological, GIS and point pattern approaches to analysing environment and health data.

Despite examples showing the usefulness of geographical information systems (GIS) and spatial point pattern analysis in health research, there remain barriers to their widespread use within health service settings. This paper explores potential users' views on the relative usefulness of such approaches for analysing spatially referenced environmental health data. Our findings indicate that researchers and practitioners do not always prefer the approach with which they are most familiar. In addition, there is a need for higher levels of understanding of, and confidence in, GIS and point pattern analysis techniques amongst health service professionals. The greatest need is for multi-disciplinary research which uses the most appropriate approach for each investigation, rather than that with which researchers are most familiar.

[Primary prevention and health promotion--health science fundamentals for health policy].

There is a broad health policy consensus about the necessity of augmenting and improving primary prevention. To be successful, the current initiatives need to be supported by concepts, categories and classifications, meeting the criteria of logical consistency, political consensus and suitability for administration. This survey paper presents suggestions regarding to the relationship between primary prevention and health promotion, for the definition of different levels and starting-points of primary prevention as well as to concepts of evaluation and quality assurance.

Environmental health indicators in policy evaluation.

In carrying out two projects involving environmental health indicators--a national environmental health programme evaluation and an international environmental health indicator system--in parallel, it became apparent that an international indicator set has limitations regarding the evaluation of a national programme such as the Swiss National Environment and Health Action Plan (NEHAP). The international indicator set proposed by WHO serves the structured description of the underlying cause-effect chains, allows an integrated monitoring of the general environment and health situation and provides valuable international comparisons. However, the relevance of an international indicator set varies in the national context. Moreover, it does not allow the evaluation of a national implementation process, which is highly important in assessing success or failure of an environmental health promotion programme. For a comprehensive evaluation of such a programme, a specific evaluation concept derived from the formulated goals and targets needs to be developed with emphasis on evaluation of the implementation process.
