Case report 703: Multifocal osteosarcoma.

Abstract

There are two theories concerning the origin of multifocal osteogenic sarcoma: In one, the lesions all arise synchronously as multiple, simultaneously appearing, primary tumors, and in the other, there appears to be one dominant site with early and rapidly progressive metastatic disease. We believe that our patient fits into the second group with a primary right (distal-end) femoral osteogenic sarcoma with early and rapid metastasis. Bone scan, chest CT, and MRI examinations played an essential role in the initial evaluation and follow-up of this patient with osteosarcomatosis. The multiple modalities also offered a greater sensitivity in the detection and for the surveillance of the progression of the condition.

The WHO Histological Typing of Odontogenic Tumours. A commentary on the Second Edition.

This article introduces the revised World Health Organization (WHO) classification of odontogenic tumors and jaw cysts and certain bone lesions that either are peculiar to the jaws or have distinctive features in that location. The new and revised classification is compared with the previous version, the reasons for the changes are outlined, and reference is made to a number of newly characterized lesions that have been included.

[MR imaging of bone metastasis].

One hundred and three patients with known malignancy and suspected bone metastasis underwent magnetic resonance imaging. In 93%, the metastatic lesions, detected both on plain films and radionuclide bone scans, showed decreased signal intensity on T1-weighted images and increased signal intensity on T2-weighted images. All lesions, except one lesion showing much blastic change on plain films, showed contrast enhancement with Gd-DTPA administration. Although further investigation is needed, a correlation between the type of primary malignancy and the signal intensity of the metastatic lesion is suggested. Among 312 metastatic lesions detected by MR imaging, 272 lesions (87%) and 134 lesions (40%) were also detected by radionuclide bone scans and plain films, respectively. In five cases, MR imaging clearly revealed the lesion compressing the spinal cord, which helped us formulate a therapeutic plan. We conclude that MR imaging is the most sensitive examination for detecting bone metastasis and is necessary for planning treatment.
