[Relation between the Toynbee phenomenon and tube dysfunction].

Abstract

The effect of the Toynbee phenomenon in patients with nasal packing was studied. Nasal packing was used in 35 patients admitted for surgical correction of septal deviation. Middle ear pressure was measured during nasal packing and the results were compared with nasopharyngeal pressure gradients recorded during the Toynbee maneuver. The aim of the study was to demonstrate that one of the main causes of tube dysfunction during nasal obstruction is the Toynbee phenomenon.

[Dynamics of paranasal sinus and tympanic membrane changes in prolongely intubated patients].

67 prolongely intubated patients from intensive care unit were under our observation. Were performed ultrasound examination and CT scan of paranasal sinuses in prolongely intubated patients. In the same patients it was performed the examination of tympanic cavity by acoustic impendansometry. Also was performed bacteriologic examination of nasal discharge in prolongely intubated patients. Our investigations showed, that in prolongely intubated patients there is a high risk of development of polysinusitis and evstachitis, which can lead to so called fever of unknown origin and even to chronic sepsis. According to the authors, sanitation of nasal cavity must be performed every day at least 1 time, and if there is the shade--at least 2 times every day. To create the negative pressure, it is rational to use iamic--catheter and, through which activation of mucocilliary transport is performed. Changing of nasogastral probe is rational after one day by turns in each nasal pass. It is rational to cleanse places of pressing of these probes with the antiseptic solutions.

Eustachian Tube↗

[Function tests for the Eustachian tube. Current knowledge].

The Eustachian tube regulates the homeostasis of the middle ear. Problems with its function are predominantly found in childhood. As a consequence, otitis and hearing impairment occur. The most important muscle is the tensor tympani muscle. The complexity of the functional anatomy and physiology are reasons why no function test alone is capable of fulfilling all diagnostic needs. The predictive value of the various Eustachian tube function tests for the outcome of a tympanoplasty is not yet clear. An overview of clinically and scientifically relevant tests for the Eustachian tube function is provided.
