[New developments in pediatric surgery].

Abstract

New concepts in pediatric surgery have been developed concerning inter-disciplinary problems such as ureter implantation or urinary undiversion as well as deformities of abdominal organs such as biliary atresia, anorectal malformations, adrenogenital syndrome and sinus urogenitalis. Considerable improvements in fetal surgery as a part of pediatric surgery were achieved. For diagnostic, surgical and ethical reasons, however, these efforts are, for the time being, only of theoretical interest.

Anal atresia and abdominal wall defect as unusual symptoms in EEC syndrome.

In this report we document the concurrence of a large omphalocoele and anal atresia in a female newborn with EEC syndrome (Ectodermal dysplasia-Ectrodactyly-Cleft palate). Up to now, the association anal atresia-EEC syndrome has been noted in 4 other unpublished patients indicating that anal atresia may constitute another true manifestation of this autosomal dominant syndrome with variable expression.

An unusual case of meconium peritonitis associated with perforated hydrocolpos.

Meconium peritonitis results from intestinal wall perforation with meconium spillage into the peritoneum during intrauterine life. Anal atresia is associated with meconium peritonitis and is also associated with cloacal malformation. A female neonate presented with a distended abdomen, anal atresia, and meconium peritonitis as diagnosed by calcification in the peritoneal cavity on abdominal radiograph. Sonography and cystography demonstrated a cloacal malformation with perforation of the hydrocolpos and resultant spillage of meconium into the peritoneum. There was no evidence of further perforation of bowel at surgery. This unusual case of meconium peritonitis is reported.

Intraabdominal calcifications in the newborn: an unusual case with anorectal malformation and other anomalies.

Intraabdominal calcifications are an unusual radiographic finding in the newborn and can easily be misinterpreted as meconium peritonitis. The authors refer to a patient with anorectal malformation (ARM), intraluminal calcifications and other anomalies. Careful differentiation of the type of intraabdominal calcifications can suggest the possibility of other initially unsuspected anomalies.
