Progress in cataract surgery using microsurgical technique.

Abstract

A microsurgical technique for cataract extraction is presented. The operations were performed with a corneal incision and a continuous nylon 10-0 suture. This technique is evaluated by the study of 1289 cases, consecutively operated on. The complication rate was very low. Haemorrhage in the anterior chamber was found in 1%. Delayed reformation of the anterior chamber occurred in only two cases. No cases of iris prolapse occurred. The intraocular pressure was not interferred with by the operation. Wound rupture following removal of the suture three months post-operatively occurred in 1%. The advantages of the corneal incision and continuous nylon suture are discussed (less irritation, good and secure wound closure).

[Results of cataract extraction and lens implantation].

98 patients were invited for a follow-up examination 2 years after cataract extraction. They then completed a questionnaire dealing with their daily functioning, comparing the situations prior to and 2 years after the operation, and were asked as to their satisfaction with the results of the operation. Statistically significant improvement was found in measurements of both near and distance vision and in performance of most aspects of daily functioning. Most patients were pleased with results of the operation.

Current uses of ophthalmic lasers.

Current laser treatments are quick, relatively painless, and well tolerated. Some ophthalmic techniques can be performed only by laser while others have a lower morbidity than alternative treatments. Peripheral retinal photocoagulation and focal photocoagulation now offer greatly improved visual prognosis for diabetic patients with proliferative diabetic retinopathy or diabetic macular disease. Selected cases of macular degeneration may be treated by focal laser photocoagulation. The role of lasers in treating sub-retinal neovascular membranes is limited by the extent and location of the membrane at presentation and the high risk of recurrence after treatment. Patients with distorted vision must be referred urgently for specialist ophthalmic assessment. Flat retinal holes and tears may be sealed by laser therapy, thus preventing retinal detachment. Short pulsed neodymium-YAG photodisruptive capsulotomy effectively clears the visual axis of thickened posterior lens capsule after cataract surgery. Short pulsed neodymium-YAG photodisruptive iridotomy may be used to treat and prevent angle closure glaucoma. Laser trabeculoplasty aids the control of open angle glaucoma. Research is continuing into the role of other lasers in managing open angle glaucoma and of photoablative lasers in treating refractive errors and superficial corneal disorders.
