Reflex modulation: a hearing test for the difficult-to-test.

Abstract

This study was designed to determine the effectiveness of reflex modulation (RM) audiometry in assessing the hearing sensitivity of difficult-to-test subjects. RM describes the inhibitory effect of a tone upon a subsequently elicited eyeblink reflex. Five ENT subjects were referred to the University of Kansas audiology clinic because of medicolegal problems or doubts regarding their hearing status. After conventional pure tone and speech audiometry had produced equivocal results, the subjects were tested by RM. The study indicates that RM is effective as an objective test of hearing for these subjects. Since RM sensitivity is not frequently dependent, it has a potential role as an adjunct to the brainstem evoked response (BSER) for testing low frequencies.

Multi-channel evoked response detection using only phase information.

The phase consistency of contiguous segments of the electroencephalogram (EEG) has been used in the detection of evoked responses to rhythmic stimulation. One of such techniques is the component synchrony measure (CSM), which is often used since the threshold for the detection task is easily obtained based on the estimates of asymptotic sample distribution. In this work we investigated the appropriateness of such thresholds for practical number of segments (M). The performance of CSM was next evaluated by Monte Carlo simulations with different signal-to-noise ratios (SNR) and values of M, and the results, compared with those for the magnitude-squared coherence. A way of improving the detection with CSM was also proposed, by suggesting the estimation taking into account the mean phase angle of a set of N signals. This multivariate detector was evaluated in simulations and an illustration of the technique was also given with the EEG of 14 subjects during photic stimulation. In simulated signals with equal SNR, the detection rate with this multivariate measure increased with N. The application to EEG data lead to similar results in 70% of the subjects, which suggests that improvements might be expected when more signals are available to detect evoked responses in EEG.

Evidence for the different additivity of the temporal and frontal generators of mismatch negativity: a human auditory event-related potential study.

The auditory sensory-memory mechanisms in the human brain were investigated using the mismatch negativity (MMN) component of the event-related potential. MMNs were recorded to stimuli deviating from the repetitive standard stimuli simultaneously either in one or two features (frequency, intensity). If the processing of these two features is independent of each other, the MMN to the double deviant should equal the sum of the MMNs elicited by the corresponding single deviants. The double-deviant MMNs were found to be additive at the electrode sites below the Sylvian fissure but not at the frontal scalp areas. The results suggest that the temporal subcomponent of MMN is additive whereas the frontal is non-additive. The pattern of results was similar in ignore and attend conditions, suggesting that the components were not attentionally modulated.

A temporal window for the central inhibition of stuttering via exogenous speech signals in adults.

We explored a possible temporal window for central stuttering inhibition via exogenously presented speech signals. Thirteen adults who stutter were asked to read while listening to a continuous vowel /a/, or a repeating 1 s vowel /a/ followed by 1, 3 and 5 s silences. In all conditions, stuttering was significantly reduced. However, the continuous and 1 s repeating conditions showed the greatest reduction in stuttering relative to all other conditions. Furthermore, these conditions did not differ significantly from each other, suggesting a temporal window of at least 1 s for stuttering inhibition induced by a 1 s stimulus. We propose that exogenous speech signals provide an additional speech source that engages mirror neurons for 'on-line' stuttering inhibition during continous speech. Employing dual speech sources results in 'on-line' stuttering inhibition and continuous speech flow. In contrast, endogenous (single source) inhibitory techniques require speech flow to be interrupted and go 'off-line' to derive the mirror neuronal.
