[Insufflators for endoscopy].

Abstract

Operative laparoscopy needs a space to be made inside the abdominal cavity. This can only be achieved by suspension or by positive pressure. The endoscopic insufflator makes it possible to create this positive pressure, and does indeed enable the operating space to remain open. But it would be too limiting to consider these devices from the visibility point of view only, for they can also be exploited for their action on hemostasis and dissection. With this in mind we feel that the endoscopic surgeon ought to be perfectly familiar with his apparatus and not simply limit himself to adjusting the pressure and flowrate at the beginning of the operation. This hyper pressure in the abdomen is not without consequences for the patients' homeostasis and can indeed require an operation to be halted or converted to laparotomy. So knowledge of the biomedical aspects of his equipment will also enable the surgeon to increase the safety of his operations.

Endoscopy under operating microscope. A new device.

Existing technical conditions make it difficult for a single surgeon to simultaneously observe the endoscope and the observation field of the operating microscope. Video endoscopy is not adapted, because it respects neither the constraints of microsurgery (the surgeon has to stop observing the binoculars of the microscope to watch the video screen) nor the micromanipulations necessary for the specialties using micro endoscopy. A new device is described that closely joins endoscope and microscope. This coupling device allows the surgeon to permanently control and alternate the images according to his choice. The surgeon is able to observe in the binoculars the images transmitted by the endoscope and/or the operating microscope. This device respects the requirements of the microscope and, thanks to its optical capabilities, the endoscopic image is improved. The professions requiring double observation will improve their ability in diagnosis and therapy.

Atraumatic retractor for endoscopic surgery.

A novel retractor has been designed to achieve atraumatic retraction of the liver and displacement of hollow viscera during endoscopic surgery. The functional section of the instrument, when locked, forms a silicon-covered uniplanar hook which can be deployed for horizontal lift of solid organs such as the liver or be used to engage bowel for displacement from the operative field. Following laboratory testing, the instrument has been evaluated clinically and its ease of deployment and atraumatic nature have been confirmed. It has been found to be particularly effective for elevation of the thick fatty left lobe of the liver during antireflux surgery.
