The Nursing Home Behavior Problem Scale.

Abstract

Nursing home patients frequently have serious disturbances of behavior that can lead to use of chemical or physical restraints. To support research into better management of these problems, we developed the Nursing Home Behavior Problem Scale (NHBPS), a 29-item inventory of serious behavior problems designed to be completed by nurses and nursing assistants. NHBPS scores were obtained for two samples of nursing home residents: 431 in Tennessee and 122 in Texas. The interrater correlation was .754 in the Tennessee sample and .827 in the Texas sample. The NHBPS had a correlation of -.747 with the NOSIE scale and .911 with the CMAI. There was a pronounced association of increased NHBPS scores with mental impairment and use of sedative drugs or restraints. These data suggest the NHBPS is a useful research instrument for measuring serious behavior problems in nursing home residents.

The inversion In(2L)t impacts complex, environmentally sensitive behaviors in Drosophila melanogaster.

Genetic variation in behavioral traits allows organisms to respond and adapt to environmental challenges. Genetic variation in behavior is often affected by many genes and thus has a complex genetic basis. Inversions, the reorientation of genes along the chromosome, tightly link genetic variants together because they suppress recombination. Therefore, inversions are believed to have a major impact on phenotypic variation because they combine the effects of multiple genes, which can pleiotropically alter multiple aspects of behavior. This study investigates how the inversion In(2L)t, found in Drosophila melanogaster populations around the world, impacts different aspects of behavior in an environment-sensitive manner. We test the activity, foraging, and startle-induced behavior of flies with different In(2L)t genotypes across sex and temperatures. We observe that Drosophila homozygous for In(2L)t sleep less frequently, spend more time away from a food source, and have a longer duration of startle response. Additionally, the impacts of In(2L)t on aspects of behavior can be sex-specific and are largely consistent across temperatures. Taken together, our research demonstrates that inversions can regulate aspects of behavior, and suggests hypotheses explaining the distribution of In(2L)t across space and time.

Can a developed country's maternal mortality review be used as the 'gold standard' for a developing country?

BACKGROUND: Relative to other public health problems, maternal mortality ratio (MMR) differences between developed and developing countries are greater than expected. South Africa (SA) produced its first national report on maternal deaths in 1998. UK's last report covered the period 1994-1996. OBJECTIVE AND METHOD: Compare the two reports to document reasons for the MMR differences using the Safe Motherhood analytical model of maternal mortality as a template. RESULTS: The MMR for SA was estimated to be 12.3 times greater than the UK's. Under-reporting was bigger in SA. Substandard medical care was common, but other quality of care issues were not assessed. Disease pattern differences included AIDS, non-pregnancy-related infections and postpartum haemorrhage in SA compared to thromboembolism and medical disorders in the UK. Autopsies were problematic. Demographic differences centred around ethnic origins. Biological differences may involve the immune system. Socio-economic or behavioural factors were not documented. CONCLUSIONS: Awareness of the magnitude of the problem requires better data collection systems. Sepsis and HIV/AIDS are a major problem in SA. Beyond the mutually common problem of substandard medical care, other quality of care issues were inadequately assessed.

The brain-machine disanalogy revisited.

Michael Conrad was a pioneer in investigating biological information processing. He believed that there are fundamental lessons to be learned from the structure and behavior of biological brains that we are far from understanding or have implemented in our computers. Accumulation of advances in several fields have confirmed his views in broad outline but not necessarily in some of the strong forms he had tried to establish. For example, his assertion that programmable computers are intrinsically incapable of the brain's efficient and adaptive behavior has not received much examination. Yet, this is clearly a direction that could afford much insight into fundamental differences between brain and machine. In this paper, we pay tribute to Michael, by examining his pioneering thoughts on the brain-machine disanalogy in some depth and from the hindsight of a decade later. We argue that as long as we stay within the frame of reference of classical computation, it is not possible to confirm that programmability places a fundamental limitation on computing power, although the resources required to implement a programmable interface leave fewer resources for actual problem-solving work. However, if we abandon the classical computational frame and adopt one in which the user interacts with the system (artificial or natural) in real time, it becomes easier to examine the key attributes that Michael believed place biological brains on a higher plane of capability than artificial ones. While we then see some of these positive distinctions confirmed (e.g. the limitations of symbol manipulation systems in addressing real-world perception problems), we also see attributes in which the implementation in bioware constrains the behavior of real brains. We conclude by discussing how new insights are emerging, that look at the time-bound problem-solving constraints under which organisms have had to survive and how their so-called 'fast and frugal' faculties are tuned to the environments that coevolved with them. These directions open new paths for a multifaceted understanding of what biological brains do and what we can learn from them. We close by suggesting how the discrete event modeling and simulation paradigm offers a suitable medium for exploring these paths.
