[Bladder compression caused by hip prosthesis displacement].

Abstract

Case report of one patient with extrinsic vesical compression secondary to a displaced hip prosthesis. Interesting case due to increasing hip prosthesis surgery over the last few years involving, although not frequently, ureteral and vesical injuries of varying consideration which require early diagnosis and management.

In vitro release studies of methylmethacrylate liberation from acrylic cement powder.

Bone cement or polymethylmethacrylate (PMMA) is commonly used for anchoring cemented prosthesis to the bone. Cytotoxic effect of culture media exposed to PMMA powder may be related with long term problems associated with acrylic cement application, being the monomer (methylmethacrylate) one of the cement's component partly responsible for the cytotoxic effect. The present work reports the studies of monomer release from acrylic bone cement powder under different experimental conditions: setting time of PMMA (in solution and air) and different culture media composition. High-performance liquid chromatography was used for the determination of residual monomer. Mathematical models were applied to experimental dissolution data revealing that monomer release is lightly affected by the studied variables. The monomer release seems to be a surface phenomena, suggesting that the possible actions of monomer will mainly be due to the initial loss of non polymerized monomer rather than to further depolymerization of the already polymerized cement.

Use of alpha-tricalcium phosphate (TCP) as powders and as an aqueous dispersion to modify processing, microstructure, and mechanical properties of polymethylmethacrylate (PMMA) bone cements and to produce bone-substitute compounds.

Addition of tricalcium phosphate (alpha-TCP) powders as an aqueous dispersion to a polymethylmethacrylate (PMMA) bone cement is shown to produce a class of composites that due to their microstructure and mechanical properties may be suitable for application as bone substitutes. The PMMA forms a solid cellular matrix with open cells about 100 micrometer in size and incorporating TCP clusters. The TCP aggregates inside the cells form a porous network, with average mesopore diameters of about 0.1 micrometer, that is accessible from the outer surface. If TCP is added to PMMA in the form of dried powders, the composites are not applicable as bone substitutes. The dynamic elastic modulus (DEM) and compressive and tensile strengths were measured and discussed for both classes of composites. The mechanical properties of the bone-substitute composites, although lower than the other class of composites, are still competitive with those properties of a porous ceramic matrix of hydroxyapatite and with those of natural bones.

The effect of a silane coupling agent on the bond strength of bone cement and cobalt-chrome alloy.

Debonding of the cement-implant interface has been hypothesized to be the leading initial indicator of failed total hip prostheses. Many attempts have been made to increase the bond strength of this interface by precoating the implant, increasing the implant's surface roughness, and creating macro-grooves or channels on the implant. However, each of these approaches introduces new complications. This study introduces a unique silane coupling agent used to chemically bond the bone cement to the implant. Cylindrical cobalt-chrome samples were treated with the silane coupling agent, bonded to polymethylmethacrylate, and pushed out to failure. The mean shear strengths were compared to the failure strengths of untreated samples. Half of the specimens were tested immediately following cement curing, and the other half were tested after immersion in saline solution for 60 days. The mean shear strength of the silane-coated samples ranged from 18.2 to 24.1 MPa, and the mean shear strength of the uncoated samples ranged from 7.6 to 15.0 MPa. The increase in strength following silane coating noted in this study may increase the longevity of the implant by decreasing debonding at the interface and, therefore, subsequent failure due to loosening.
