Cryptococcosis: a diagnostic challenge.

Abstract

Human cryptococcosis is an exogenous human infection of increasing frequency and growing importance. Early diagnosis is essential for the clinical outcome, no matter whether the infection is primary or secondary to underlying disease. Reliable and quick diagnostic methods are outlined and some pitfalls are discussed. Direct microscopy is essential, followed by cultivation procedures and serologic tests.

Comparison of selective culture and serologic agglutination of Treponema hyodysenteriae for diagnosis of swine dysentery.

Samples of faeces and of serum were collected from pigs of various ages on 21 farms. Faecal samples were cultured on trypticase soy agar containing 5 per cent citrated bovine blood and 400 micrograms per ml spectinomycin, incubated at 42 degrees C in Gaspak jars under an atmosphere of 80 per cent hydrogen: 20 per cent carbon dioxide. Antibody titres to Treponema hyodysenteriae were determined by a microtitration agglutination method using merthiolate-inactivated whole cell antigen prepared from a beta-haemolytic isolate. Results indicated that mean titres in pigs from which beta-haemolytic T hyodysenteriae was isolated were significantly higher than in pigs which yielded isolates of weak beta-haemolytic T innocens or in culturally negative pigs (P less than 0.0225). Mean titres of herds where beta-haemolytic T hyodysenteriae was isolated were significantly higher (P less than 0.005) than the mean titres of either of the other two groups. However, mean titres of herds where no isolates were obtained were not significantly different from mean titres of herds where weak beta-haemolytic T innocens was isolated.

[Existence of several non-specific recognition systems in macrophages].

Rat peritoneal cells were made to bind five particle species: immunoglobulin-coated Sheep red cells, glutaraldehyde-treated Sheep red cells, latex beads, leishmania and tumor cells. The dependence of binding on various physico-chemical parameters was studied. The binding of latex beads or Leishmania was not inhibited by cold (4 degrees C), sodium azide, cytochalasin B and ethyleneglycol or dimethylsulphoxide. The binding of immunoglobulin-coated Sheep red cells was unaffected by cold and azide, but it was inhibited by cytochalasin B, ethyleneglycol and dimethylsulphoxide. The binding of glutaraldehyde-treated Sheep red cells was inhibited by cold, azide and ethyleneglycol, but it resisted cytochalasin B and dimethylsulphoxide. The binding of tumor cells was inhibited by azide, cytochalasin B, ethyleneglycol and dimethylsulphoxide. It is concluded that: (a) macrophages are endowed several sets of non-specific binding structures that are differently affected by physico-chemical parameters, which provides a simple way of characterizing them; (b) the expression of a given binding structure on the macrophage membrane is modulated by metabolic inhibitors; (c) some lymphocytes were able to bind tumor cells or Leishmania. Thus, lymphocytes and macrophages might share some non-specific adhesive structures.
