Abstract

The community and the scientific world are developing a great interest in phytoestrogens. This article reviews the current literature regarding the effects of phytoestrogen consumption in humans in relation to menopause, cardiovascular disease and cancer. Studies that adhere to scientific scrutiny such as randomised double blind studies are assessed.

Polycystic ovary syndrome: the new millenium.

Our understanding of Polycystic Ovary Syndrome (PCOS) has been hampered by varying diagnostic criteria, and ignorance of the etiology of the syndrome. PCOS women are uniquely insulin resistant and obesity aggravates this underlying predisposition to insulin resistance. Diagnostic criteria which focus on hyperandrogenism and/or menstrual irregularity are more likely to identify insulin resistant women, than such criteria as abnormal gonadotropin secretion or ovarian morphology. The lack of a clear etiologic mechanism to the syndrome has led to a multitude of symptom-oriented treatments with few therapies improving all aspects of the endocrine syndrome of PCOS. Improving insulin sensitivity has become established as a baseline treatment strategy in PCOS. There are, however, few randomized controlled trials of adequate power to provide an evidence based guide to treatment in PCOS.

Simulation study of the effect of the early mortality exclusion on confounding of the exposure-mortality relation by preexisting disease.

The authors conducted a simulation study to evaluate whether exclusion of the early mortality (deaths occurring during a prespecified period immediately after baseline) reduces confounding of the exposure-mortality relation by preexisting disease. The simulation specified an exposure that decreased mortality risk in the absence of confounding and then introduced confounding by preexisting disease that biased the "true" protective effect of exposure towards greater risk. In 2,000 cohorts, exclusion of the early mortality (deaths occurring during the first 25 months of a 60-month follow-up period) did not alter the mean hazard ratio for exposure under conditions of confounding by preexisting disease that produced a constant, threefold increase in mortality risk during follow-up (the mean hazard ratio was 1.72 for all subjects and 1.72 after exclusion of the early mortality). However, when the authors specified confounding by preexisting disease which produced a threefold increase in mortality risk that attenuated over time, exclusion of the early mortality consistently identified the "true" protective effect of exposure (the mean hazard ratio was 1.07 for all subjects and 0.31 after exclusion of the early mortality). Thus, under conditions of confounding by preexisting disease which produces an increase in mortality risk that attenuates over time--an effect that does have empirical support-the early mortality exclusion can be very effective in revealing the "true" exposure-mortality relation.

Community-based noncommunicable disease interventions: lessons from developed countries for developing ones.

Community-based programmes for prevention and control of cardiovascular diseases (CVD) started in Europe and the USA in the early 1970s. High mortality from CVD in Finland led to the start of the North Karelia Project. Since then, a vast amount of scientific literature has accumulated to present results and discuss experience. The results indicate that heart health programmes have a high degree of generalizability, are cost-effective and can influence health policy. In the 1980s the focus of programmes expanded from CVD to noncommunicable diseases (NCD), mainly because of the common risk factors. Attention has now turned to promoting this approach in developing countries, where the prevalence of NCD is growing. Theory and experience show that community-based NCD programmes should be planned, run and evaluated according to clear principles and rules, collaborate with all sectors of the community, and maintain close contact with the national authorities. In view of the burden of disease they represent and of globalization, there is a great need for international collaboration. Practical networks with common guidelines but adaptable to local cultures in a flexible way have proved to be very useful.
