Cuff expander eases enema tip assembly.

Abstract

Some barium enema kits are manufactured with two-part enema tips, enabling a cuff to be placed on the tip only if required. Difficulty can be experienced during enema tip assembly while attempting to place the cuff on the tip. A readily available gadget used in the farming sector provides a neat and cost-effective solution to this problem.

[Paraclinical studies of endometriosis].

Investigations are performed in women with external endometriosis to confirm the diagnosis and to evaluate extension of the disease before treatment. Elevated serum CA 125 level is correlated with the severity of the disease. CA 125 may be a helpful to assess the efficacy of the treatment or to detect recurrences. Imaging diagnosis of endometriosis have a good sensitivity to detect cysts and nodes forms of the disease. Magnetic resonance imaging is more sensitive than ultrasonography to detect small nodular lesion and is able to made the diagnosis of deeply infiltrating endometriosis (sometimes not visualised by laparoscopy). Rectal endosonography, barium enema or excretion urography can be usefull if an involvement of the rectosigmoid or the bladder is suspected. Hysterosalpingography is an integral part of the initial fertility survey, but shows only indirect signs of endometriosis. Laparoscopy is the definitive diagnosis procedure for endometriosis, and permit to classify the lesions and to draw-up a therapeutic strategy.

Is barium enema reduction safe and effective in patients with a long duration of intussusception?

The aim of this study was to determine whether barium enema reduction (BER) is safe and effective in patients with a long duration of intussusception. Over the last 17 years, 104 patients were admitted to our hospital with a diagnosis of intussusception. All except 1 with peritonitis underwent BER primarily. Of the 103 intussusceptions treated primarily by BER, 84 (82%) were reduced by the enema alone, whereas 19 (18%) underwent surgical reduction. There were no differences in mean duration of disease between the patients with successful and failed enema reduction (successful: 15 +/- 14 h; failures: 14 +/- 11 h, P = 0.6). The success rate of BER was 85% within 12 h of symptoms, 76% for 12-24 h, and 71% for more than 24 h. Of 8 cases with a second trial, 4 (50%) were reduced by repeated barium enema. There were no deaths and no intestinal perforations. The success rate of more than 70% even in patients with a long duration of intussusception suggests that BER is safe and effective regardless of the duration of the disease.

Laparoscopic repositioning of an adjustable silicone gastric band for pouch dilatation and stoma obstruction.

Pouch dilatation with stoma obstruction is a well-known late complication after adjustable gastric banding operations for morbid obesity. Surgical treatment of this problem usually results in removal of the band, with or without replacement by another, or in repositioning of the band via laparotomy. We present the case of a patient with late pouch dilatation and stoma obstruction after placement of a Laparoscopic Adjustable Gastric Banding system (LAGB-Bioenterics) and in whom the adjustable band was laparoscopically opened, disconnected from the access port, and repositioned more proximally on the stomach. The postoperative course was uneventful. A postoperative radiographic contrast examination showed a correct repositioning of the band. The case demonstrates that the LAGB can be successfully opened and repositioned by a minimal invasive procedure. This is the first time to our knowledge that such a procedure has been reported.
