Evaluation of the evidence that tobacco-specific nitrosamines (TSNA) cause cancer in humans.

Considerable evidence exists that TSNA may cause cancer in humans. Strong evidence suggests that the NNN and NNK in snuff are at least partially responsible for the excess of oral cancer among snuff dippers. Tobacco smoke, because it is a combustion product, is a far more complex matrix than is smokeless tobacco. Nonetheless, NNK is well established as a lung carcinogen in laboratory animals at levels similar to those experienced by heavy smokers. Although the epidemiology of the smoking/lung cancer association has been studied extensively and is well established as a causal relationship, few attempts have been made to determine the effective constituents in tobacco smoke, much less to quantify them and to reduce their levels in consumer products. More work is needed to determine TSNA uptake in smokers and users of smokeless tobacco and to determine the factors involved in the possible endogenous formation of TSNA and in their metabolic activation or inhibition of their formation. Exploratory work is in progress with various biological markers that may prove useful in quantifying the levels of TSNA exposure in humans. Epidemiologic evidence indicates that in addition to tobacco use other factors are causally related to the development of cancers of the oral cavity, lung and esophagus and that these factors (e.g., radon exposure in uranium miners who smoke) can act synergistically. Experimental investigation of some of these synergistic effects may prove to be a step in the direction of determining the proportion of various cancers that are likely to be attributable to various causes and ultimately to the various constituents of tobacco exposure.

Hospital activity. Barred facts.

Barcode technology is applicable to a number of crucial management issues in the NHS, including internal efficiency, contract pricing and the effectiveness of care. Its enormous potential lies in the ability to record in detail over a number of dimensions such as time, staff group, patient type, and cost. But the effort involved is considerable. Projects have to be actively managed, users have to be willing participants and data has to be analysed. Where short cuts have been sought, projects have failed to deliver their full potential. The data is merely a means to an end, but a very powerful one. It brings together people who may normally work independently, and the process of collecting barcode data is a perfect mechanism for bringing together the patient, the professional and the purse strings.

The How Are You? scale: a quality-of-life outcomes measure for routine practice.

The How Are You? scale, a quality-of-life outcomes self-report measure to be completed by patients, was developed to address the concerns of the mental health consumer movement. Specific concerns included using less technical language in assessing mental health problems and developing a collaborative dialogue between clinician and patient. The How Are You? scale is a user-friendly instrument that allows patients to be actively involved in their assessment and treatment process. To implement the How Are You? scale into a recording and measurement system, its psychometric properties were assessed using two samples: (1) An employee group from The Menninger Clinic was used to assess internal consistency and stability, and (2) a patient group from The Menninger Clinic was used to assess known-group and discriminant validity, as well as sensitivity to change. The How Are You? scale was found to have good internal consistency and stability and good known-group and discriminant validity. It was also found to be sensitive to change. Although data continue to be collected at The Menninger Clinic, initial indications are that the How Are You? scale is a psychometrically sound outcomes measurement tool.

Web-based multimedia courseware for emergency cardiac patient management simulations.

This is a multidisciplinary inter-departmental/faculty project between the departments of computer science, electronic, communications and electrical engineering and nursing and paramedic sciences. The objective is to develop a web based multimedia front end to existing simulations of cardiac emergency scenaria. It will be used firstly in the teaching of nurses. The University of Hertfordshire is the only University in Britain using simulations of cardiac emergency scenaria for nurse and paramedic science education and therefore this project will add the multimedia dimension in distributed courses over the web and will assess the improvement in the educational process. The use of network and multimedia technologies, provide interactive learning, immediate feedback to students' responses, individually tailored instructions, objective testing and entertaining delivery. The end product of this project will serve as interactive material to enhance experiential learning for nursing students using the simulations of cardiac emergency scenaria. The emergency treatment simulations have been developed using VisSim and may be compiled as C code. The objective of the project is to provide a web based user friendly multimedia interface in order to demonstrate the way in which patients may be managed in critical situations by applying advanced technological equipment and drug administration. Then the user will be able to better appreciate the concepts involved by running the VisSim simulations. The evaluation group for the proposed software will be the Department of Nursing and Paramedic Sciences About 200 nurses use simulations every year for training purposes as part of their course requirements.

A fantastic journey: 3D cardiac ultrasound goes live.

With a recent product introduction, live 3D echo is now clinically practical. It is already beginning to have a profound impact on the way we care for patients at The University of Chicago Medical Center. In the past, dynamic cardiac 3D rendered images were possible by sequentially acquiring 2D images and then using a workstation to input 2D images for Cartesian coordinate conversion and volume rendering. Outside research settings, this time-consuming process proved cumbersome and was simply impractical. Now that these technical and practical issues have been addressed, real-time 3D cardiac sonography has great potential to impact both patient care and throughput in a number of ways, including better pre- and post-surgical planning, improved measurement of heart function, decreased exam times, and enhanced communication between clinicians and their patients. With real-time 3D cardiac ultrasound images, clinicians will be able to better quantify size, shape and function of the heart. However, the most important contribution of real-time 3D sonography in cardiology may be improvement in locating abnormalities for surgical planning. The new technology will also provide important information regarding surgical outcomes. A great benefit to obtaining more diagnostic information and higher diagnostic confidence from real-time 3D cardiac ultrasound images is that it could lead to more rapid exam times and the reduction of patient wait times. Being able to see the whole heart makes examinations more simple and rapid, benefitting the staff and patient. The utility of this technology is unusually broad, as it is able to move beyond diagnostics into a key role in therapeutic procedures. As with any new technology, there will be a learning curve to understanding 3D imaging. Though the matrix transducer is somewhat larger than a standard 2D probe, the ergonomics are quite similar. The interface of the ultrasound unit is also very user friendly. Because real-time 3D cardiac ultrasound involves looking at the heart as if you are holding it in your hands, with the additional ability to turn it any way you want, we expect that the transition from 2D to 3D will be easily achieved.

RADIATION PROTECTION IN CANADA. II. THE ROLE OF THE RADIATION PROTECTION DIVISION IN SAFEGUARDING THE HEALTH OF RADIATION WORKERS.

The current status of radiation protection in Canada is discussed in the second of a three-part series and particular emphasis is placed on the role of the Radiation Protection Division of the Department of National Health and Welfare. Administrative and operational control procedures have been developed, involving prior approval of health safeguards in the radioisotope user's facilities and techniques, and systematic monitoring and inspection. Where necessary, a medical follow-up of accidents and excessive radiation exposures is carried out. In 1963 more than 1600 radioisotope licences were issued. Filmmonitoring service was provided to about 15,500 isotope and x-ray workers. Semiautomatic handling procedures have been developed to meet the increasing demand for film-monitoring services.Monitoring and inspection services have been provided for x-ray workers, and a committee has been formed to develop administrative procedures for health and safety control in x-ray work. Committees have also been set up to review the health and safety aspects of the operation of nuclear reactors and particle accelerators.

[Epidemiological, clinical and therapeutical aspects of HIV/HCV coinfection in a series of HIV seropositive Umbrian patients].

Because hepatitis C virus (HCV) and human immunodeficiency virus (HIV) share common transmission pathways, HIV-HCV co-infection is frequent, involving about 40% of seropositive subjects particularly injection drug users and patients with hemophilia. We performed a retrospective analysis on clinical, epidemiological and therapeutical aspects in a population of HIV-HCV coinfected patients, observed in our Department during the period 2001-2003. Forty per cent of 404 observed patients had a co-infection; 90% of those were drug addicts and most (90.2%) were on HAART treatment. Seventy-three per cent of co-infected patients showed transaminases alterations, and 85% had detectable viremia. Prevalent genotypes were 1 (44.6%) and 3 (36.4%). The association PEG-IFN and ribavirine obtained sustained responses in 55% of 9 treated patients.

A distributed information analysis for information search tasks.

Information search in a distributed environment is an interactive process that involves both retrieval and the processing of information across users and artifacts. How the information is distributed across internal representations and external representations affects the efficacy of information search. Using a human-centered method called UFuRT, we developed an information search model and a taxonomy of search tasks. We further developed several prototypes of information search interfaces with different patterns of distributed information and investigated the relations between search tasks and interface types. The results from the analyses show that UFuRT is a useful process that not only provides design guidelines but also generates estimates of representational efficiencies, task complexities and user behavioral outcomes.

[Current status of bath water treatment techniques].

The Federal German DIN standard No. 19644 that has been enforced in West Germany since 1989 had to be revised in view of the fact that German legislation is expected to issue an ordinance on swimming pool and public bath water within the framework of the Federal Communicable Diseases Act; furthermore, the need for revision also arose from the solution of the hygienic problems of treatment and disinfection of warm whirlpool water, and from the necessity of modifying several now outdated regulations. It is the aim of public health legislation to prevent health risks of bathers and of attendant staff by Legionella pneumophila in all types of basins with additional water cycles and intake of air (aerosol formation); likewise, adverse health effects of side reaction products of the disinfectant chlorine (i. e. chloroamines and halogenated organic compounds, especially the trihalomethanes) must be avoided. Recent research has shown that these aims can be achieved by altering the mode of running and backwashing filters of all process combinations including those using ozone and introducing an additional absorption on powdered activated carbon, and the process combination of flocculation-filtration-chlorination. Improved treatment would be without effect if turnover rates could not be assessed reliably for basins of irregular geometry--as they are preferably used today--operated with additional water cycles involving intake of air. Lack of costly investigations prompts users to estimate turnover rates on the basis of experience collected with basins of square or rectangular dimensions.

Voice analysis to predict the psychological or physical state of a speaker.

A vocal message, apart from its semantic content, carries information on the psychological and physiological condition of the speaker. Physical fatigue and especially psychological stress are the pathological elements of the condition. The accepted term for the cause of these effects is the "workload." This article describes the main research carried out since the 1940's to measure the acoustic modifications of the voice brought about by a workload. It concludes by a critical analysis of the studies and a short description of the perspectives for research. Their results mainly concern astronauts and pilots involved in specific high-stress tasks and possible users of voice recognition systems. All the studies show an excellent approach to this field of research but deserve to be widened, deepened, and made more accurate to enable estimating the nature or level of reaction to a workload.

Carers' perspectives.

The issues around elder abuse and carers are complex. They concern individual carers and users and their relationships. They concern professionals who are involved and have implications for social policy. Alternatives to family care must be made available and we must address the issue of how long-term care is to be provided.

An interactive technique for three-dimensional image registration: validation for PET, SPECT, MRI and CT brain studies.

UNLABELLED: A multipurpose three-dimensional registration technique was validated with PET, SPECT, CT and MRI scans, which had been obtained under normal clinical conditions. In contrast to fully automated procedures, this coregistration method is highly interactive, which has the advantage that it does not impose rigid restrictions by data type and by alterations in normal anatomy or brain function resulting from disease. METHODS: Basically, a computer program provides a variety of tools to examine the accuracy of coregistration visually and to specify necessary translations and rotations in all three dimensions. Tools and criteria to accept coregistration were applied according to a standardized protocol. Reproducibility was assessed with five independent users on nine pairs of image sets. In two pairs of these image sets, coregistration was repeated three times by each user. RESULTS: Depending on the resolution of the images involved, the reproducibility of translation distances ranged from 0.32 to 2.22 mm (s.d.) and of rotation angles from 0.32 to 1.70 degrees. It was always much smaller than the point-spread full-width half maximum of the device with the lower resolution. The accuracy of coregistration was examined using two arbitrarily misplaced image sets. Interindividual and intraindividual variance were similar, which suggested that the influence of subjectivity was not significant. Average displacements after coregistration were 0.43 and 0.29 mm or less for PET and MRI data, respectively, which indicated the absence of a systematic bias. CONCLUSION: The results indicate the high reproducibility and accuracy of this three-dimensional coregistration technique, which is comparable or superior to those of automated techniques and methods based on external artificial landmarks.

Laser hazards in research laboratories.

Laser use is expanding rapidly in laboratories, where multiple use strategies of research are not often amenable to laser engineering safeguards employed in industry. We report on five cases of laser-associated incidents requiring medical follow-up from a single research institution, and present exposure circumstances from four other incidents. Eye injuries from direct and reflected beams resulted from open alignment procedures in the absence of appropriate eye protection. Nd:YAG lasers operated with doubling crystals appear to create a considerable risk. For several reasons, we believe the importance of laser-associated injuries are underestimated and the incidence underreported in the research setting. Although knowledge personnel are shown to suffer lapses in standard operating procedures, most incidents and all injuries involved relatively inexperienced personnel. We propose that research laser users be registered and required to undergo formal training in laser safety. Student researchers are a key target population. Collaborative implementation between the equipment supplier and research user will be required to achieve this widespread implementation of effective training.

The impact of IAIMS on the work of information experts. Integrated Advanced Information Management Systems.

Integrated Advanced Information Management Systems (IAIMS) programs differ but have certain characteristics in common. Technological and organizational integration are universal goals. As integration takes place, what happens to those implementing the vision? A survey of 125 staff members, or information experts, involved in information or informatics at an IAIMS-funded institution was conducted during the last year of the implementation phase. The purpose was to measure the impact of IAIMS on the jobs of those in the library and related service units, and the computing, telecommunications, and health informatics divisions. The researchers used newly developed scales measuring levels of integration (knowledge of and involvement with other departments), customer orientation (focus on the user), and informatedness (changes in the nature of work beyond automation of former routines). Ninety-four percent of respondents indicated that their jobs had changed a great deal; the changes were similar regardless of division. To further investigate the impact of IAIMS on librarians in particular, a separate skills survey was conducted. The IAIMS librarians indicated that technology and training skills are especially needed in the new, integrated environment.

Smoker's melanosis. Report of a case.

Melanin pigmentation of the oral cavity among tobacco smokers, "smoker's melanosis", was first described by Hedin in 1977. Studies performed on dark skinned ethnic groups found that although nearly all non-tobacco users had oral melanin pigmentation; tobacco smokers had significantly more oral surfaces pigmented than non-tobacco users. We present a case of oral smokers melanosis involving the tongue of a 37-year-old black female.
