[Inserts for foot deformities].

Abstract

Inserts are orthopedic aids in the treatment of foot disorders that result from changes of the static or dynamic situation. Provision of appropriate orthopedic devices can relieve the pain caused by forefoot deformities either in lieu of surgical intervention or in rare cases also following surgical treatment to improve the symptoms of residual pain.Available materials provide support, padding, and cushioning. Inserts are custom-made to measure and/or based on a plaster impression. Determining the indication, prescribing the inlay, and checking the orthosis are the tasks of the physician. One treatment option for relieving the pain of forefoot deformities consists in conservative therapy with an insert combining features of padding and support as well as adjusting a ready-made shoe. The shoe and inlay should constitute a functional unit since often the optimal effect is only achieved with a combination of insert and orthopedic adjustment of the ready-made shoe.

Neglected isolated plantar dislocation of middle cuneiform: a case report.

BACKGROUND: Four cases of plantar dislocation of middle cuneiform have been reported in the english literature. All of them were fresh cases and treated with open reduction. We are reporting a case of neglected plantar dislocation of middle cuneiform which was treated with excision. CASE PRESENTATION: A farmer presented with a painful plantar dislocation of middle cuneiform bone after 9 months of injury. The bone was deformed and was excised by a plantar incision. It resulted in painless foot with no disability. CONCLUSION: The neglected plantar dislocated middle cuneiform bone becomes deformed due to repeated weight bearing. The gap gets filled with Fibrous tissue. Excision of the cuneiform gives good results.

The tailor's bunionette deformity: a field guide to surgical correction.

The author presents a detailed review of the Tailor's bunionette deformity of the fifth metatarsal with special emphasis on radiographic analysis and surgical correction. The surgical techniques discussed include (1) partial metatarsal head ostectomy; (2) metatarsal head resection; (3) minimal incision osteotomy; (4) osteotomies about the metatarsal head-neck, shaft, and base; and (5) ancillary soft tissue procedures. Techniques employed to prevent and correct potential complications are discussed in detail for each osteotomy.
