Disseminated intravascular coagulation and bacterial infections in the elderly.

Anticoagulant screening tests (prothrombin time, thrombin time, kaolin-cephalin time and serum fibrinogen levels), platelet count and estimation of fibrinogen degradation products were carried out on 34 elderly patients with proven bacterial infections, 42 elderly patients with suspected bacterial infections and in 20 controls, to investigate the existence of a sub-clinical form of disseminated intravascular coagulation. The prothrombin time was significantly prolonged in bacterial infections of the elderly and subclinical intravascular coagulation was not found to be a feature. The screening tests did not show any correlation with the outcome of the disease or the presence of septicaemia.

Mitral valve prolapse in a patient with epidermolysis bullosa.

Mitral valve prolapse has been associated with several connective tissue and developmental disorders, including Ehlers-Danlos syndrome, Marfan's disease; pseudo-xanthoma elasticum, myotonic dystrophy, cardiomyopathy, Ebstein's anomaly of the tricuspid valve and atrial septal defect of the ostium secundum variety. Presented here is a case of mitral valve prolapse in a patient with epidermolysis bullosa. The author is unaware of any previously reported cases of this association.

Locked in state. A clinicopathological study.

Two patients developed the locked-in state characterised by quadriplegia and mutism with an alert sensorium. Initially they had mild dysarthria and uncrossed hemisensory or hemimotor deficits involving the face and ipsilateral extremities. Both patients died. Rostral brainstem infarctions were found at autopsy in them. Acute onset of uncrossed hemisensory and hemimotor deficits with dysarthria may be caused by infarction of the pons which may predispose to locked-in state.

Hypotension and bradycardia after intrathecal ketanserin and phentolamine in the rat.

Intrathecal administration of ketanserin and phentolamine elicited dose-dependent falls in arterial blood pressure and heart rate in anaesthetized rats without attenuation of the pressor responses to i.v. injections of phenylephrine. It is concluded that both ketanserin and phentolamine can decrease arterial blood pressure by acting within the spinal cord.

Gliosarcoma with cartilage formation.

A case of gliosarcoma with cartilaginous component is described. Immunohistochemical and electron microscopic studies confirmed the presence of glial and fibroblastic elements. A major part of the sarcomatous tissue was undifferentiated and not labeled by any of the markers used including those for endothelial cells. The cartilage cells also were not labeled either by antiglial fibrillary acidic protein or any other marker. The occurrence and histogenesis of cartilage in gliomas and gliosarcomas have been reviewed.

Complications of parotid surgery: the need for selectivity.

The morbidity rate following 112 operations on the parotid gland is reported. Facial nerve palsy (temporary, 38 per cent; permanent, 9 per cent) and Frey's syndrome (11 per cent) were common following superficial parotidectomy. For pleomorphic adenoma and malignant lesions this procedure is justified. Benign conditions, which can be diagnosed preoperatively, should be treated by more limited surgery which has a lower complication rate.
