Computer utilization for intravenous nutrition in surgical neonates preliminary report.

Abstract

A program was developed to store important numerical clinical data such as daily weights, intravenous caloric intake, and laboratory test results on surgical neonates who required total intravenous nutrition. Preliminary experience with the data from the first 5 surgical neonates requiring intravenous nutrition indicated that this data processing is easily achieved. Data retrieval by the development of output programs provided rapid access to stored records, rapid data tabulation, and graphic print-out of parameters, such as intravenous calories per kilogram per day and weight gain, that can be placed in the patient's record.

Infant growth modelling using a shape invariant model with random effects.

Models for infant growth have usually been based on parametric forms, commonly an exponential or similar model, which have been shown to fit poorly especially during the first year of life. An alternative approach is to use a non-parametric model, based on a shape invariant model (SIM), where a single function is transformed by shifting and scaling to fit each subject. In the model a regression spline is used as the function, with log transformation of the data and a simplification of the SIM, obtained from the relationship with the exponential model. All subjects are fitted as a nonlinear mixed effects model, allowing the variation in the parameters between subjects to be determined. Methods for the inclusion of covariates in growth models based on SIM are developed, with parameters for time independent covariates included in the model by varying either the shape, the size parameter or the growth parameter and time-dependent co-variates included by transforming the time axis, to either increase or decrease the growth rate dependent on the co-variate, similar to methods used for accelerated failure-time models. The model is used to fit weight data for 602 infants, measured from 0 to 2 years as part of the Childhood Asthma Prevention Study (CAPS) trial, and to determine the effect of breastfeeding on infant weight.

[Impact of nutrition management in patients with head and neck cancers treated with irradiation: is the nutritional intervention useful?].

AIMS: The head and neck tumors are most often associated with a precarious nutritional status. Radiotherapy increases the risk of denutrition because of its secondary effects on the secretory and sensorial mucous membranes. The purpose of our retrospectively study was to evaluate the interest of a precocious and regular nutritional therapy on the ability to maintain the nutritional status of the patient during the radiotherapy. PATIENTS AND METHODS: The fifty-two patients included in the survey have been classified retrospectively in two different groups based on their observance to the nutritional therapy: group 1 "good observance", group 2 "bad observance". RESULTS: The 31 patients of group 1 have lost an average of 1.9 kg by the end of the irradiation, whereas the 21 patients of group 2 have lost an average of 6.1 kg (p<0.001). The almost stability in weight of patients in group 1 was linked to a lower frequency of breaks in the radiotherapy (6 vs 33% p=0.03) and in a decrease in grade of inflammatory mucous membranes (10% of grade 3 in group 1 vs 52% in group 2, p=0.006). The quantity of calories ingested in form of nutritional supplements was greater in group 1 and consequently enabled patients to stabilized their weight (1200 calories in group 1 versus 850 calories in group 2, p<0.005). CONCLUSIONS: The given nutritional advice and the prescription of adapted nutritional supplements consequently allowed limiting efficiently the weight lost during the irradiation and the grade of mucositis. The systematization of a precocious nutritional therapy for patients irradiated for head and neck tumors seems absolutely essential.
