[Silver-containing hydrocarbonate-calcium-magnesium mineral water in complex treatment of patients with chronic pyelonephritis].

95 patients suffering from chronic pyelonephritis in active inflammation phase received antibacterial therapy with adjuvant drinking of silver-containing mineral water Serebryany klyuch (21 patients) or without such drinking (17 patients). Spa treatment with mineral water Serebryany klyuch in combined therapy of chronic pyelonephritis is shown to relieve clinical symptoms, decontaminate urine completely in 88.6% cases, to eliminate signs of immunodeficiency and endotoxicosis and to produce a 25.6% decrease in the recurrences for the nearest 2 years.

[Treatment results in urethral strictures].

A total of 231 patients with urethral strictures were treated. The disease resulted from trauma, adenomectomy, chronic urethritis, prostatic sclerosis in 92, 72, 51 and 16 patients, respectively. Fifty-one patients underwent conservative treatment which involved bougienage, resolving and anti-inflammatory agents. 38 of them responded, 13 nonresponders were operated on. 72 patients were subjected to partial urethral tunneling according to an original technique which brought success in 90% of the cases. Out of 92 traumatic urethral strictures 88 were cured after end-to-end plastic reconstruction. It is believed that urethral strictures should be managed individually basing on the stricture cause, location, length, severity, complications.

[Features of mental disorders in patients with chronic pyelonephritis].

To study prevalence of mental disorders in patients with chronic pyelonephritis (CP), 50 CP patients were examined clinically and with special tests (fundus of the eye, EEG. AMPI, Luscher's test). The patients were diagnosed to have a wide spectrum of mental disorders: psycho-organicsyndrome, paroxysmal disorders, hallucinations, obsessions depression, asthenic syndrome. The presence of psychoorganic syndrome, paroxysms, hallucinations point to developing organic disorders of the brain the severity of which correlates with the age. The depressions had a complicated genesis (psychogenic, somatogenic and organic). The results of the study can be used for design of therapeutic and rehabilitative measures improving quality of CP patients' life.

[The correction of the humoral and hemodynamic disorders in patients with urolithiasis complicated by chronic kidney failure].

The paper presents the analysis of 52 cases of urolithiasis complicated by chronic renal failure in the intermittent stage. The patients were examined for renal blood flow, central hemodynamics, myocardial function, factors of humoral regulation of the circulation kallikrein-kinin, sympathoadrenal systems, renin activity, renal function. The test were performed preoperatively and 1, 3, 7, 14 days after surgery. The patients entered two groups. 24 patients of group I received routine symptomatic treatment, 28 patients of group II were treated with regard to the changes revealed and underwent preoperative enterosorption. Group I demonstrated severe shifts in renal and central hemodynamics, reduction of renal circulation, hypokinetic circulatory syndrome, phasic syndrome of myocardial hypodynamics, hypoactivity of kallikrein-kinin and sympathoadrenal systems, hyperactivity of renin. The disturbances aggravated postoperatively and disappeared only on day 14 since the operation. having been corrected by cardiac glycosides, desaggregants, hypotensive drugs diuretics. Group II patients developed increased renal circulation, weak activation of humoral blood regulation factors. Hemodynamic disorders showed no progression. As a results, renal postoperative function improved.

[Extracorporeal shock-wave therapy in the treatment of Peyronie's disease].

The authors analyse the results of treatment of 28 patients with Peyronie's disease using extracorporeal shock-wave lithotripsy (ESWL) performed on Dornier U15 lithotriptor. A total of 2-6 sessions were made, maximal number--12. The efficacy was controlled by clinical indices and ultrasonic investigation (Doppler mapping of the blood flow). ESWL proved to be efficient in the treatment of Peyronie's disease (PD), primarily, in patients with early disease before appearance of severe fibroplastic alterations. Less plaque vascularization by energetic Doppler mapping due to ESWL is an important diagnostic criterion of PD treatment efficacy. Conservative treatment is not indicated in marked deformities and plaque calcification, erectile dysfunction. Moreover, any injection into the tunica albuginea, especially complicated by hematomas may be a damaging factor which triggers fibrous inflammation. Such patients should be treated surgically. If the patient is interested in immediate results or is not interested in continuation of sexual life, the treatment is prognostically uneffective. Thus, ESWL is an effective, safe method of PD treatment but requires further study and accumulation of clinical experience.

[Prostatic calculi: diagnostic and therapeutic aspects].

Strength properties of prostatic calculi (PC) were studied by microhardness HV. Fifteen calculi under study were divided into 3 groups by the size: 2-3 mm (group 1), 4-6 mm (group 2), > 6 mm (group 3). The surface of the thin sections of the calculi appeared as concentric, spherical layers of different size indicating formation of the calculus from the center. In the latter one could see a pigmented nucleus of organic nature. Some calculi had two nuclei under one cover--attached calculi. Correlations between the structure, morphology of prostatic calculi and their strength properties were established. These correlations allowed one to characterize destruction features of the calculi in different destructing impacts. We have designed and introduced into practice a complex of local physiotherapy in prostatolithiasis. This physiotherapy was conducted in combination with medicines for chronic prostatitis and included exposure of the prostate to ultrasound in impulse regime (4 mc, 0.4-1.0 W/cm2). The ultrasound was delivered through the rectum (insertion of the setting IUT 2.264-7-2.09u to the site of the concrements location) and alternated with laser therapy (insertion of the radiator P-2 into the rectum and radiator LO1 to the perineal area (5 W, 80 Hz, 64-128 s) and introduction of sulfide mud heated with the device PPK-01 Pra, electrostimulation for better passage of the prostatic section. The above therapy destructed the concrements which eliminated from the prostatic ducts as a result of which prostatic function recovered. The complex produced a good effect: 65% patients stated enhancement of the erectile function after 3 days of the treatment. After 5-6 procedures pain syndrome relieved in 40% patients, after 11-15 procedures in 87%. Ultrasonic investigation showed that posttreatment size of the prostate diminished from 0.8 to 0.2 cm. The absence of the calcinates's shadows were recorded in 87% patients, the symptoms of chronic prostatitis disappeared. The results of the combined physiotherapy demonstrate efficacy of the above method in true prostatoliths.

[Naiz (nimesulid) in combined treatment of patients with chronic non-bacterial prostatitis with chronic pelvic pain syndrome].

Naiz (nimesulid) was given to 46 patients (age 24-52 years) with chronic abacterial prostatitis suffering from chronic pelvic pain syndrome (CPPS) which appeared from 20 to 3 years before the treatment. Microscopy of prostatic secretion showed normal count of leukocytes. Transrectal ultrasound investigation with dopplerography has found normal echostructure of the prostate but blood flow parameters were subnormal. Naiz was given in a dose 100 mg (1 tablet) twice a day for 15-20 days. The treatment relieved pain in 68-87% patients (mean 77.5%). The control ultrasound investigation detected much better circulation. Quality of life improved significantly. Side effects were mild. The effect persisted for 3 months of the follow-up. Thus, the addition of such nonsteroid antiinflammatory drug as naiz (nimesulid) to treatment of patients with chronic abacterial prostatitis is justified. The drug improves quality of life, induces side effects rarely and can be used for a long time with a persistent efficacy.

[Use of cryoprecipitate in combined therapy of acute purulent pyelonephritis].

The treatment of 120 patients with acute purulent pyelonephritis (APP) brought about the following results. 86% patients given standard therapy showed deterioration of their condition, DIC syndrome and inflammatory process were not cured, pyoinflammatory process in the kidney continued. On the contrary, 84.1% patients on combined conservative treatment improved: DIC syndrome and active inflammation relieved, fibronectin content, IgA, IgM and IgG levels rose. Ultrasonic examination and dopplerography indicated that combined conservative therapy including cryoprecipitate arrested development of a purulent focus in the kidney, reestablished intraorganic circulation. Mean stay in hospital was 13.9 days.

[Efficacy of natural therapeutic factors of Belokurikha health resort in rehabilitation of patients with chronic prostatitis].

A total of 135 patients with chronic prostatitis have undergone rehabilitation in Belokurikha health resort. Of them, 35 patients of group 1 received basic therapy (nitrosiliceous baths, therapeutic exercise, massage of the prostate, psychotherapy); 40 patients of group 2 received basic therapy plus rectal tampons with sulphide mud from the lake Gorkoye of the Altai Administrative Area; 60 patients of group 3 were given basic therapy plus rectal lavage of the prostate location with nitrosiliceous thermal water of Belokurikha health resort. Treatment efficacy was assessed by scales of the symptoms and sexual disorders, ejaculate tests, testosterone levels, immune status, results of Doppler vascular mapping of the prostate. The highest effect was achieved in group 3 (87.3% response). This therapy relieves inflammation, stimulates a sexual function, improves hemodynamics in the prostate, raises the number and motility of spermatozoa, corrects immune disorders, normalizes testosterone concentration in the blood. In group 2 the response was obtained in 81.6% patients. This treatment improves hemodynamics, raises the number of spermatozoa. In group 1 the effect was observed in 60.4% patients, it manifested with an insignificant improvement in prostatic hemodynamics, spermatogenesis, hormonal and immune systems.

[Transrectal magnetotherapy of the prostate from Intramag device in prophylaxis of postoperative complications of transurethral resection of prostatic adenoma].

The authors analyse preoperative preparation of 91 patients with benign prostatic hyperplasia (BPH). Two groups of patients received conventional preparation (group 1) and magnetotherapy (group 2) before TUR of the prostate. The examination covered immune system, bacteriological indices of urine and prostatic tissue. Infection of the urinary tract is a main risk factor of complications after TUR. Conventional preoperative preparation fails to correct immunity, to change bacterial urine flora, to improve hemodynamics in the prostate. Transrectal magnetotherapy with running magnetic field eliminates deficiency of T- and B-cell immunity, raises functional activity of B-lymphocytes and phagocytic ability of neutrophils, reduces endogenic intoxication, tissue edema, bacterial contamination, number of thrombohemorrhagic complications. This leads to a decrease in the number of postoperative complications.

[Low-intensity laser radiation in preoperative preparation of patients with benign prostatic hyperplasia].

Low-intensity laser therapy administered in the form of intravenous blood irradiation, transrectal and transurethral prostatic irradiation and their combination as preoperative preparation and correction of immunity disturbances in patients with benign prostatic hyperplasia (BPH) were studied. The response to the treatment was evaluated by positive changes in the immune status and bacterial contamination of the urine and prostatic tissue. Conventional preoperative preparation (uroantiseptics, antibiotics and phytotherapy) fails to correct signs of T-cell immunodeficiency, depression of phagocytic activity of neutrophils, significantly reduce bacteriurea. Laser therapy as intravenous laser blood radiation acts immunomodulatorily on cellular immunity and normalized the proportion of T-helpers of the first and second order (T-suppressors) and neutrophil phagocytosis. The antibacterial effect of this technique on urinary microflora and prostatic tissue is not very high. Local laser therapy is a potent immunostimulator of T- and B-lymphocytes, increased the index of immunoregulatory cells' proportion, activated phagocytosis of neutrophils. It has pronounced antibacterial effect against gram-negative urinary microflora and tissue of the prostate. Combined laser therapy produced the highest immunomodulating action on T-lymphocytes and immunostimulating one on B-lymphocytes, potentiated phagocytic ability of neutrophils, elevated index of the immunoregulatory cells, but was unable to correct their imbalance completely. Antibacterial effects of combined laser therapy were the highest, including the bacterial group Proteus-Providencia. Preoperative low-intensity laser therapy of BPH reduced the number of postoperative pyoinflammatory complications, hospital stay, severity of postoperative period.

[The treatment of the disseminated intravascular coagulation syndrome in urosepsis patients].

The hemostasis system has been assessed in 42 patients in the age range of 18 to 60 years with secondary urosepsis. All patients had been operated for upper urinary tract obstruction or suppurative destructive renal and retroperitoneal lesions. Hemostasis was examined before and after the operation using coagulation screen. Middle-molecular peptides were assayed in the patients before and after plasmapheresis. Revealed hemostatic disorders were treated with heparin, antibacterial agents and infusion of fresh frozen plasma. Plasmapheresis is indicated in patients who are unresponsive to antibacterial therapy and surgical drainage of suppurative lesions. Postoperative hemostatic disorders presented as both hypo- and hypercoagulation, with thrombinemia in most cases. The patients with urosepsis showed latent hypercoagulation phase of DIC. By removing acute phase proteins, plasmapheresis prevented fibrinolytic failure. Exfusion of small volumes of plasma did not affect antithrombin III levels and produced a coagulation-anticoagulation balance. Declining levels of middle-molecular peptides after plasmapheresis were indicative of detoxication, presenting as better clinical status and improvement of laboratory findings.
