[Comparative characteristics of stress tests--bicycle ergometry test and psycho-emotional test--in patients with ischemic heart disease].

AIM: To compare efficacy bicycle exercise (BE) and psychoemotional tests (PET) in provoking myocardial ischemia and arrhythmia in patients with acute and chronic forms of ischemic heart disease (IHD). MATERIAL AND METHODS: BE and PET (mathematical count test) were performed in 108 IHD patients. 30 patients had stable effort angina (FC II-III), 48 patients had progressing effort angina and 30 patients were at the subacute stage of myocardial infarction. RESULTS: PET was less efficient in detection of myocardial ischemia but is more adequate in detecting episodes of painless ischemia and ventricular rhythm disorders compared to BE. The greatest differences in efficiency of the tests occurred in patients with acute forms of IHD. CONCLUSION: Application of different stress tests in IHD patients raises efficacy of detecting episodes of painful, painless myocardial ischemia and rhythm disorders, specifies mechanisms of coronary failure development.

[Normative data for the MMSE, the Benton visual retention test, the Isaacs's set test, the digit symbol substitution test and the Zazzo's cancellation task in subjects over the age 70: results from the PAQUID Study].

UNLABELLED: Introduction. The aim of this study was to produce norms for the Mini-Mental State Examination, the Benton visual retention test, Isaacs's set test, the digit symbol substitution test and Zazzo's cancellation task (short 8-line version), in elderly people. METHODS: Data was collected in a representative sample of 1780 subjects aged 70 years and older, followed-up for five years after inclusion in the PAQUID study. Only individuals who did not have characteristics likely to alter their cognitive performance were considered. RESULTS: Descriptive statistics (median, tenth percentile, first and third quartile) were used to define gender-, age- and education-specific norms. CONCLUSIONS: These norms can be used by clinicians to interpret a patient's performance on several widely used cognitive tests according to the scores obtained by individuals of the same age, gender and educational level.

To test or not to test: an ethical conflict with presymptomatic testing of individuals at 25% risk for Huntington's disorder.

The first presymptomatic test for Huntington's disease was developed in the 1980s. With the detection of the gene causing the disorder in 1993, it became possible to do direct mutation tests with almost 100% sensitivity and specificity. The author discusses some of the ethical issues that arise when an adult child at 25% risk for the disease wishes to have the test, but the parent(s) at 50% risk refuses to have one. If the child tests positive, the genetic status of the parent will also be disclosed. No matter what course of action is chosen in this situation, the ethically legitimate interests of either child or parent might be violated. The author examines different alternatives and suggests a solution that might be acceptable to all parties.

Practice parameter--the lupus erythematosus cell test. An obsolete test now superseded by definitive immunologic tests.

The lupus erythematosus (LE) cell test (synonyms: LE prep, LE phenomenon; CPT No. 85544, LE Cell Prep) is a diagnostic test for systemic lupus erythematosus (SLE) that is based on an in vitro immunologic reaction between the patient's autoantibodies to nuclear antigens and damaged nuclei in the testing medium. It is subject to numerous experimental variables and dependent on subjective interpretation. It should be abandoned in favor of more definitive, quantitative immunologic tests for this condition.

Testing platelet activation with a shear-dependent platelet function test versus aggregation-based tests: relevance for monitoring long-term glycoprotein IIb/IIIa inhibition.

BACKGROUND: Tests developed to monitor glycoprotein (GP) IIb/IIIa blockade do not properly reflect platelet function in vivo and need a baseline (pretreatment) value. Because GP IIb/IIIa is essential in platelet aggregation and thrombosis under shear conditions, a flow-dependent approach to monitor its inhibition can be used. METHODS AND RESULTS: We compared a test based on flow-dependent platelet deposition, the Cone and Platelet Analyzer (CPA), with in vitro platelet aggregometry and the Rapid Platelet Function Assay (RPFA) on platelet function after GP IIb/IIIa inhibition. In vitro, increasing concentrations of abciximab (0% to 100% receptor occupancy) were tested. Ex vivo, platelet function was monitored with the CPA and with aggregometry for up to 1 week after abciximab administration. The CPA was better correlated with the percentage of free GP IIb/IIIa receptors than was aggregometry or the RPFA. Only the RPFA, when expressed as a ratio over baseline (pretreatment), was comparable to the CPA. Ex vivo, the CPA, but not aggregometry, showed prolonged platelet inhibition with gradual recovery from GP IIb/IIIa receptor blockade in the first week after abciximab administration. CONCLUSIONS: Platelet function assessment by shear-induced deposition is a reliable test to monitor a wide range of GP IIb/IIIa inhibition. Its accuracy does not require a baseline reference. The effects of GP IIb/IIIa blockade on platelet function should be examined under high shear conditions.

Evaluation of immunobead test (IBT), tray-agglutination test (TAT), and sperm immobilization test (SIT).

The present investigation relates the results obtained by the indirect immunobead test (IBT) to those found during the tray-agglutination test (TAT) and sperm immobilization test (SIT) performed on 16 positive blood sera samples. An IB binding of greater than 50% can be regarded as significantly positive (p less than 0.05), since 77% of the positive IB samples with an IB binding of greater than 50% also revealed a TAT titer of greater than 1:8. The IBT is an excellent test for the detection of sperm antibodies during routine screening procedures.

[Evaluation of the E-test: comparison of E-test and microbroth dilution method in azole antifungal susceptibility test].

The susceptibilities of clinical isolates of Candida spp. to fluconazole (FLCZ), itraconazole (ITCZ) and ketoconazole (KCZ) were determined by a new method, E-test (AB BIODISK, Sweden) and compared with those obtained by the microbroth dilution method. The MIC values of the 3 azoles obtained by the two methods were, in general, in good agreement. While MICs of FLCZ and KCZ demonstrated higher essential agreement rates and better correlation by both methods, MIC values of ITCZ by the E-test had a tendency to be higher than those of the microbroth dilution method in azole resistant strains. Potential usability of E-test as a broth dilution method for susceptibility tests for clinically isolated Candida spp. including azole resistant strains was discussed.

[Uprighting test, deep respiration test and Valsalva test in patients with diabetes type 1].

Autonomic diabetic neuropathy of cardiovascular system is one of the late diabetic complications. The aim of the study was to compare heart rate time domain analysis in 23 healthy controls and in diabetic patients without (N0; n = 34) and with complications (N1, n = 55) such as retinopathy, peripheral neuropathy as well as autonomic neuropathy. The study was performed in a group of 89 patients with diabetes mellitus type 1. Classical tests of Ewing battery (upright test, Valsalva test, deep breathing test) were performed using the ProSciCard machine as well as parameters of time domain analysis were observed during prolonged to 15 minutes standing in upright body position. Comparing group N1 to group N0 significantly higher heart rate, lower standard deviation of RR interval as well as lower value of rMSSD both at rest and during prolonged stay in upright position were demonstrated.

[Study on correlation between in vivo and in vitro tests of biomaterials--correlation between leukocyte chemotaxis test and muscular implant test in rabbits].

The purpose of this study was to investigate the correlation between the results of in vivo and in vitro tests of biomaterials. With muscular implant test and leukocyte chemotaxis test, the levels of tissue toxicity of seven biomaterials were evaluated. The samples with high-level toxicity showed weak leukocyte chemotaxis, which would influence the anti-inflammatory reaction to foreign bodies. By the method of Spearman rank correlation, the coefficient was between 0.89286-0.98198 indicating that the correlation of the two tests is excellent.

[Genotoxicity of the preparation "binase" in tests on Salmonella: Ames test and ARA-test].

The present work deals with mutagenicity determination of enzyme sample "binase" (Bacillus intermedius ribonuclease) in microbial test-systems: Ames test and Ara-test. The weak mutagenic effect of "binase" high concentration was established in both tests by induction of forward Ara-mutations and Histidine-reverse mutations. A metabolic activation is seen to remove this effect.

[Value of the treadmill test, bicycle ergometry test and the isometric test in evaluating the severity of coronary insufficiency].

Possibilities of the treadmill test, bicycle ergometry and the isometric test are reviewed with respect to the diagnosis and assessment of the severity of coronary heart disease (CHD). A study of 40 normal males and 95 coronary patients (including 57 aneurysm-free ones and 38 with left ventricular postinfarction aneurysm) demonstrated low diagnostic value of standard dynamic load programs for the assessment of coronary insufficiency. An original treadmill test program has been developed where each new step carries a 1 MET increment of the load, allowing to assess the severity of CHD within a wide range of clinical manifestations, and group patients according to the involvement of 1 or 2 coronary arteries. The isometric test is of little value for the diagnosis of CHD, yet it can be useful in the assessment of the severity of coronary insufficiency and identification of patients with the involvement of 2 and 3 coronary arteries.

[From the Thematic Apperception Test (TAT) to the Salzburg Thematic Construction Test. Remarks on an overdue test revision].

In autumn 1985 the "Thematische Gestaltungstest Salzburg", which is a further development of the well-known "Thematic Apperception Test" (TAT) H. A. Murray's, has been published by Beltz. The revision of this test includes not only a modification of the partly already antiquated pictures, but also a fundamental reorientation of the theoretical concept and of the system of analysis developed by Revers. The authors summarize the essential ideas of this test revision and explain the conclusions to be drawn for the discussion of the projection concept in psychodiagnostics. The article also displays a synopsis of the handbook for the TGT-Salzburg, which has been published together with the test.

[The audio-visually determined reaction evaluation test (A-V-R test) (a hearing test for infants and small children)].

The A-V-R-test is accomplished in a free sound field, either with a play audio-metric table (clinical use) or with a textile paravent (practice). The main effort of this reactiontest is the auditive-visual determination. The examiner provokes the auditive readiness with a supraliminal impulse, which prepares the starting position for offering an acoustic test impulse in a low signal strength on the left or right side. If the test is realized with the textile paravent, frequency and intensity proved music instruments or other sound producing objects (bunch of keys, Ewing-rattle, etc.) are offered to the child. If the A-V-R-test is accomplished with the play-audio-metric table, the tester presents a frequency specific narrow band noise to the examinee. Only the turn of the infant's head towards the source of the sound is evaluated. Infants older than 2 years may show specific reactions, depending on the stage of their development.

[Preliminary tests with Hemastix and Sangur test strips and Phosphatesmo KM test paper do not modify DNA typing of blood and semen stains].

Some of the commonly used presumptive test reagents for identification of blood and semen could affect the recovery or the analysis of high-molecular-weight DNA from evidentiary samples. The study presents data on restriction fragment length polymorphisms (RFLP) and polymerase chain reaction (PCR) analyses from blood and semen stains treated with Hemastix (Bayer Diagnostics), Sangur (Boehringer) and Phosphatesmo-KM-paper (Macherey-Nagel). The results demonstrate that the three tests examined do not have any effect on quality and quantity of the DNA nor on DNA typing by RFLP and PCR analyses. In conclusion, presumptive testing of blood and semen stains using Hemastix, Sangur, or Phosphatesmo paper can principally be carried out prior to submission to DNA analysis. In spite of these findings, it is recommended to continue the prudent practice of testing only small portions of an evidentiary stain in order to prevent contamination.

[A cooperative clinical study on the evaluation of an antibody detection test kit (MycoDot Test) for mycobacterial infections. Cooperative Study Group for MycoDot Test].

To determine the usefulness of a diagnostic kit for mycobacterial infection, we performed a five-hospital cooperative clinical study using serodiagnosis kits (MycoDot Test) to detect antibody for lipoarabinomannan (LAM) which is a membrane-derived component of mycobacterial species. We tested LAM antibody in the sera of patients with mycobacterial infection as well as healthy persons. Procedures for using the serodiagnosis kit are actually simple. Out of 130 cases of active pulmonary tuberculosis, 103 cases (79%) were positive for anti-LAM, and cases out of 24 cases of active atypical mycobacterial infection, 15 (63%) were positive. On the contrary, only 4% of healthy volunteers (1 out of 25 persons) were positive on this test. In conclusion, this diagnostic kit might be a useful test for early and supportive diagnosis of mycobacterial infections based on its sensitivity and specificity.
