School and work release evaluations.

Abstract

Increasingly high levels of absenteeism for minor illnesses and injuries have created serious problems for American schools and industries. Short-term absenteeism also creates serious ethical and professional dilemmas for primary care physicians who routinely perform school and work release evaluations. Through an inductive analysis of three typical cases, these problems are identified. The major ethical issues involved are discussed, including truth-telling, confidentiality, and a confusion of the professional boundaries of medicine, education, and industry. Through a deductive analysis, it is shown that the conventional approaches to school and work release evaluations fail to achieve satisfactory results. To circumvent these problems, three solutions are proposed: primary care physicians can refuse to do such evaluations, inform the patient of the purpose and rules of the encounter, or perform evaluations only in the context of treatment.

Air pollution and school absenteeism among children in Mexico City.

To determine the acute effects of ozone exposure, the authors conducted a short follow-up study of respiratory illness in a population of 111 preschool children frequently exposed to ozone levels that regularly exceed 0.120 parts per million (ppm). The children attended a private kindergarten in the southwestern part of Mexico City. Parents completed a questionnaire on demographic data, medical history, and potential sources of indoor air pollution. To determine the relation of ozone and respiratory-related school absenteeism, the authors used a logistic regression model for longitudinal data. During the 3-month follow-up, 50% of the children had at least one respiratory-related absenteeism period, and 11.7% had two or more. Children exposed for 2 consecutive days to high ozone levels (> or = 0.13 ppm) had a 20% increment in the risk of respiratory illness. For children exposed for 2 consecutive days to a high ozone level and the previous day to low temperature (< or = 5.1 degrees C), the risk reached 40% (odds ratio = 1.44, 95% confidence interval 1.37-1.52). This study suggests that ozone exposure might be positively associated with the risk of respiratory illness in children and that it may have an interactive effect with low temperature exposure.

[The relationship between job appreciation and absenteeism studied with the use of a simple questionnaire].

In the period February 1986-January 1988 a questionnaire study was carried out among 455 men aged 39-55 years working for the Netherlands Post Office to determine whether a correlation exists between job appreciation and absenteeism. Job appreciation was estimated by means of seven questions concerning: pleasure at work, fatigue after work, job satisfaction, mental stress due to the work, tension in contact with colleagues, tension in contacts with superiors and the physical exertion of working. Answers were scored on a 7-point scale. It was found that a particular score area exists that is correlated with significantly more absenteeism. The results apply to groups, not to individuals.

Incubation period, severity of disease, and infecting dose: evidence from a Salmonella outbreak.

The associations between infecting dose, incubation period, and the severity of disease were examined in a large outbreak of Salmonella typhimurium which occurred at a medical conference in Wales in 1986. Persons who had eaten two or more pieces of the chicken vehicle had, on average, shorter geometric mean incubation periods than those who had only eaten one piece: 16.6 hours (95% confidence interval (CI) 13.5-20.5) compared with 20.7 hours (95% CI 19.0-22.6) (t = 1.97, p < 0.05). Incubation period was negatively correlated with the maximum frequency of diarrheal stools (r = -0.46, 95% CI -0.56 to -0.33), the maximum temperature reached (r = -0.34, 95% CI -0.50 to -0.16), the duration of symptoms (r = -0.41, 95% CI -0.53 to -0.26), and the amount of time taken off from work (r = -0.54, 95% CI -0.65 to -0.41). Those with shorter incubation periods were more likely to have been hospitalized. There was no association between chicken consumption and any of the measures of severity. The authors discuss the evidence that incubation period is inversely related to dose, the use of incubation period as a marker for dose, and the role that individual differences in susceptibility play in determining both the incubation period and the outcome.
