[Treatment of forced weeping (author's transl)].

A derivate of gamma-aminobutyric acid was found to have a favourable therapeutic effect in patients with forced weeping (weeping sickness). All 10 treated cases were shown to have motor neuron lesions resulting from other cerebral changer (cerebrovascular accident, brain atrophy, diffuse vascular changes).

Recognition and management of Tourette's syndrome and tic disorders.

Tic disorders and Tourette's syndrome are conditions that primary care physicians are likely to encounter. Up to 20 percent of children have at least a transient tic disorder at some point. Once believed to be rare, Tourette's syndrome is now known to be a more common disorder that represents the most complex and severe manifestation of the spectrum of tic disorders. Tourette's syndrome is a chronic familial disorder with a fluctuating course; the long-term outcome is generally favorable. Although the exact underlying pathology has yet to be determined, evidence indicates a disorder localized to the frontal-subcortical neural pathways. Tourette's syndrome is commonly associated with attention-deficit/hyperactivity disorder, obsessive-compulsive disorder, behavior problems and learning disabilities. These comorbid conditions make the management of Tourette's syndrome more challenging. Management of Tourette's syndrome should include timely and accurate diagnosis, education, and behavior or pharmacologic interventions. Use of neuroleptic medications and dopamine D2 antagonist drugs can be effective but may be associated with significant side effects.

Eight fires in two months: Touro's encounter with arson.

Eight fires in two months at this major New Orleans hospital attracted the attention of the news media and arson investigators. The author provides an interesting look into the way this awkward situation can be handled by a hospital that has been stricken by an arsonist.

[Aggression].

The aggressiveness is not a drive and has nothing in common with Freudian death instinct, but it is one of the ways to fulfill the possession desires and the narcissistic desires. As the way to achievement, it comes into a conflict with the fear of failure, with the co-operation advantages and with the altruistic human drives. As the latter ones are rather weak in an average person, the co-operation--as a basis of society--is in all its strata imbued with aggression. Each social structure, up to the present day, has contained aggression as an organic component. The hope of creating a society free from aggression is not based on the betterment of the human character, but on the growth of the productive forces. The existence of the interpersonal aggression has been concealed from a child, brought up in the spirit of the centuries-old fallacious educational principles. As a result, the child develops its first neurosis--the Superego neurosis. The Superego is not a structural component of the human psyche, but it is a neurotic formation. In the subsequent years, the confrontation of the child and the youth with the true aggression may affect his or her development in various ways. In some cases, the given individual disregards the Superego commands and pays for it with the neurosis of permanent guilt feelings. In other cases, the individual deals with the Superego as with the ethical values in themselves, and develops an inhibition neurosis (Gehemmtheitsneurose). Still in other cases he or she revolts against the mendacity of the society and develops a pseudopsychopathy. The proper education should strive to awake the nobe feelings in the child, but at the same time to reveal the whole truth about the existence of aggression in the human interrelations. In these conditions, through the conscious choice between the egoistic and the altruistic drives, could the growing person form the moral attitudes fitting his or her own nature. Because the ethics of the values in themselves is but a philosophical fiction, the only real ethics is that which is based on feelings.

Sexual burglaries and sexual homicide: clinical, forensic, and investigative considerations.

Burglary, the third most common crime after larceny-theft and motor vehicle theft, is rarely the focus of forensic psychiatric study. While most burglaries are motivated simply by material gain, there is a subgroup of burglaries fueled by sexual dynamics. The authors differentiate two types of sexual burglaries: 1) fetish burglaries with overt sexual dynamics; and 2) voyeuristic burglaries, in which the sexual element is often covert and far more subtle. Many forensic practitioners have informally noted the relationship of burglaries to sexual homicide, but this relationship has not otherwise been studied in any detail. In this article, the incidence of (sexual) burglaries by 52 sexual murderers whom the authors evaluated, as well as the incidence in cases reported by others, is reported. Implications of these findings for forensic assessments and profiling of unidentified offenders are discussed.

Histiocytosis X and compulsive water drinking: report of a case.

We describe a patient with histiocytosis X and compulsive water drinking. The association of diabetes insipidus with histiocytosis X is well recognized, and this patient was initially considered to have diabetes insipidus. It was only after further testing that the proper diagnosis was made.

Catastrophic medical complication in psychogenic excoriation.

Compulsive skin picking generally has limited potential health consequences. We describe a case with severe neurologic sequelae and review the literature to assess factors that likely contributed to the failure to prevent this severe outcome. Despite efforts to integrate treatment, our patient had severe medical complications, an epidural abscess and subsequent paralysis, as a result of compulsive skin picking. Psychogenic excoriation should not be underestimated as a cause of medical complications or as a treatment challenge, particularly in light of the numerous complicating factors often present in such cases.

Compulsive spitting--a culture bound symptom.

Compulsive Spitting, as a culture bound symptom has not been previously reported in the literature. Of 26 cases described, 8 were suffering from schizophrenia followed by 5 cases having mania, 4 each with depression and OCD, 3 with tic disorder and 2 with seizure disorder. More studies are warranted to study and report the culture bound symptoms in india and other countries.
