Indirect composite restorations.

Abstract

The evolution of composite and adhesive technology has benefited dentistry significantly. The 1990s have brought us, for the first time in the history of our profession, the ability to restore tooth structure in a conservative and esthetic fashion. The ability to give consistently high-bond strengths to both enamel and dentin and the excellent physical properties of these restorations have made them a permanent part of restorative dentistry. This paper summarizes the authors' current opinion about indirect composite restorations, outlines indications for their use, and describes a clinical technique for their preparation and cementation.

Occlusion and stability of synthetic femoral canal plugs used in cemented hip arthroplasty.

The occlusion and stability of five synthetic plugs used to restrict the femoral canal prior to cemented arthroplasty was assessed. A model was employed consisting of a hollow wooden dowel to simulate the canal, with adapters fixed to both ends to accommodate cement insertion and pressurization, and to produce a closed distal cavity. Three different canal diameters within the range accommodated by the plug selected were employed to assess insertion force, distal pressure during both insertion and pressurization of acrylic cement, and plug migration and leakage for each device. There was a wide variation among plugs in the magnitudes of the force and distal pressure upon plug insertion. During cement pressurization, cement leakage and/or plug migration was noted in all trials, and either of these events resulted in pressure rises in the distal cavity. Most notably, differences of only 0.5 mm in the diameter of the canal resulted in marked changes in the occlusion and stability achieved. It is concluded that for the range of canal sizes anticipated intraoperatively, these synthetic plugs appear deficient to varying extents in their ability to occlude or remain stable in the intramedullary canal.

Temperature increase during removal of incisal extensions on resin-bonded castings.

To assist seating or relocation of resin-bonded retainers on abutments, extensions of the cast framework can be incorporated and removed after cementation. There has been apprehension concerning the heat created during the grinding of these extensions, particularly if there is a substantial bulk of metal. This study recorded the elevation of temperature when cast incisal extensions were removed by grinding after cementation. A total of 20 retainers were waxed for 0.3 mm thickness, cast in a nickel-chrome alloy, and bonded in-vitro to an acid-etched natural tooth with composite resin cement. The temperature of the coronal dentin was measured while the tag was ground with a high-speed dental handpiece. The maximum temperature and mean temperature elevations that exceeded the resting temperature were recorded. A significant temperature rise was apparent when complete incisal wrapover framework extensions were reduced compared with the removal of fingerlike locating extensions. In conclusion, care should be exercised when the locating extensions of resin-bonded retainers are removed with high-speed dental handpieces.
