[Technical problems on right lung transplantation].

Abstract

This report describes the technique of the allo-transplantation of the right lung of an adult mongrel dog. Compare to the transplantation of the left lung, we found several pitfalls in the right lung transplantation. 1) The intraatrial groove of the recipient must be divided carefully just at the middle of the right and the left atrium in order to make a generous cuff for the left atrial anastomosis. 2) It is preferable to divide azygos vein to perform easy anastomosis of right pulmonary artery and right bronchus. 3) Care must be taken to make left atrial cuff in donor lung since we did not divide intraatrial groove in the donor animal. It was necessary to make the left atrial cuff with a part of the right atrium attached.

Revival of the side-to-side approach for distal coronary anastomosis.

Side-to-side anastomosis was employed by just ten proportional stitches while performing distal anastomosis during coronary artery surgery. This technique is simple and quick. Here this simple technique is described in detail and the postoperative status of grafted conduits is reported.

Review article: Indeterminate colitis--definition, diagnosis and management.

BACKGROUND: The term indeterminate colitis is imprecise and without a generally accepted definition. Nevertheless, it is a term that is commonly used by gastroenterologists, pathologists and surgeons. AIM: To offer an opinion supported by published data, about the appropriate use of the term indeterminate colitis by addressing a series of questions. METHODS: A PubMed database search was performed using the keywords, 'colitis' and 'inflammatory bowel disease' each combined individually with the following adjectives: 'indeterminate', 'unclassified', 'undefined', 'undiagnosed' and 'non-specific'. RESULTS: There is no generally accepted definition of indeterminate colitis. All current applications of the term rely on exclusionary criteria and there is no confirmatory diagnostic test. Nevertheless, the diagnosis of indeterminate colitis appears to be durable in a subgroup of patients, suggesting that this group represents a unique phenotype. CONCLUSIONS: Indeterminate colitis has expanded from a strictly defined, postcolectomy pathological diagnosis to become, in addition, a clinical diagnosis without generally accepted criteria. A diagnosis of indeterminate colitis should be qualified with descriptors for the diagnostic criteria applied.

Acellular dermal matrix grafts for prevention of microarterial thrombophlebitis.

OBJECTIVE: To examine the role of acellular dermal matrix grafts for prevention of microarterial anastomotic thrombophlebitis. DESIGN: Bilateral femoral artery microvascular anastomoses were created in the field of established wounds infected with Staphylococcus aureus in 12 rats. In each animal, 1 femoral microarterial anastomosis was wrapped with an acellular dermal matrix graft, and the contralateral femoral anastomosis was left unprotected. The incidence of femoral artery thrombosis was determined after 4 days by wound reexploration. SETTING: David Geffen School of Medicine, University of California, Los Angeles. MAIN OUTCOME MEASURE: The patency of femoral artery anastomoses was determined after 4 days by wound reexploration. RESULTS: The incidence of femoral artery thrombosis in vessels wrapped with acellular dermal matrix grafts was 17%. The incidence of femoral artery thrombosis in unprotected vessels was 100%. This difference was statistically significant (P<.05). CONCLUSION: Acellular dermal matrix grafts seem to have a protective effect in the prevention of acute thrombophlebitis when arterial microvascular anastomoses are performed in infected surgical fields.
