Methicillin-resistant Staphylococcus aureus. Optimization of the agar screen plate method.

Abstract

Swab inoculation of oxacillin agar screen plates was compared with drop inoculation for detection of methicillin-resistant Staphylococcus aureus. The poor sensitivity of the swab method (59%) was related to heteroresistance of the S. aureus isolates. We recommend the drop method (100% sensitivity) because interpretation of results was significantly easier, making it more reliable.

Evaluation of the Vitek 2 system for identification and antimicrobial susceptibility testing of Staphylococcus spp.

The Vitek 2 system was assessed against reference methods with 197 methicillin-resistant Staphylococcus aureus from Belgian hospitals and 121 clinically significant blood culture isolates of Staphylococcus spp. Vitek 2 identified 95% of staphylococcal isolates correctly, detected oxacillin resistance with a sensitivity/specificity of 99/96%, and showed acceptable accuracy for susceptibility testing of five of eight other evaluable antibiotics. The median time for reporting results was 2 h 45 min for identification and 7 h for susceptibility tests.

Prevalence of the ST239 clone of methicillin-resistant Staphylococcus aureus and differences in antimicrobial susceptibilities of ST239 and ST5 clones identified in a Korean hospital.

A total of 188 nonduplicate methicillin-resistant Staphylococcus aureus (MRSA) isolates obtained between 2001 and 2004 in a university hospital in Daegu, Korea, were analyzed for their clonal types by molecular typing techniques, including multilocus sequence typing, spaA typing, staphylococcal chromosomal cassette mec (SCCmec) typing, and pulsed-field gel electrophoresis (PFGE). They were examined for their antimicrobial susceptibilities. The majority (87%) of MRSA isolates belonged to sequence type 239 (ST239; n = 100; 53%) and ST5 (n = 63, 34%) on the basis of sequence typing. MRSA isolates belonging to ST239 were genotypically homogeneous, while those belonging to ST5 showed variations in spaA type, SCCmec type, and PFGE patterns. The rates of resistance of the MRSA isolates belonging to ST239 to trimethoprim, sulfamethoxazole, tobramycin, gentamicin, erythromycin, and tetracycline were significantly higher than those of the isolates belonging to ST5 (P < 0.05). This study demonstrated that the ST239 clone, while rarely detected in Korea, was prevalent and that the antimicrobial susceptibility of the ST239 clone was significantly different from that of the ST5 clone.

[Mechanisms of methicillin and vancomycin resistance in Staphylococcus aureus].

The mechanism of methicillin and vancomycin resistance in Staphylococcus aureus (MRSA) is discussed, focusing on the phenotypic expression of resistance; i.e., pre-methicillin resistance, Egle-type resistance, heterogeneous and homogeneous resistance. Acquisition of staphylococcus cassette chromosome(SCCmec) by a recipient MSSA strain in the mecA-positive, and methicillin-susceptible status(pre-MRSA status) of S. aureus. Subsequent of the mecI-gene-mediated repression of mecA gene transcription results in the heterogeneous expression of methicillin resistance. It is only with the alternation of another chromosomal locus that the heterogeneous-to-homogeneous (hetero-to-homo) conversion of methicillin resistance occurs. Acquisition of a part of vancomycin-resistance(the level of hetero-VRSA) is associated with hetero-to-homo conversion of methicillin resistance, and full expression of vancomycin resistance(MIC = 8 mg/l) is achieved by additional acceleration of cell-wall synthesis causing apparent cell-wall thickening.
