Abstract

The purpose of this study was to examine at ultrastructural level the neural changes in human apical pulps of teeth clinically diagnosed as having ulcerative or hyperplastic pulpitis. Seventeen specimens were prepared from six teeth with ulcerative or hyperplastic pulpitis and four specimens from two sound teeth. After routine procedures the specimens were examined under an electron microscope for changes in the structure of myelinated and unmyelinated nerve fibres. Although the teeth used in this study responded positively to vitality test, the results revealed that moderate to severe degenerative changes occurred in the nerve fibres of the apical pulps of the teeth showing these two diseases.

Instant assessment of pulpal blood flow after orthodontic force application.

The purpose of the present investigation was to measure the effect of tooth intrusion and extrusion on pulpal blood flow in man. The flow changes were measured in 10 lateral incisors (6 subjects) by means of laser Doppler flowmetry (Perimed, Sweden) applied on the buccal surface through metal tubes bonded to the teeth. Brackets were bonded onto teeth 13 and 23, and intrusive and extrusive forces of 2 N were applied in random order to the experimental tooth for 5 min before unloading. The contralateral tooth served as a control. Extrusion of the teeth gave no significant changes in pulpal blood flow during loading or unloading. However, intrusion of the teeth reduced the pulpal blood flow by 20% during the first minute after force application. The pulpal blood flow gradually increased towards the preloading flow values for the next 4 min, and was returned to the prestimulus level 3 min after unloading. In conclusion orthodontic intrusion of teeth with a 2 N force evoked a temporary reduction in the pulpal blood flow, whereas extrusion had no effect on the pulpal blood flow.

Environmental effects on laser Doppler pulpal blood-flow measurements in man.

The increasing number of experiments using laser Doppler flowmetry in man for pulpal blood-flow recordings leads to questioning of the experimental recording conditions. The present study focused on three points: the design of the laser probe holder, the isolation of the tooth, and the influence of the recording site. A rigid polyurethane splint used in addition to different isolation devices (cotton roll, metal shield, rubber dam) was compared with a silicone splint. The silicone resulted in significantly higher values (+341%) than the polyurethane splint. The combination of the polyurethane splint with isolation devices decreased, in all cases, the flux values. The polyurethane/rubber-dam combination was the most efficient in individualizing the pulpal blood flow (-69% decrease). Recordings on non-vital teeth confirmed the hypothesis that there was periodontal contamination of the recorded flow, as the signal was abolished when using the polyurethane/rubber-dam combination. Cervical recording sites gave significantly higher values than occlusal sites (+42%). It is concluded that, in man, the part played by the periodontium may have been underestimated in previous recordings of pulpal blood flow. The use of a rubber dam in combination with a rigid splint to enhance the validity of recordings is proposed.

Detection of pulpal circulation in vitro by pulse oximetry.

Evaluation of pulse oximetry as a potential method of determining pulp vitality was the subject of this research. An in vitro model of pulpal circulation was fabricated to test the design for a dental pulse oximetry sensor. Blood samples equilibrated with hypoxic gas mixtures were circulated through the model by a peristaltic pump. A pulse was simulated by introduction of gas bubbles into the blood circulation. Pulse oximeter readings for saturation were recorded and compared with blood gas analysis results. Statistical analysis revealed no difference between pulse oximetry and blood gas analysis with a highly significant correlation coefficient. Clinical evaluation of this application is currently in progress.
