[Causal therapy for diseases of the inner ear? (author's transl)].

The examples of sudden deafness and Meniere's Disease are used to discuss the possibilities of causal therapy for inner ear dysfunctions. In sudden deafness, etiological and therapeutic considerations are focused on the problem of disorders of the arterial and venous microcirculation, contributing to oxygen deprivation of the organ of Corti. Disruption of the arterial microcirculation leads to irreversible damage of the sensory organ within minutes and non-responsiveness to therapy, whereas even serious venous circulation disturbances can be treated successfully within days after onset of the acute disease. While knowledge of pathological changes in sudden deafness is still limited, temporal bone studies in Meniere's disease provides important information on its pathogenesis. Many authors have described enlargement of the cochlear duct as evidence for endolymphatic hydrops. Maximum enlargement of the endolymphatic system disrupts the membrane system which separates endo- from perolymph, and can be confirmed by histologic examination. This indicates that a change of the ionic concentration in the inner ear fluids is responsible for inner ear dysfunction. Thus, the acute Meniere attack can be explained as a consequence of the "burst hydrops." Because of the still unknown etiology of hydrops, only its consequences can be treated at the present time: i.c., immediate removal of the toxic potassium concentration from the perilymph by increasing the microcirculation of the inner ear before irreversible damage occurs.

Schistosomiasis and carcinoma of the bladder: a critical appraisal of causal relationship.

The pathology of 154 cases of carcinoma of the urinary bladder diagnosed in surgical specimens in Ibadan is reviewed. Schistosoma haematobium-infections are found in association with all varieties of urothelial carcinoma. The majority of tumours in this and previously reported series from bilharzial and nonbilharzial underdeveloped regions presented in advanced stages. It is suggested that the preponderance of squamous cell carcinoma in these regions is a reflection of late clinical presentation. The evidence so far presented to support a causal relationship between schistosomiasis and carcinoma of the bladder is reviewed. It is concluded that such relationship is probably not true.

A causal model for turnover for nurses.

Longitudinal data on 1,091 registered nurses in seven hospitals were used to estimate a causal model of turnover in organizations. Total effects on turnover were found to be the greatest for four determinants: intent to stay, opportunity, general training, and job satisfaction. The relative importance of the determinants of turnover and the explanatory power of the model are presented and discussed.

[Work accidents in Brazil. A study in São Paulo state, Brazil, the Botucatu region, using the causal tree method].

This paper discusses, within the prevailing Brazilian situation, the possibility of applying "causal tree" (CT) method in investigating occupational accidents by safety personnel in the public health services and workers' unions. The method was developed during the seventies in France, for use by plant safety personnel. The authors used this method in Botucatu, state of São Paulo, Brazil, in order to investigate 40 serious occupational accidents that occurred in industrial plants during the second half of 1993, that had been registered by social security. In these cases, the predominance of situations in which the lack of safety measures were identified by inspection indicates that in most instances, the use of CT is unnecessary. However, the authors discuss its use by safety personnel from the public health services and workers' unions to investigate certain accidents to contribute to the knowledge base and help overcome the cultural based guilt which, in Brazil, has turned the victim into the person responsible for the accident.

The paradigms of causality and treatment for autoimmune disease.

A key concept in medicine is that rational therapy rests on accurate diagnosis; quite simply, therapy that is not tuned to the cause of the disease will not cure the patient. This does not mean that effective treatments cannot emerge from faulty diagnoses. In truth, much of our therapeutic ensemble is composed of drugs developed as a result of chance observation, random screening, intuition or prescientific tradition. Nevertheless, the way to effective therapy is best paved by understanding. Effects are inherent in their causes, so if we want to cure autoimmune diseases using the scientific method, we are obliged to inquire into their causes. By reducing the discordant complexity of the disease to the single cause that underlies it, we can hope to learn the most efficient way to manipulate the disease process. How do we identify a cause when we see one? Quite simply, a single cause is that which is both necessary and sufficient to produce the effect. Here, I explore the general paradigm of autoimmune causality, using multiple sclerosis as a specific example.

[Causality relationship of drug adverse reactions: experience in the use of HIV-protease inhibitors].

PURPOSE: To establish the exact cause and effect relationship between protease inhibitors (PIs) and adverse events. MATERIALS AND METHOD: Prospective, cohort, multicenter study on HIV-positive patients who are beginning treatment with a PI. Causal relationships are evaluated using the RUCAM algorithm. RESULTS: Since the beginning of the study 1207 patients have been enrolled. Average time of observation is 10.7 months. To date, 784 adverse events have been observed, distributed as follows: excluded 3.8%, improbable 18.5%, possible 41.3%, probable 30.1%, and highly probable 6.3%. Saquinavir shows a statistically significant difference in the rate of non-correlated events with respect to other groups. CONCLUSIONS: Over 20% of adverse events during PI treatment are shown to be non-correlated to these drugs. Saquinavir shows the highest rate of non-correlated events.

[Causal relationship between Helicobacter pylori infection and upper gastroduodenal diseases].

Helicobacter pylori infection causes chronic gastritis (nonatrophic gastritis), which progresses to atrophic gastritis and intestinal metaplasia over a period of decades. Atrophy may result from inflammation and apoptosis caused by H. pylori infection. H. pylori is an important risk factor for peptic ulcer disease. Duodenitis in the gastric metaplasia of the duodenum, hypergastrinemia, and impaired proximal duodenal mucosal bicarbonate secretion are considered causal factors for duodenal ulcer disease. Low-grade B-cell gastric lymphoma of mucosa-associated lymphoid tissue (MALT) develops in response to H. pylori infection. Studies of Mongolian gerbil model demonstrated that H. pylori had an initiator or promoter effect on gastric carcinogenesis.

Combined effect of context change and retention interval on interference in causality judgments.

Four experiments studied the effects of context change and retention interval on retroactive interference in human causal learning. Experiment 1 found evidence of retroactive interference. Experiment 2 found that either a 48-hr retention interval or a change in the context after the interference treatment decreased retroactive interference. An interaction between context change and retention interval effects was also found, eclipsing the context change effect after the 48-hr retention interval. Experiments 3 and 4 found additivity between context change and retention interval effects when participants were remained of the difference between physical contexts before the test, independently of whether the context change involved a return to the original acquisition context. These results add to the evidence suggesting that spontaneous forgetting is caused by a change in either the physical or the temporal contexts where information is acquired.

The association of unilateral congenital glaucoma and congenital lower lid entropion: causal or casual?

This case documents unilateral congenital glaucoma associated with congenital lower lid entropion. A 2-year-old female infant was referred for evaluation and treatment of right-side buphthalmos caused by congenital glaucoma associated with bilateral congenital lower lid entropion that was prominent on the right side and present at birth. Examination disclosed a lower eyelid entropion of the right side that was treated surgically by reinserting the disinserted retractor aponeurosis to anterior inferior tarsal border. After three weeks, the patient was successfully treated with primary combined trabeculotomy-trabeculectomy for congenital glaucoma. The entropion of the left lower lid was asymptomatic and did not require any surgery. Buphthalmos caused by congenital glaucoma may be associated with congenital lower lid entropion and the association may be causal or coincidental.

[Therapy of polyneuropathies. Causal and symptomatic].

In the first instance, polyneuropathies are treated causally. The most common underlying cause is diabetes mellitus or alcohol abuse. In a large number of patients with polyneuropathy, however, the underlying cause cannot be definitively identified. For these--but equally for patients with etiologically clear polyneuropathy--a stock-taking of clinical symptoms should be carried out and, where indicated, symptomatic treatment initiated. In addition to medication aimed at combating pain, muscular spasm, autonomic functional disorders, and for the prevention of thrombosis, physical measures (physiotherapy, foot care, orthopedic shoes) are of primary importance.

Causal connections between socio-economic status and health: reciprocal effects and mediating mechanisms.

Using structural equation modeling techniques on data from a nationally representative longitudinal survey, we first explored the reciprocal relationships between socio-economic status (SES) and health status. We then estimated the degree to which health-related lifestyles/behaviors and psychosocial distress are mediating mechanisms of these relationships. As predicted, SES positively affects health, and health positively affects SES. Although the causal path from SES to health is stronger than the reverse, these findings confirmed the hypothesis that both social causation and health selection contribute to social inequalities in health. In terms of the mediating mechanisms through which SES and health affect each other, more than a third of the overall SES-health relationship was accounted for by health-related lifestyles/behaviors and psychosocial distress. A notable part of the effect of SES on health is due to differences in psychological distress, with the effects of health-related lifestyles/behaviors being much smaller. On the other hand, in terms of the effects of health on SES, differences in weight and sleeping behavior are more important than psychological distress.

[Profession-related disorders of the locomotor apparatus. A multi-causal problem?].

This article provides information on the prevention of disorders among dentists and dental hygienists, for instance neck, shoulders, upper extremity, wrist and hand disorders. Many daily (strained) operating postures are described, and research data from the Sonde prevention project is mentioned. This data is compared with the newly implemented NEN-ISO-standard 11226. The standard lists the limits of static working postures. It becomes clear from this data that many dentists exceed these limits daily and work in overstrained postures. The article will further go into the complicated interrelationship of and connection between various interactive factors which intentionally and often unintentionally lead to static use of muscles. It may be a matter of cumulative effects that are multi-causally related, with the possible consequences. A further awakening within the dental profession, conditioning of young students into adopting a correct operating posture and purchasing ergonomically sound equipment may contribute to a healthier situation.

[Melanoma while using levodopa due to Parkinson's disease--causal association unlikely].

A 75-year-old woman with Parkinson's disease presented with an amelanotic melanoma on her right lower leg with an inguinal lymph node metastasis. For the previous six years she had been treated with levodopa-carbidopa. The disease disappeared after inguinal lymph node dissection and hyperthermic isolated regional perfusion. It is sometimes thought that levodopa may have an activating effect on melanoma, due to its possible role in the biosynthetic pathway of melanin. However, on the basis of the literature available, such a causal association is unlikely.

Urbanisation in China: a review of its causal mechanisms and spatial relations.

This article reviews "the forces underpinning Chinese urbanization.... This paper is divided into two main parts. The first addresses the (non-spatial) causal mechanisms between 1949 and 1977. Neither the ideological, the class, nor the economic formulation has touched on the more systemic mechanisms related to the socialist state and the shortage economy. This paper attempts to redress the imbalance by examining the advantages of combining Kornai's shortage model with Foucault's concept of governmentality. By drawing on concepts of spatial contingency, spatial boundary and locality effects, the second part of the paper argues that spatial relations do play significant roles in revealing Chinese urbanisation policies and patterns."

[Selection for disease resistance in domestic ruminants and swine by indicator traits as well as marker and causal genes--a review. Part 1: Short introduction to immunogenetics of MHC and non-MHC genes and special immunogenetics of cattle and swine].

The introduction deals with immunogenetic investigations on the field of life-stock. The main chapter is outlined as a tabular overview of current opportunities of the application of indicator traits as well as marker and causal genes in breeding for disease resistance in cattle, sheep, goats and swine. In the discussion of the second part, emphasis was layed on diseases of small ruminants in central and western Europe. Indicator traits are discussed with respect of their advantages and disadvantages. The rigorous selection of specific traits is connected with an increase of the number of homozygotes. In contrary, pathogens do undergo mutations, thus escaping the host's immune system. Out of this point of view it is advisable, to set on selection very cautiously. The role of technologies of modern immunogenetics is pointed out in respect of constructing disease resistant animals.

Culture and judgment of causal relevance.

The authors hypothesized that because the causal theories of East Asians were more holistic and complex than those of Americans, the amount of information considered before making a final attribution would be larger for East Asians than for Americans. This hypothesis was supported through 4 studies. When participants attempted to explain a deviant behavior (Study 1) or a prosocial behavior (Study 2), Korean participants took into consideration a greater amount of information than did either American or Asian American participants. Study 3 replicated the findings of Studies 1 and 2 within each culture. Finally, Study 4 established a link between the present findings and past research on culture and attribution. Namely, Study 4 found that Koreans made more external attributions than Americans because Koreans considered more information than did Americans.

[Down syndrome and ionizing radiation: causal effect or coincidence].

Annual and monthly prevalence of Down's Syndrome has been analyzed in Belarus for a 19-year period (1981 to 1999). The distribution of 2786 DS cases during this period showed no significant long-term prevalence trend. Meanwhile, 2 marked increases of DS prevalence were registered in 1987 and 1990. The most remarkable annual increases were observed in 1987 in Minsk city, in Vitebsk oblast (considered as contamination-free administrative district) and in Minsk oblast (partially contaminated area), data not shown in the text. Much more marked and significant increases were recorded in January 1987, in Minsk city, Gomel oblast and Minsk oblast. However it is impossible to establish a causal relationship between these clusters of DS and exposure to the Chernobyl fallout, at a geographical level. The presence of such a cluster 9 months after the explosion in one of the most exposed area (Gomel oblast) should incite to careful conclusions and prompt further studies.
