Scuba diving accidents.

Abstract

The principal scuba diving medical problems of barotrauma, air embolism and decompression sickness have as their pathophysiologic basis the Ideal Gas Law and Boyle's Law. Hyperbaric chamber recompression therapy is the only definitive treatment of air embolism and decompression sickness. However, with a basic knowledge of diving medicine, the family physician can provide effective supportive care to the patient prior to initiation of hyperbaric therapy.

Unintentional deaths from carbon monoxide poisoning--Michigan, 1987-1989.

Deaths from unintentional carbon monoxide (CO) poisoning in the United States result primarily from exposure to motor-vehicle exhaust and occur more often during the cold months of the year and in northern and midwestern states (1-3). In Michigan, from 1987 through 1989, 103 deaths were related to unintentional CO poisoning. To identify approaches for prevention of unintentional CO poisoning in Michigan, the Michigan Council on Injury Control (MCIC) studied death records and medical examiner (ME) records to determine the manner of deaths related to unintentional CO poisoning in that state from 1987 through 1989. This report summarizes findings of the investigation.

Cardiotoxicity after accidental herb-induced aconite poisoning.

Aconitine and its related alkaloids are known cardiotoxins with no therapeutic role in modern western medicine. The rootstocks of Aconitum plants, which contain aconite alkaloids, have been common components of Chinese herbal recipes. We have documented life-threatening intoxication in 17 Chinese subjects after accidental herb-induced aconite poisoning. All patients developed symptoms of aconite toxicity within 2 h of herb ingestion. Most developed tachyarrhythmias, including ventricular tachycardia and fibrillation from which 2 patients died. Toxicological evaluation revealed that aconites from the Aconitum rootstocks were the only plausible casual factor for intoxication. These cases point to the need for strict surveillance of herbal substances with low safety margins.
