Lymphocyte surface phenotypes in pernicious anemia.

Abstract

Because an immunological defect of an unknown nature is thought to be a factor in the pathogenesis of pernicious anemia, we studied lymphocyte surface phenotypes in 40 patients and compared them with 113 healthy controls. The only significant difference that emerged was in their slightly decreased number and proportion of surface immunoglobulin (lambda chain) -bearing cells. Of specific interest, the numbers of OKT4+ and OKT8+ lymphocytes and the T4+/T8+ ratios were not significantly different from control values. A sizable minority of patients had increased ratios, while a smaller number had decreased values. No explanation or identifying feature was apparent for those patients with either increased or decreased T4+/T8+ ratios. (One patient with a very low ratio, who was excluded from our analysis, developed acquired immune deficiency syndrome.) No differences were apparent in T4+/T8+ ratios or any other lymphocyte surface phenotypic characteristics when patients were segregated by presence of anti-intrinsic factor antibody or anti-parietal cell antibody, or by race, sex, or age. Our results in a racially heterogeneous group of patients do not support the suggestion that T4+/T8+ ratios are usually abnormal in prenicious anemia or that the presence of anti-intrinsic factor antibody is associated with such abnormality.

Murine experimental autoimmune gastritis models refractive to development of intrinsic factor autoantibodies, cobalamin deficiency and pernicious anemia.

Researchers have developed murine lymphopenic, non-lymphopenic, transgenic, spontaneous and infectious agent based models to induce an experimental autoimmune gastritis (EAG) for the study of human organ-specific autoimmune disease. These models result in a chronic inflammatory mononuclear cell infiltrate in the gastric mucosa, destruction of parietal and zymogenic cells with autoantibodies reactive to the gastric parietal cells and the gastric H+/K+ ATPase (ATP4), arguably hallmarks of a human autoimmune gastritis (AIG). In the case of AIG, it is well documented that, in addition to parietal cell antibodies being detected in up to 90% of patients, up to 70% have intrinsic factor antibodies with the later antibodies considered highly specific to patients with pernicious anemia. This is the first report specifically investigating the occurrence of intrinsic factor antibodies, cobalamin deficiency and pernicious anemia in EAG models. We conclude, in contrast to AIG, that, in the three EAG models examined, intrinsic factor is not selected as a critical autoantigen.

Treatment of solitary gastric carcinoid tumor by endoscopic polypectomy in a patient with pernicious anemia.

Type I gastric carcinoid tumors result from hypergastrinemia in 1%-7% of patients with pernicious anemia. We diagnosed pernicious anemia in a 48-year-old female patient with complaint of fatigue for three months. She had no gastrointestinal symptoms. Endoscopic examination ot the upper gastrointestinal tract revealed atrophic gastritis and a polypoid lesion in the corpus of 3-4 mm in size. Endoscopic polypectomy was performed. Histopathological examination of the specimen revealed positive chromogranin A and synaptophysin stainings compatible with the diagnosis of a carcinoid tumor. Serum gastrin level was increased, urinary 5-hydroxyindoleacetic acid was within the normal range. There was no other symptom, sign, or laboratory finding of a carcinoid syndrome in the patient. No metastasis was found with indium-111 octreotide scan, computed tomographies of abdomen and thorax. Type I gastric carcinoid tumors are only rarely solitary and patients with tumors < 1 cm in size may benefit from endoscopic polypectomy.
