[Localization technic and positioning methodology in afterloading therapy].

Abstract

For use in afterloading therapy there was constructed a special combination consisting of X-ray multiplier and a gynaecological table. Because of the constant magnification of radiographs topometric examinations became easier. Examining the displacement of points of interest when the patient is restored from a lithotomic position into a position with stretched legs we concluded that from the point of view of treatment planning it will be necessary to carry out afterloading irradiation with stretched legs like in the teletherapy. Only in such a way it is possible to calculate a common dose distribution of teletherapy and afterloading therapy.

Prospective, conservative management of localized prostate cancer.

BACKGROUND: Currently, no randomized trials of the various treatment options for localized prostate cancer have been reported. Therefore, radical prostatectomy and external irradiation are offered as the primary management options and are thought by many to be curative. METHODS: All patients in this series with a confirmed diagnosis of cancer of the prostate, localized, were given the option of expectant management in addition to the popular ones. Therefore, 233 patients with localized Stage A and B cancer were observed prospectively and conservatively for a period of 22 years, with a management plan of watchful waiting for most patients until signs and/or symptoms of disease activity occurred. In 44 patients interstitial I-125 irradiation implantation and pelvic lymphadenectomy were performed in those who received surgical intervention. RESULTS: No patient received external irradiation or radical prostatectomy in this series. The overall survival of patients was not statistically different from the United States life-tables probability for men of similar ages when the study ended for reporting purposes in June 1989. Exclusive of the 6 patients lost to follow-up, 52 patients survived less than 3 years, whereas 181 patients have survived more than 3 years. CONCLUSIONS: Because of the favorable results in this expectant management study, it appears prudent that all patients with localized prostate cancer should have this management option as well as radical surgery or external irradiation.

[Endobronchial intraluminal brachytherapy for patients with tracheobronchial carcinoma with high dose-rate 192-Iridium remote afterloading method].

Endobronchial intraluminal brachytherapy was conducted for 5 patients with tracheobronchial carcinoma using 192Iridium high dose-rate afterloading machine (microSelectron HDR) and guide tubes of 1.9 mm in outer diameter. Four patients with bronchial non-small cell carcinoma and one with tracheal adenocarcinoma were applied, and a dose of 6 Gy/Fr. at points 1 cm from the source once a week, and a cumulative dose of 18 Gy/3 Fr. was delivered. Guide tubes could be placed up to 3 tubes via the vocal airway, and new computer planning system could afford to calculate dose from independent lining of the source into optimization. This could be done with safety, and no remarkable acute side effects were observed attributable to this method.
