Abstract

The public has to be educated in the peculiarities of medical jurisprudence. Mass disasters play into the hands of the press. A sensationalized media interest may exacerbate grief to the bereaved families and detract from informed disclosure of a legitimate coronial investigation.

Forensic autopsy costs in the city of São Paulo.

CONTEXT: Modern medical practice involves cost analysis of therapeutic and diagnostic procedures. There are no papers dealing with this theme in relation to forensic autopsies in our country. OBJECTIVE: Analysis of direct costs of forensic autopsies. TYPE OF STUDY: Cost analysis. SETTING: São Paulo Medical Examiner's Central Office. SAMPLE: Year 2001 activity. PROCEDURES: Routine forensic autopsies. MEAN MEASUREMENTS: Analysis of direct costs of personnel and material. RESULTS: Cost of personnel represents 90.38% or US$ 93.46. Material expenses comprised 9.62% or US$ 9.95. Total costs were calculated to be US$ 103.41. CONCLUSIONS: Forensic autopsies have a high cost. Cases to be autopsied should be judiciously selected. Our results are similar to international studies if data are rearranged based on the number of annual necropsies.

[Researchers show growing interest in antenatal mortality. Increased understanding of intrauterine fetal death may reduce the number of cases in the long run].

The incidence of intrauterine fetal death (from the 28th gestational week) is reported to be 3.6/1000 births in Sweden. Continuous improvements in perinatal care have resulted in a dramatic decrease in early neonatal mortality during the last few decades, but no comparable reduction of antenatal mortality has been observed. A better knowledge of the etiology of stillbirth is imperative to achieve a further decrease in the perinatal mortality rate. Advanced maternal age, smoking during pregnancy, maternal overweight/obesity and low socioeconomic class has been demonstrated to increase the risk for stillbirth. Several studies have been published concerning specific causes of intrauterine fetal death. Abruption of the placenta, some infectious disorders, fetal chromosomal aberration/malformation and maternal disease such as diabetes are some of the conditions that are associated to stillbirth. In order to arrive at a potential cause, a number of investigations have been recommended in the diagnostic evaluation of IUFD.
