Densitometric and chromatin texture parameters of hepatocyte nuclei during controlled autolysis.

OBJECTIVE: To determine the extent of the effect of autolytic changes in hepatocyte nuclei on the results of measurements of integrated optical density (IOD) of nuclei stained by the Feulgen method and its possible relationship with changes in chromatin texture. STUDY DESIGN: Cytologic smears obtained from five pig livers by means of fine needle biopsy were fixed in 10% formalin at six time points after slaughtering (0-18 hours) and stained per Feulgen. In the hepatocyte nuclei IOD measurements were performed and supplemented with quantitative texture analysis. Texture analysis was carried out with our own algorithm, based on the movement of global gravity centers of heterochromatin granules during two-level, iteratively changed, grey thresholding. RESULTS: The variability coefficients of IOD of diploid hepatocytes during autolysis increased from 0.061 (0 hours) to 0.363 (18 hours), and the ploidy histograms became aneuploid. The texture of chromatin became more homogeneous, but other features of the texture did not indicate a significant correlation with the duration of autolysis. CONCLUSION: Autolysis is an important factor, which may influence the accuracy of DNA content and shape of ploidy diagrams. Changes in the morphologic pattern of heterochromatin may play a role in this, but the study was not conclusive because only one of six texture parameters (number of granules) changed parallel to mean IOD and autolysis time.

Diagnostic value of fine needle aspiration biopsy for breast fibroadenoma diagnosis.

The diagnostic value of fine needle aspiration biopsy was estimated in the study. Cytological features, which allow us to diagnose and distinguish fibroadenoma from other breast benign lesions, were also examined. The diagnostic effectiveness was estimated depending on the observance of the described criteria (features) characteristic for the breast fibroadenoma by a comparative analysis of cytological findings in three diagnostic centers.

The effect of somatostatin analog octreotide on diethylstilbestrol-induced prolactin secretion, cell proliferation and vascular changes in the rat anterior pituitary gland.

The effects of diethylstilbestrol (DES) and of long-acting somatostatin analog, octreotide (SMS) on the rat anterior pituitary microvasculature have been studied by means of computer-assisted image analysis. Additionally, the effects of DES and SMS on prolactin secretion and anterior pituitary cell proliferation have been studied, as well. The vascularization was visualized using Selye's method modified by Poely et al. (1964). The prolactin serum levels were estimated by radio-immunoassay. The proliferation indices were assessed using bromodeoxyuridine incorporation assay. As expected, it was found that DES sharply increased serum prolactin levels and enhanced cell proliferation in the anterior pituitary gland. DES also induced changes in parameters of vascularization. Simultaneous treatment of rats with SMS inhibited the DES-induced elevation of prolactin levels and pituitary cell proliferation. It also suppressed some but not all DES-induced changes in the anterior pituitary vascularization. These data suggest that the angio-inhibitory activity of SMS might be involved in its anti-tumor action on pituitary adenomas, but not as a sole or principal mechanism.

Inhibitory effects of fumagillin and its analogue TNP-470 on the function, morphology and angiogenesis of an oestrogen-induced prolactinoma in Fischer 344 rats.

The process of angiogenesis occurs in many physiological states, but it is also essential for the growth of solid tumours and metastasis formation. An abnormal arterial vascularization has been shown in prolactin-secreting pituitary adenomas induced by prolonged treatment with oestrogens in Fischer 344 (F344) rats. It is thought that anti-angiogenic agents might be useful in therapy for these tumours. Fumagillin and its analogue TNP-470 are known to inhibit endothelial cell proliferation selectively, but their effect on lactotroph cell secretory function and prolactinoma formation has not yet been described. The aim of the present study was to examine the effects of fumagillin and TNP-470 on prolactin secretion, and morphological and vascular changes within the anterior pituitary in long-term oestrogen-treated male F344 rats in vivo and in vitro. As expected, 7 weeks after s.c. implantation of Silastic tubes containing 10 mg diethyl-stilboestrol (DES), a very high rise in serum prolactin levels was found. Both angiogenesis inhibitors injected s.c. at doses of 10 mg/kg body weight for 24 days attenuated the stimulatory effect of DES on prolactin production and release. They also diminished prolactin cell density and inhibited cell proliferation expressed as the number of anterior pituitary cells labelled with bromodeoxyuridine (BrdU), but the effect of TNP-470 was minor compared with fumagillin. Both angioinhibitors suppressed neo-vascularization within the anterior pituitary with similar potency but, on the other hand, they did not affect DES-induced increases in prolactin secretion from cultured rat pituitary cells and cell proliferation in vitro. In conclusion, our results provide strong evidence for the anti-tumour and anti-prolactin activity of angiogenesis inhibitors in the experimentally oestrogen-induced pituitary adenoma; this might be mediated indirectly through the inhibition of angiogenesis.

[Pleural needle biopsy in diagnosis of pleural effusion].

Diagnostic reliability of percutaneous parietal pleural biopsy with use of the Abrams needle technique performed over a period 1989-1994 was assessed in 37 patients with pleural effusion of unknown etiology, aged 24-82 years. Adequate diagnostic specimens were obtained in 33 patients. Histopathological diagnosis established neoplasma (39,4%), tuberculosis (9.1%) and chronic nonspecific pleuritis (51,5%). No complications of the procedure were encountered. Pleural needle biopsy should be more widely applied as a basic method in the diagnosis of pleural effusions of unknown etiology.

Helicobacter pylori infection in the etiopathogenesis of duodenal ulcer in children.

The study evaluates the frequency of Helicobacter pylori (H. pylori) infection, as well as systemic cellular immune response to H. pylori in children with duodenal ulcer (DU). The study group comprised 47 children with DU, aged 6-17 (mean 13, 1 +/- 4, 2). H. pylori detection was based on urease test, histology, culture and serologic tests. Endoscopic and morphologic findings were analysed according to Sydney System criteria. In 12 children from the overmentioned group subsets of blood lymphocytes B and T (CD3, CD4, CD8, CD3/DR, CD19) and NK cells, some neutrophils functions (phagocytosis, chemiluminescence) and phagocytes receptors before and one month after H. pylori triple treatment were investigated. H. pylori infection was detected in 44 of the investigated children. In addition, pathologic examination revealed chronic gastritis in 44 children and chronic duodenitis in 42 of them. In immunosystemic examination decreased percentage of CD8 lymphocytes and NK cells, increased CD4/CD8 ratio, decreased mitogen-induced response and changes of function and receptor expression of neutrophils were found. After H. pylori treatment in follow-up endoscopy no ulcers were found and histologic examination did not reveal chronic active gastroduodenitis, while the rate of nonactive gastritis was increased. Eradication of H. pylori infection in 41 children and normalisation of immune parameters in 11 children were obtained. The results of our investigation indicate, that H. pylori infection plays an important role in the pathogenesis of DU in children.

The effect of angiotensin II receptor antagonists on diethylstilbestrol-induced vascular changes in the rat anterior pituitary gland: a quantitative evaluation.

The effects if diethylstilbestrol (DES) and of angiotensin II (Ang II) receptor antagonists, such as losartan (selective AT1 receptor antagonist) or PD 123319 (selective AT2 receptor antagonist) on the anterior pituitary microvasculature were studied by means of computer-assisted image analysis. The vascularization was visualized using Selye's method modified by Poely et al. (1964). It was found that DES induced a sharp increase in vessel area, mean vessel diameter and perimeter, whereas mean vessel number was reduced. These DES-induced changes were inhibited by simultaneous administration of losartan. On the other hand, PD 123319 was less effective. These findings suggest an involvement of Ang II, acting mainly via AT1 receptors, in the mechanism of estrogen-induced vascular changes in the rat anterior pituitary gland.

[Chronic gastritis and ulcer disease in young men].

Examination has been carried out on 34 young men (average age: 22 years) with chronic erosive gastritis. Chronic erosions were situated most frequently in the prepyloric part of the stomach (88.2%). Duodenal ulcer-was found in 14 patients (41.1%), gastric ulcer was found in 1 patient (2.9%). In 19 examined patients (56.0%) chronic erosive gastritis existed without any other disease of the gastrointestinal tract. Clinical picture, gastric secretion and histopatological examination were compared in two group: I-chronic erosive gastritis without any other changes within the gastrointestinal tract and group II-chronic erosive gastritis coexisting with duodenal ulcer in the clinical picture of the group I, pain usually occurred after a meal and was felt as a heaviness sensation in epigastrum, while in the group II patients felt suction, hunger pain night pain and heartburn. Gastric secretion in the two group did not differ much. Microscopic examination of the mucosa usually confirmed chronic active gastritis. A typical feature for both group was regenerative glandular hyperplasia of the gastric mucosa. Despite many common features, it is impossible to identify explicitly and univocally the relationship between chronic erosive gastritis and ulceration.

[Logistic model in diagnosis of lung cancer].

Specificity of the diagnosis of lung cancer (l.c.) based upon clinical and radiological criteria was investigated in patients who died in Hospital of Lung Diseases in Lódź between in 1985 and 1989. Autopsy revealed diagnostic errors in 37% cases. Twenty most common disease features were taken into consideration. Logistic analysis helped to create clinical-mathematical model which allows for interpretation of disease features in two distinct categories: l.c. (non-neoplasmatic lung disease). Eight features proved to be significant (changes in RBC, weight loss, chest pain, increased sputum production, haemoptisis, hilar tumor, pleural effusion, peripheral round shadow on chest X-ray). Diagnostic usefulness of the model was checked in prospective study a group of patients who died from properly diagnosed l.c. (499 cases). Our model allowed for proper diagnosis of neoplasmatic cause of the disease with higher probability than morphological methods (including both intravital and autopsy examination).

T lymphocytes and immunoglobulins in small and large intestinal mucosa in children with ulcerative colitis.

In 52 children, aged 4-16 years, with ulcerative colitis, behaviour of A,M,G,E globulins and T lymphocytes in the small and large intestinal mucosa was evaluated. The investigations were performed both in the acute phase and remission of the disease. In the final stage of the disease, numerous plasma cells with intensive reaction to IgA and IgM were found in the stroma, while on the epithelial surface they were less marked. An intensive reaction to IgA was observed in remission. Reaction to T lymphocytes was found in single mononuclear stroma cells both in the acute phase and in remission.

Morphological and immunohistochemical examinations of the dynamic changes of gastric mucosa associated with the treatment of helicobacter pylori infection in children.

The aim of this study was to investigate the consequence of Helicobacter pylori eradication on gastric mucosa and antral G and D-cells. Forty children, aged 5-17 years with Helicobacter pylori infection were assessed. Helicobacter pylori was detected by a urease test and identified by serological and microbiological methods. Twenty children were again assessed after the therapy (the combination of colloid bismuth subcitrate, amoxycillin and metronidazole). Gastroscopic examination was performed and at least six bioptic specimens were taken from the antrum, body and fundus. Tissue samples, processed with the paraffin method and stained with hematoxyllin and eosin, were assessed. Monoclonal antiserum Gastrin PAP kit 516 and somatostatin PAP kit 512 (DAKO) in the peroxidase-antiperoxidase technique (PAP) have been used to detect G and D-cells. Helicobacter pylori in the gastric mucosa was demonstrated with the Giemsa method. The results show the coincidence of Helicobacter pylori infection and the count of antral G and D-cells and active chronic gastritis in children. After the treatment Helicobacter pylori was eradicated in 70% of children. In 34% of these cases the eradication was followed by a diminution of activity of gastric antral mucosa inflammation and in 20% of these children the resolution of the inflammatory infiltration in the gastric mucosa was seen. A decrease of the antral G and D-cells count and also a diminution of G/D index in these cases were observed.

Chronic erosive gastritis on morphological analysis.

The present study was carried out to systematize the histopathological markers of chronic erosive gastritis referred to also as gastritis varioliformis (GV). The study population consisted of 74 patients aged 20 to 69 years with gastroscopic GV. The samples were obtained from the erosion site and representative areas of the prepyloric region and gastric body. In routine microscopic examinations we evaluated the severity, type and location of changes in the gastric mucosa. Helicobacter pylori in tissue samples was identified with Giemsa's staining. Gastroscopic GV is accompanied in 76% by microscopic gastritis. In the vicinity of GV the gastric mucosa shows inflammation signs but less severe. Regenerative glandular hyperplasia of the gastric pit layer is the most frequent microscopic marker of GV.

Histomorphometric investigations of the human heart microvasculature in coronary disease.

Comparative results of morphometric investigations describing the microvasculature of the human left ventricle heart wall muscle have been presented. The control group consisted of 11 autopsied cases of the atherosclerotically non damaged hearts and the second group consisted of 13 autopsied cases of the hearts with double or triple vessel chronic coronary atherosclerosis. The age of subjects ranged from 47 to 85 (median 65). The study was carried out on microscopical slides using digital image analyser. In the control group a significantly lower number and profile area of capillaries was noticed in the subendocardial zone than in the remaining ones. The differences between control and atherosclerotic hearts were most prominent in the subendocardial zone, although they were observed in the subepicardial and intramyocardial zones as well. If the value of each parameter in the subendocardial zone of the control hearts will be fixed equally to 100%, the corresponding values in atherosclerotic group are: the number of capillaries--34%, total area of capillaries--65%, number of collaterals--170%, total area of collaterals--612%, muscle fibre/capillary ratio--137%, the degree of microvasculature anisotropy--164%. On the ground of this study as well as on the qualitative investigations it is postulated that the chronic atherosclerosis of extra-myocardial coronary arteries corresponds to intramyocardial rebuilding of the capillary network worsen the conditions of muscle fibre nutrition.

Some ultrastructural aspects of Helicobacter pylori gastritis.

Ultrastructural examinations of biopsy specimens of the gastric mucosa were performed in 20 patients with chronic gastritis proven by endoscopy and microscopy. The presence of the Helicobacter pylori bacteria was found in close contact with epithelial cells of the antrum and corpus of the stomach. The bacteria were not present in the areas of frequently observed intestinal metaplasia. There were ultrastructural changes on the surface and within the cytoplasm of the epithelial mucous cells, which indicated their disturbed metabolism.

Dietary risk factors in intestinal and diffuse types of stomach cancer: a multicenter case-control study in Poland.

A hospital-based, multicenter, case-control study has been performed in Poland covering 741 incident stomach-cancer cases (520 males and 221 females) and the same number of controls. All stomach-cancer diagnoses were evaluated for histologic type according to the Lauren criteria. Fifty-one percent were of the intestinal type, 35 percent of the diffuse type, and 8.5 percent of the mixed type. The frequency of consumption of individual food items and several food groups was analyzed and the association of various foods with stomach cancer risk was evaluated after controlling for sex, age, occupation, education, and residency. Increased consumption of sausages was related significantly to gastric cancer risk, whereas increased consumption of cheese products, nonwhite bread, vegetables, and fruit was associated with decreased risk. A particularly strong decrease in risk was associated with consumption of radishes and onions. When consumption of fruits and vegetables, sausages, nonwhite bread, and cheese were introduced simultaneously in a multivariate model, independent effects were found only for fruit and vegetables, sausages, and nonwhite bread. The use of table salt, the frequency of eating hot meals, and an irregular eating pattern were also associated with increased risk, while additional consumption of fruit between meals showed reduced risk. If a reduction in vegetable and fruit consumption took place after marriage, an increased risk for stomach cancer was found, whereas augmented consumption of these food items after marriage decreased the risk. Separate risk models were calculated for stomach cancer of the intestinal and diffuse types, but both histologic varieties showed the same pattern of associations with dietary risk factors.

The toxicodynamics of benzene, ethanol, and benzene plus ethanol based on the histopathological examination of selected organs in the rat.

The influence of ethanol on benzene toxicity in the course of prolonged 26-week intoxication was investigated in Wistar rats. A dose of 1/30 DL50/24h of benzene in oil solution was administered subcutaneously once daily, 5 days a week. Drinking water was replaced by a 10% water solution of ethanol ad libidum. Specimens of the liver, spleen, kidneys, lungs, and heart were taken for histopathological examination. Changes in the liver confirmed the hepatotoxic action of benzene and were unaffected by the simultaneous administration of ethanol. In the lungs and spleen the lymphatic system showed signs of damage which became more pronounced in joint exposure to benzene plus ethanol. The authors conclude that alcohol abuse by people professionally exposed to benzene and other lipophilic petroleum derivatives (as in the petrochemical industry) may increase the toxic effects of these substances.
