The timed Allen test.

Abstract

A technique for timing hand revascularization utilizing the Allen test was standardized in 800 hands of 400 control subjects. The average timed radial artery fill was 2.4 +/- 1.2 seconds. The average ulnar artery fill was 2.3 +/- 1.0 seconds. Seven per cent of ulnar arteries and 2% of radial arteries did not fill completely within 6 seconds. Ninety-one per cent of the hands tested were considered complete with uniform brisk refill. The clinical observation of inadequate or absent revascularization correlated with reduced digital blood pressure. For the test to be accurate and reproducible at least 11 pounds of digital pressure are necessary to uniformly occlude both arteries and reduce the digital blood pressure to 0 mm Hg.

Central venous pulse pressure analysis using an R-synchronized pressure measurement system.

OBJECTIVE: The information derived from central venous catheters is underused. We developed an EKG-R synchronization and averaging system to obtained distinct CVP waveforms and analyzed components of these. METHODS: Twenty-five paralyzed surgical patients undergoing CVP monitoring under mechanical ventilation were studied. CVP and EKG signals were analyzed employing our system, the mean CVP and CVP at end-diastole during expiration were compared, and CVP waveform components were measured using this system. RESULTS: CVP waveforms were clearly visualized in all patients. They showed the a peak to be 1.8+/- 0.7 mmHg, which was the highest of three peaks, and the x trough to be lower than the y trough (-1.6+/- 0.7 mmHg and -0.9+/- 0.5 mmHg, respectively), with a mean pulse pressure of 3.4 mmHg. The difference between the mean CVP and CVP at end-diastole during expiration was 0.58+/- 0.81 mmHg. CONCLUSIONS: The mean CVP can be used as an index of right ventricular preload in patients under mechanical ventilation with regular sinus rhythm. Our newly developed system is useful for clinical monitoring and for education in circulatory physiology.

Beyond inpatient and outpatient care: alternative model for hypertension management.

Hypertension is a major contributor to worldwide cardiovascular mortality, however, only one-third of patients with hypertension have their blood pressure treated to guideline recommended levels. To improve hypertension control, there may need to be a fundamental shift in care delivery, one that is population-based and simultaneously addresses patient, provider and system barriers. One potential approach is home-based disease management, based on the triad of home monitoring, team care, and patient self-care. Although there may be challenges to achieving the vision of home-based disease management, there are tremendous potential benefits of such an approach for reducing the global burden of cardiovascular disease.
