[Low molecular weight heparins].

Abstract

The apparition in the 80's of low molecular weight heparins (LMWHs) obtained by chemical or enzymatical splitting, modified the fields of prophylactic treatment of thromboembolic diseases. These drugs present on heparin numerous advantages: equal antithrombotic activity with a smaller bleeding risk, higher bioavailability, longer pharmacokinetic, which simplify their use. The definite mode of action remains unknown, but LMWH act at different steps like coagulation (by AT III), fibrinolysis, blood cells, endothelial cells. Further, like heparin, they can be neutralized by protamine. However the right, simple, specific biological assay which presents a good correlation with the antithrombotic activity, remained to be established.

Cancer and venous thromboembolism. Clinical implications of strong association.

Venous thromboembolism is a common occurrence inpatients with cancer. Among risk factors for thromboembolism are prolonged immobilization, especially during hospital stay, surgery, and chemotherapy with or without adjuvant hormone therapy. Although prophylaxis and treatment of thromboembolism in cancer patients encompass those agents that are commonly utilised in patients free from malignancy, there are many unique issues in cancer patients that often make their use more problematic. Low-molecular weight heparins are the cornerstone of prophylaxis and treatment of venous thromboembolism in cancer patients. Furthermore, they have the potential to prolong patients survival, at least in those with more favourable prognosis. Approximately 10% of patients with idiopathic venous thromboembolism harbour an underlying malignancy that can be detected by an extensive diagnostic work-up. However, whether screening for occult malignancies ultimately improves prognosis and prolongs survival remains to be demonstrated.

Thrombosis and cancer: the role of screening for occult cancer and recognizing the underlying biological mechanisms.

The association between cancer and thrombosis is well recognized. What is not known, however, is the exact relationship between these two common medical conditions. Although the development of venous thromboembolism (VTE) in a patient with known cancer is the most common presentation, in some patients, VTE may precede the diagnosis of malignancy by many months. The variation in clinical presentation is likely due to the heterogeneous biology of different tumor types and also reflects the limitations of detection or available diagnostic methods. Accumulating evidence now suggests that critical oncogenic events may also trigger activation of the coagulation cascade, leading to a prothrombotic environment that not only manifests as venous thromboembolic disease but also promotes the growth and progression of the malignancy. This chapter will review the evidence for screening for occult malignancy in patients presenting with unprovoked or idiopathic thrombosis, briefly outline the known biological relationships between malignancy and thrombosis, and summarize the clinical data on the potential anticancer effects of low molecular weight heparins (LMWHs).

Long-term therapy of venous thromboembolism in cancer patients.

Venous thromboembolism (VTE) is a common complication in cancer patients that results in significant morbidity and mortality. Long-term treatment options for cancer patients who experience VTE include vitamin K antagonists (VKAs), low molecular weight heparins (LMWHs), and inferior vena caval (IVC) filters. Cancer patients have a two- to fourfold higher risk for experiencing recurrent VTE and major bleeding during chronic VKA therapy than patients without malignancies. Recent randomized clinical trials have shown that LMWHs rather than oral VKAs are preferred for initial chronic treatment of VTE in patients with advanced cancer. One factor potentially limiting the broader use of LMWH for chronic therapy in the United States is its higher acquisition cost. Efficacy, cost, drug availability, patient comorbidities, and concomitant medications all need to be considered when selecting chronic VTE therapy. Cancer patients with VTE should be treated for as long as their disease is active to minimize the incidence of recurrence. Use of IVC filters should generally be reserved for patients at high risk for recurrent VTE who have contraindications to anticoagulation. Several new anticoagulants are being investigated that promise greater therapeutic choices and potentially better outcomes for cancer patients with VTE.
