Institutional countertransference.

Abstract

The authors present the case of a patient with an institutional transference and point out the existence of institutional countertransference, which confounded his therapy and obscured his potential for change. Confronted with the patient's tradition in the institution, the new therapist had to overcome feelings of unimportance and therapeutic impotence to focus on the patient as a person in his own right. The authors discuss the signs of institutional countertransference and describe how it can seriously hinder clinical assessment of patients with institutional transferences.

Therapist sexual feelings in hypnotherapy: managing therapeutic boundaries in hypnotic work.

This article elaborates ways that using hypnosis may create special vulnerability for the clinician, not only to experiencing sexual feelings toward patients but also to becoming confused about the meaning of these feelings and their relevance to treatment, as well as about the maintenance of appropriate patient-clinician boundaries. Special qualities of the hypnotic experience and relationship likely to generate erotic feelings and impulses in patients and clinicians alike are addressed. A clinical case example illustrates many possible meanings of therapist sexual feelings and the impulses to avoidance or acting out they may provoke. Clinically appropriate and inappropriate ways of managing boundaries in the presence of sexual arousal and of using sexual feelings to deepen clinical understanding and direct treatment interventions are discussed.

[The ethics of psychoanalytic technique].

The ethic of psychoanalytic technique which goes back to Freud and emphasizes the importance of anonymity, abstinence, neutrality and the central role of interpretation is subjected to a critical examination. The author traces the changes that have taken place since Freud and proposes a new ethic of psychoanalytic technique. Proceeding from the theory of object relations, Treurniet stresses the symmetrical relationship between analyst and analysand permitting both to assume a "meta-position" in order to reflect on the analysis material. The author further suggests that, beyond the projections of the analysand, the analyst should be open to his own subjectivity, as this openness is the key to the essential feature of analytic procedure, the enactment of countertransference. Finally, Treurniet reformulates his advocacy of a non-intrusive, affirmative attitude on the part of the analyst, a spontaneous willingness to "fall into the analysand's trap", an ability to oscillate between acting-out and introspection, to live out countertransference involuntarily and finally to incorporate the non-ideal into his conscience. These rules of technique must be controlled not only by the conscience and the countertransference of the analyst, but also--apart from intervision and consultation with collegues--by the analysand himself, whose opinion of the analytic situation the analyst should ask for.

Countertransference in group psychotherapy: waking a sleeping dog.

This paper reviews and critiques the literature on countertransference (CT) in group psychotherapy. The literature review is organized within a framework that calls attention to the origins, triggers, manifestations, and effects of CT in groups, as well as CT management factors. An overriding critique is that the present literature lacks a research foundation of any kind. An argument is made for beginning research in this domain, and guidelines for doing so are presented.
