In vitro effects of levamisole and ivermectin against Echinococcus granulosus protoscoleces.

The in vitro effects of levamisole and ivermectin against Echinococcus granulosus protoscoleces were studied by means of light and electron microscopy. Both drugs had a protoscolicidal activity that increased proportionally with increasing concentrations of the drugs. Ivermectin showed the more rapid effects and caused contraction and paralysis of protoscoleces. A paralyzing effect was also observed with levamisole, followed by irreversible tissue vacuolation leading to death.

Ticlopidine as a hemorrhagic risk factor in coronary surgery.

We retrospectively reviewed the postoperative hemorrhagic risk in patients undergoing surgery for coronary revascularization who had received platelet antiaggregants preoperatively. A significant increase in postoperative hemorrhage and a higher percentage of reoperations for this reason was observed in the patients treated with ticlopidine. However, there were no major differences in the platelet counts among the groups compared.

Hypoglycemic coma after cardiac surgery.

Hypoglycemic coma occurring 5 h after cardiac surgery in a nondiabetic adult patient was primarily triggered by: low cardiac output leading to severe hepatic functional derangement; metabolic acidosis, probably secondary to peripheral hypoperfusion; and possible inhibition of the adrenergic response due to anesthesia with fentanyl. Glucose infused at 8 g/h progressively increased the blood glucose level and paralleled hemodynamic improvement.

Pulmonary function and tissue oxygenation studies during anesthetic induction with etomidate.

The modifications of pulmonary function and peripheral oxygenation arising during anesthetic induction with Etomidate were studied in 36 unpremedicated patients programmed for elective surgery. The evaluation was carried out through measurement of ventilatory parameters and of the blood gases and oxygen saturation in arterial and mixed venous blood, calculation of the cardiac output, and derivation of data of peripheral oxygenation. A global reduction of the minute volume was observed with a minimal effect on the blood-gas values. The arterial blood oxygenation was reduced during the anesthesia time due to increased alveolo-arterial oxygen gradients and to increased intrapulmonary shunt. The tissular oxygenation data evidenced no significant alterations.

Acute pulmonary oedema due to lithium intoxication.

We present a case of acute lithium ion intoxication in which the main clinical feature was respiratory failure secondary to pulmonary oedema possibly due to myocardial depression, associated with profound stupor. The initial lithium ion concentrations were 3.15 mmol/l in plasma and 27.6 mmol/l in urine. Under symptomatic treatment with oxygen, digitalis and diuretics, both the pulmonary and the neurological disorders reverted gradually, in parallel with the decreasing plasma lithium ion concentrations.

Heart surgery in patients under haemodialysis.

Several cases of open-heart surgery on patients with severe chronic renal failure have been reported in the last few years. The present study reviews the main problems posed by this situation and analyses our recent experience of three successfully managed patients. Emphasis is made on preoperative preparation, drugs employed in anaesthesia, and postoperative management. We conclude that the prognosis of these patients is good, and that their management is quite similar to that of nephrologically healthy patients.

Paraplegia following balloon assistance after cardiac surgery.

Contrapulsation by means of an intra-aortic balloon is an effective and well-known therapeutic measure in the postoperative period after cardiac surgery, mainly when interrupting cardiopulmonary bypass in left ventricular failure situation (3, 4). We present the case of a patient who developed paraplegia 38 hours after surgery, which was attributed to contrapulsation.

Iatrogenic myocardial dysfunction after formalization of the heart.

A 20-year-old patient with myoepicardial echinococcosis was referred to our hospital for surgical treatment. After polycystectomy had been performed, the surgical area was mistakenly washed with formaldehyde solution, and severe myocardial dysfunction ensued. Histological alterations were compatible with toxic cellular damage.
