Mosaics of islet-1-expressing amacrine cells assembled by short-range cellular interactions.

The nervous system has a modular architecture with neurons of the same type commonly organized in nonrandom arrays or mosaics. Modularity is essential to parallel processing of sensory information and has provided a key element for brain evolution, but we still know very little of the way neuronal mosaics form during development. Here we have identified the immature elements of two retinal mosaics, the choline acetyltransferase (ChAT) amacrine cells, by their early expression of the homeodomain protein Islet-1, and we show that spatial ordering is an intrinsic property of the two Islet-1 mosaics, dynamically maintained while new elements are inserted into the mosaics. Migrating Islet-1 cells do not show this spatial ordering, indicating that they must move tangentially as they enter the mosaic, under the action of local mechanisms. Clonal territory analysis in X-inactivation transgenic mice confirms the lateral displacement of ChAT amacrine cells away from their clonal columns of origin, and mathematical models show how short-range cellular interactions can guide the assemblage of these mosaics via a simple biological rule.

Configuration of hepatic veins in the right surgical lobe of the human liver with special reference to their complementary territorial relationships: morphometric analysis of controlled specimens with clearly defined portal segmentation.

The configurations of hepatic veins, particularly the complementary territorial relationships between the veins, in the right surgical lobes of 156 human livers, the segments of which were identified clearly according to the portal ramification, were studied. In order to assess the functional roles of the vessels, we compared the diameter values of the vessels in the upper region of the lobe under the assumption that this parameter corresponds very closely to the actual blood flow volume through the vessel. The right hepatic vein (RHV) sometimes (17%) developed poorly and drained neither segment V (S5) nor VI (S6); instead, the inferomedial part of the S6 was often (26%) drained by the middle hepatic vein (MHV). However, most thick short hepatic veins (SHVs) did not drain S6 specifically instead of a poorly-developed RHV, but usually drained both S6 and segment VII (S7), irrespective of the configurations of other veins. Sometimes (8%), the RHV, rather than the MHV, drained a large part of S5. Overall, in the lower region of the lobe, the RHV, SHV and MHV showed complementary venous drainage relationships. Furthermore, the RHV usually (75%) ran not between S5 and S6 but through S6, corresponding to its usual territory, and could not be regarded as the intersectorial landmark in the lower region. Although various names have been given to the thick venous tributaries in the upper region of the right lobe, we systematically classified these tributaries into 5 types of "right superior radicles" (RSR), according to their topographical relationships with the right superior portal branches (P7 and P8), RHV, MHV, SHVs and their tributaries. When just thick RSRs were considered, the 5 types (anterior, posterior, lateral, medial and intersegmental) were observed in 90%, 88%, 89%, 38% and 34%, respectively. Notably, all but the intersegmntal RSRs almost always corresponded to a specific segmental territory (S7 or segment VIII (S8) and had specific terminals (at the RHV or MHV), suggesting that these 4 RSRs were proper segmental or intrasegmental veins, whereas, the 5th type showed a different configuration, i.e., an intersegmental vein that drained both S7 and S8, ran along and above the RHV and merged with it. Our morphometric examination of the upper region revealed that the sizes of P7 and P8 increased or decreased simultaneously, and suggested that the RSRs played limited roles in venous return. Rather than the RSRs, the SHVs appeared to drain the overflow in excess of the venous return capacity of the RSRs. Despite their limitations and possible complementary relationships with some veins, however, all the superior veins tended to developed evenly well or poorly. Overall, the venous return did not seem to converge into a single particular vein, but was carried away by multiple superior veins: the RHV, MHV, SHVs, and 5 RSRs.

[Epidemiological characterization of Ixodes tick-borne borreliosis in the Krasnoyarsk territory].

The analysis of morbidity in tick-borne infections for many years indicates that in the 1960s-1990s in the total structure of the clinical forms of tick-borne encephalitis the erythematic form amounted to 1.4-20.9, which 1.9- to 3.1-fold exceeded the morbidity characteristics for the whole of Russia. The epidemiological analysis of morbidity at the territory for the period of 1992-2001 is indicative of the presence of the combined foci of tick-borne encephalitis and Ixodes tick-born borrelioses (TBB) in 42 regions of the territory. The cyclic character of morbidity and the irregularity of its spread in different regions of the territory can be observed. Among the TBB patients, town dwellers constitute, on the average, 75.8%. These tick-born infections exhibit regional differences connected with landscape variations and the patients age. However official statistics does not reflect the real epidemic situation at the territory: hypodiagnostics is primarily due to the absence of reliable laboratory confirmation in remote regions of the Krasnoyarsk territory.

Analysis of a territorial approach to the delivery of nursing home care services based on historical data.

We analyze a territorial approach to deliver nursing homecare services to a territory public health. We present the case of the CSSS assigned to Côte-des-Neiges, Métro center and Parc Extension, specifically the case of the Côte-des-Neiges site (CLSC CDN), where a territorial approach is used since 1980. We first give an historical comparison of patient visits delivered in 1998-1999 and in 2002-2003. We follow with an in-depth analysis of the home services delivered in 2002-2003 to determine whether or not the territorial approach can well support the changing needs of the population. We conclude that the territorial approach to deliver homecare nursing services does not sufficiently support fluctuations in population needs for services. Not only is it difficult to predict these fluctuations, but it is difficult to accurately quantify the true needs for services since the availability of nursing services tends to determine the services actually delivered. In sectors of the territory where resources are more scarce (based on previous population needs analyses) or demand for services is greater, the result is work overload for the nursing staff. In addition, this results in service delivery inequities across the entire territory. Therefore, a more dynamic assignment of clients to the nurses based on each nurse's work load and case load rather than based on the geographic location of clients is worth the extra administrative time in case assignment to ensure a more equitable case load attribution between nurses as well as less inequities between clients in terms of service delivery considering their needs.

A genetic analysis of chromosome territory looping: diverse roles for distal regulatory elements.

Recent studies of nuclear organization have shown an apparent correlation between the localization of genes within the interphase nucleus and their transcriptional status. In several instances, actively transcribed gene loci have been found significantly looped away from their respective chromosome territories (CTs), presumably as a result of their expression. Here, we show evidence that extrusion of a gene locus from a CT by itself is not necessarily indicative of transcriptional activity, but also can reflect a poised state for activation. We found the murine and a wild-type human beta-globin locus looped away from their CTs at a high frequency only in a proerythroblast cell background, prior to the activation of globin transcription. Conversely, a mutant allele lacking the locus control region (LCR), which is required for high-level globin expression, was mostly coincident with the CT. The LCR may thus be responsible for the localization of the globin locus prior to activation. Replacement of the LCR with a B-cell-specific regulatory element, while also extruding the globin locus, brought it closer to the repressive centromeric heterochromatin compartment. We therefore suggest that the looping of gene loci from their CTs may reflect poised and repressed states, as well as the previously documented transcriptionally active state.

Arterial anatomy of the oral cavity: an analysis of vascular territories.

Knowledge of the specific cutaneous or surface regions supplied by constant named arterial sources has allowed for increasing clinical application of flap transfers of tissue. Despite the routine use of intraoral flaps for reconstruction of congenital or acquired defects of the oral cavity and pharynx, no previous investigation has centered on understanding the surface or mucosal arterial territories of the oral cavity. In a cadaver study, six mucosal territories of the intraoral cavity were defined using selective ink and lead oxide injections through named arteries. The anatomical boundaries of these territories are predictable and constant in location for different cadavers. The six contiguous territories are based on the buccal, labial, inferior alveolar, ascending palatine, ascending pharyngeal, and lingual arteries. This study supports the safe vascular basis of existing clinical procedures of the intraoral cavity and may have implications for the design of new intraoral reconstructive procedures.

Arterial anatomy of the face: an analysis of vascular territories and perforating cutaneous vessels.

The cutaneous arterial circulation of the face and scalp was investigated by using multiple anatomic techniques in fresh cadaver specimens. Selective ink injections, dissection and measurement of latex-hardened perforators, and radiographic examination of transverse sections of barium-injected specimens defined 11 vascular territories of the face and scalp. Three distinct patterns of arterial vascularization were observed: (1) small, densely populated musculocutaneous perforating arteries supply the anterior face (facial, infraorbital arteries), (2) large, sparsely populated fasciocutaneous perforators that occur in predictable locations supply the lateral region of the face (branches of the transverse facial, submental, and zygomatico-orbital arteries), and (3) small, densely populated fasciocutaneous perforators supply the scalp (occipital, superficial temporal, and posterior auricular arteries). The locations of the perforation sites of the transverse facial, zygomatico-orbital, supratrochlear, supraorbital, and superficial temporal arteries may be predicted statistically within 2-cm limits with 95 percent certainty for individuals in the general population. The results of this study provide the anatomic framework to potentially revise or improve current reconstructive or aesthetic procedures on the face based on known arteries.

[HIV infection and parenteral virus hepatitis in the Krasnodar territory].

The analysis of the morbidity dynamics of HIV infection, hepatitis B and C in the Krasnodar territory for 1996-2003 is presented. The tendency of strengthening direct correlation between age-dependent rates in these groups of diseases has been established. The correlation coefficient (rxy) is at present +0.851 (HIV infection-virus hepatitis B) and +0.892 (HIV infection-virus hepatitis C). The highest levels of primary morbidity are registered in persons aged 20-39 years. The established epidemiological parallels between HIV infection and parenteral hepatitis require the development of the unified strategy of the prophylaxis of these diseases on the federal and regional levels.

[Chronic diseases of the upper respiratory tracts and the organ of hearing in population of the Central Federal Territory].

The analysis of the occurrence of chronic diseases of the upper respiratory tracts (URT) and the organ of hearing (OH) among population of three regions of the Central Federal Territory with a total population of more than 3.5 million people showed predominant prevalence of OH affection -- 59.0 cases per thousand population. Most common URT diseases are represented by chronic pharyngitis (22.5/1000), chronic tonsillitis (15.9/1000) and chronic rhinitis (13.2/1000). OH diseases in children account for 11.5% of chronic diseases of URT and OH, URT chronic diseases made up 79.1%. In adults it was 36,3 and 58.5%, respectively. In retired persons OH diseases were prevalent -- 67.4%.

Quantitative assessment of transcriptome differences between brain territories.

Transcriptome analysis of mammalian brain structures is a potentially powerful approach in addressing the diversity of cerebral functions. Here, we used a microassay for serial analysis of gene expression (SAGE) to generate quantitative mRNA expression profiles of normal adult mouse striatum, nucleus accumbens, and somatosensory cortex. Comparison of these profiles revealed 135 transcripts heterogeneously distributed in the brain. Among them, a majority (78), although matching a registered sequence, are novel regional markers. To improve the anatomical resolution of our analysis, we performed in situ hybridization and observed unique expression patterns in discrete brain regions for a number of candidates. We assessed the distribution of the new markers in peripheral tissues using quantitative RT-PCR, Northern hybridization, and published SAGE data. In most cases, expression was higher in the brain than in peripheral tissues. Because the markers were selected according to their expression level, without reference to prior knowledge, our studies provide an unbiased, comprehensive molecular signature for various mammalian brain structures that can be used to investigate their plasticity under a variety of circumstances.

Problem of weights in the analysis of demographic phenomena in territorial arrangement.

The author attempts to eliminate the influence of the size of a territorial unit on the results of demographic analysis, using data for Poland as an illustration. She concludes that "the inclusion of weights in statistical analysis allows us to eliminate the impact of the sizes of studied units on the values of counted parameters as well as on the measures of correlation and regression. The existence of a close connection between the results obtained and the sizes of the studied groups within the same population should be always taken into account."

Inequitable distribution of general practitioners in Australia: analysis by state and territory using census data.

The objective of this study was to describe the distribution of general practitioners in each State and Territory, stratified by statistical division and adjusted for estimated community need. The location of general practitioners was obtained from the 1996 Census of Population and Housing. Community need was estimated from crude death rates supplied by the Australian Bureau of Statistics. On average there are 920 people per full-time general practitioner in Australia. Three States are relatively oversupplied by up to 5% (Australian Capital Territory) and the rest are relatively undersupplied by up to 12% (Western Australia). Adjusted for estimated need, the Australian Capital Territory is oversupplied by 71% compared with all of Australia, while Western Australia is undersupplied by 15%. More marked differences occur within the States, with the statistical division containing each capital city in each State that is relatively overserved. The greatest oversupply is in Sydney, where 33% fewer people share a full-time general practitioner than the whole of New South Wales (adjusted for need, oversupply in Sydney is 63%). Relative undersupply is greatest in Queensland, with 133% more people sharing each general practitioner in the north-west statistical division compared with the whole State. The distribution of general practitioners between and within States and Territories is unequal and inequitable. In each State, capital cities tend to be relatively oversupplied compared with more rural areas. While these data do not inform the absolute level of service needed in a community; they do suggest that strategies to redress the inequitable distribution are required.

Technetium-99m-sestamibi SPECT to detect restenosis after successful percutaneous coronary angioplasty.

UNLABELLED: This study evaluated the accuracy of 99mTc SPECT in predicting restenosis after primary successful PTCA. METHODS: Thirty-seven patients with equivocal symptom-limited exercise stress testing were evaluated. All patients underwent separate day exercise-rest 99mTc-sestamibi SPECT. The perfusion studies were evaluated using three different methods of analysis: visual inspection, semiquantitative and quantitative polar map analysis. The perfusion studies were interpreted in absence of a pre-PTCA scan. All patients underwent a control coronary angiography within 1 mo. RESULTS: Sensitivity and specificity of 99mTc-sestamibi SPECT in predicting restenosis were 87.5-78%, 50-65% and 75-74% for visual inspection, semiquantitative and quantitative polar map analysis, respectively. Sensitivity and specificity related to the vascular territories were: LAD territory 93-73% (qualitative analysis), 53-60% (semiquantitative analysis), 80-67% (quantitative analysis); LCX territory 83-100% (qualitative analysis); and 33-100% (semiquantitative analysis), 67-100% (quantitative analysis); and RCA territory 67-80% (qualitative analysis), 67-60% (semiquantitative analysis), 67-80% (quantitative analysis). CONCLUSION: These data suggest that 99mTc-sestamibi SPECT is a useful noninvasive tool in the follow-up evaluation of patients who have undergone angiographically successful coronary angioplasty even in the absence of a pre-PTCA perfusion study.

A computer system for epidemiological surveillance.

In epidemiological surveillance, it is imperative that any unusual increase in reported cases be detected as quickly as possible. In the surveillance unit at "Pedro Kourí," it was necessary to create a computer system for the surveillance of transmissible diseases in order to manage such variables as morbidity, mortality, and the circulation of causal agents. As usage flexibility is a fundamental requirement, we developed VIGILA as a system readily adaptable to any level of the country's health organizations (national, provincial, or municipal). VIGILA permits the storage, validation, and statistical analysis of morbidity and mortality data, and it also allows the user to apply these features to causal agent information. These manipulations are all performed in the context of a specific transmissible disease. The stored information can be displayed in different types of statistical formats, such as calculated rates, accumulates and medians. The user can also design his/her own tables for display. In addition, the program offers graphics that depict the endemic forecast for a specific region and age group. Reliable forecasts based on temporal models of an epidemiological indicator are necessary for the prediction of the non-epidemic indicator and for the elaboration of an alert threshold. The endemic forecast or endemic channel is calculated by means of a modified version of R. Serfling's model [1] which adapts to data series with stationary characteristics. In this case, the model is fixed for the medical attention protocol and the circulation of agents of a specific transmissible disease. The parameters of the model are estimated by the least square method. The confidence limits are calculated with the T-Student distribution [2]. To enter information about a specific disease and level of health concern, the user must define the disease under analysis, the principal territory under surveillance and its dependencies, the causal agents of the disease, and the frequency with which the disease is reported. This system stores the number of cases (morbidity) and death (mortality) that occurred in the territories and age groups defined during entry. To facilitate the calculation of morbidity and mortality rates, information pertaining to the number of territory habitants and their age groups is also stored. The laboratory data includes the number of analyzed samples in the territories and age groups defined in the data entry, and the samples that were positive to the defined agents. Among the principal uses of the system are: calculation of cumulative cases, rates, and cumulative rates for the specific period of analysis, territory, and age group; comparison of these health indicators with the previous year's indicators or with the median of the previous years' indicators' calculation of the endemic forecast that permits the user to observe the epidemiological situation by territories and age groups; and the early detection of an increase in disease occurrence. The system also offers a geographical representation of the epidemiological situation in the territories, showing a map with the evaluation of each territory with respect to four possible risk factors. VIGILA is available in both DOS and WINDOWS 3.1 formats. This system allows the evaluation of the situation at a specific point in time in the analyzed territories, and may alert the infection control team to an epidemic early enough to allow implementation of control measures.

Territorial justice and primary health care: an example from London.

This paper is part of a larger piece of research which examines the spatial relationship between need for and provision of primary health care in London. The research reported here is concerned with empirically testing the 'inverse care law' at the DHA level. One concept that may be used to guide this analysis is 'territorial justice'. Several conceptual problems associated with the use of territorial justice are outlined. These include inadequate conceptualisation of the form of social justice assumed, of the problem of deriving need indices and of the nature of resources. A final problem is concerned with the spatial scale of the analysis. The concept of territorial justice is made operational so as to identify relatively under and overprovided DHAs. The result is that the often held assumption of a simple dichotomy of relatively underprovided inner DHAs and overprovided outer DHAs is shown not to be tenable. However, this research concentrates on the quantity of care, and does not focus on the important aspect of its quality. This preliminary analysis reveals the need for further research on this important topic.
