[Chronic chromate intoxication with renal tubular damage--report of a case].

A 46-year-old male chromium plating worker visited our hospital due to rhinorrhea, sneezing and cough with blood-tinged sputum for more than 10 years. He also had skin ulceration and chronic dermatitis on both hands Medical therapy was inefficient. Physical examinations revealed nasal septum perforation, severe inflammation of the nasopharynx cavity, and eczema of both hands. Laboratory investigations showed significant tubule proteinuria, enzymuria, hypercalciuria, etc. It is evident that renal tube damage was present in this patient. The blood chromium level was 25 ng/mL, and the 24-hour urine chromium excretion level was 2.8 mg/day. A pulmonary function test showed reduced functional residual capacity (FRC), which may be due to either long-term smoking or chromate acid exposure. To our knowledge this is the first case of renal tubal damage induced by chronic chromate intoxication Taiwan. Further evaluation of the occupational safety and health of chromium plating workers is needed on this island.

Evidence for the generation of hydroxyl radicals from a chromium(V) intermediate isolated from the reaction of chromate with glutathione.

The formation of hydroxyl radicals from a chromium(V) complex isolated from the reaction of glutathione with chromate has been demonstrated in spin trapping experiments using dimethylsulfoxide and 3,5-dibromo-4-nitrosobenzene sulfonate. Mechanisms for the formation of radicals in such systems are discussed. These results help to explain the ability of solutions containing chromate and glutathione to cause strand breaks in DNA.

Chromated steel and contact allergy. Recommendation concerning a "threshold limit value" for the release of hexavalent chromium.

Steel surfaces can be treated with zinc and chromates to prevent the steel from rusting. Cases of allergic contact dermatitis due to the release of hexavalent chromium (Cr64) from chromated surfaces have been reported. The main purpose of the present study was to develop a simple procedure for the determination of leachable Cr64 that could be used in industrial applications to check the quality of chromated products and to establish a "threshold limit value" for such products. Occlusive tests were performed in chromate-sensitive patients using chromated discs, and discs representing a release of 0.6 microgram/cm2 or more elicited positive results in all patients tested. It was proposed that the mean release of Cr64 from chromated parts should not exceed 0.3 microgram/cm2. Approximately 1 out of 4 yellow chromated parts, collected from a chromating plant and a car assembly plant, gave values above this limit. Today, several tests are performed to evaluate the technical quality of the chromate layers. It is suggested that the present method should be added to the test program in order to reduce the risk of causing chromate allergy.
