Downlink power control in multi-code CDMA for mobile medicine.

Abstract

In this paper, we propose a downlink power-control mechanism to be applied in a multi-code code division multiple access (CDMA) mobile medicine system. The mobile medicine system can provide (i) measured blood pressure and body temperature, (ii) medical signals measured by the electrocardiogram (ECG) device, (iii) mobile patient's history, (iv) G.729 audio signals, (v) Joint Photographic Experts Group 2000 Medical images and Moving Picture Experts Group 4 charge-coupled device sensor video signals. By utilizing a multi-code CDMA spread spectrum communication system with downlink power-control strategy, it is possible for this system to meet the quality of service requirements of a mobile medicine network. In addition, such a system can maximize the resource utilization. For different messages to be sent, the power is controlled according to the requisite bit error rate (BER). Higher transmission power is given to the media with lower BER requirement. Numerical analysis shows that the ratios of transmission power for voice, video, and data virtual channels is approximately 1:2:13 when the BERs for voice, video, and data are 10(wedge)(- 3), 10(wedge)(- 4), and 10(wedge)(- 7), respectively. This power ratio is similar to the ratio of signal-to-noise plus interference power ratio for voice, video, and data during transmission. For the purpose of verifying the proposed argument, a simulation has been done and the results match the derivation very well.

Central venous pulse pressure analysis using an R-synchronized pressure measurement system.

OBJECTIVE: The information derived from central venous catheters is underused. We developed an EKG-R synchronization and averaging system to obtained distinct CVP waveforms and analyzed components of these. METHODS: Twenty-five paralyzed surgical patients undergoing CVP monitoring under mechanical ventilation were studied. CVP and EKG signals were analyzed employing our system, the mean CVP and CVP at end-diastole during expiration were compared, and CVP waveform components were measured using this system. RESULTS: CVP waveforms were clearly visualized in all patients. They showed the a peak to be 1.8+/- 0.7 mmHg, which was the highest of three peaks, and the x trough to be lower than the y trough (-1.6+/- 0.7 mmHg and -0.9+/- 0.5 mmHg, respectively), with a mean pulse pressure of 3.4 mmHg. The difference between the mean CVP and CVP at end-diastole during expiration was 0.58+/- 0.81 mmHg. CONCLUSIONS: The mean CVP can be used as an index of right ventricular preload in patients under mechanical ventilation with regular sinus rhythm. Our newly developed system is useful for clinical monitoring and for education in circulatory physiology.

Beyond inpatient and outpatient care: alternative model for hypertension management.

Hypertension is a major contributor to worldwide cardiovascular mortality, however, only one-third of patients with hypertension have their blood pressure treated to guideline recommended levels. To improve hypertension control, there may need to be a fundamental shift in care delivery, one that is population-based and simultaneously addresses patient, provider and system barriers. One potential approach is home-based disease management, based on the triad of home monitoring, team care, and patient self-care. Although there may be challenges to achieving the vision of home-based disease management, there are tremendous potential benefits of such an approach for reducing the global burden of cardiovascular disease.
