Multiple eccrine hidrocystomas.

Abstract

The clinical and pathological features and treatment of two patients with multiple eccrine hidrocystomas are presented. The first case is characterized by multiple pearly papules with a bluish hue located in the periorbital region and the bridge of the nose. The second case is characterized by multiple, skin-coloured papules located in the periorbital area, forehead, chin and nose. Both were exacerbated by a hot and humid environment. Histopathologically, both demonstrated a unilocular cyst located in the dermis, with a 2-3-layer wall composed of cuboidal epithelium that was non-keratinizing. Treatment with topical atropine sulphate 1% in aqueous solution three times a day was instituted in the first case; however, this was poorly tolerated because of blurred vision and nausea. The lesions were subsequently hyfrecated with a good response. The second case was treated with topical atropine sulphate 1% in aqueous solution three times a day with a good response.

Pharmacokinetics of timolol in aqueous humor sampled by microdialysis after topical administration of thermosetting gels.

In order to develop a thermosetting gel-based formulation, the ocular pharmacokinetics of timolol was studied utilizing microdialysis sampling technique after topical administration. A linear microdialysis probe was characterized and implanted in the anterior chamber of a rabbit. Dialysate samples collected from the aqueous humor (AH) were directly injected into the HPLC system without any pre-treatment and no interference was observed in the blank sample. The measured in vitro recovery of the probe was 57.67%; however, the in vivo recovery significantly decreased to 16.78% when assessed by the retrodialysis method, which was used to calculate the timolol concentration in AH. Although in the initial 15 min the drug concentrations in AH were comparable to that of the timolol solution, increased Cmax and significantly improved ocular bioavailability were obtained for the gel. When sodium deoxycholate (DC) was incorporated in the gel as a penetration enhancer, a 2-fold increment in the ocular bioavailability was achieved with an increased Cmax and significantly suspended Tmax. The results demonstrated that microdialysis coupled to HPLC is a powerful tool to investigate the ocular pharmacokinetic, and hence facilitates the design of ophthalmic formulations.

[Treatment of paraungual HPV73-positive Bowen disease with imiquimod cream].

Bowen disease - a squamous cell carcinoma in situ - is associated with oncogenic human papilloma viruses (HPV). The association is best established for genital Bowen disease but also holds for extragenital lesions. The immunomodulatory substance imiquimod is used for the treatment of HPV-induced skin disorders. In the case of paraungual Bowen disease, established treatment options as excision, cryosurgery, radiotherapy or laser treatment might cause persistent nail damage. We successfully treated HPV73-positive Bowen disease in this location with topical imiquimod.

[Efficacy and tolerability of topical foscarnet sodium in treatment of herpes labialis. Results of post-marketing surveillance].

BACKGROUND AND OBJECTIVE: Aim of this post-marketing surveillance study was to generate further data on efficacy and tolerability of foscarnet sodium cream 2% in patients with recurrent herpes labialis in daily routine practice. DESIGN AND METHODS: 90 outpatients with recurrent herpes labialis were instructed to apply the antiviral cream to the lesions according to package insert for up to 10 days. Symptoms were recorded before beginning of treatment, after three days, and at the end of treatment. RESULTS: Paresthesia and swelling as well as the number of vesicles were reduced after three days of observation, and were improved by about 90% at the end of treatment. Erosions and crust formation also improved markedly. Efficacy as well as tolerability were evaluated as positive by the majority of physicians and patients. No adverse reactions were reported. CONCLUSIONS: Foscarnet sodium 2% cream is an effective and safe medication for the treatment of recurrent labial herpes in daily routine practice of dermatologists.
