[Inclined plate screws - in vitro measurement of the stability of transverse osteotomy of tibia].

A compression plate which is exactly contoured to the bone surface produces asymmetric compression: only part of the cortex near the plate is compressed. Therefore a compression plate is not meant to be used selfstanding, it will rather be combined with additional means. Two of these have been studied: the increased stability provided by the use of lagging, inclined plate screws was compared to the one provided by the spring effect of a plate prebent for central elevation. 7-hole dynamic compression plates (DCP) and 4.5 mm cortex screws were used to stabilize transverse osteotomies in human tibiae in vitro. Flexural and torsional testing revealed that the use of an inclined lag screw or a prebent plate increases stability compared to the one achieved with an exactly contoured plate alone. Preliminary tests suggest, that a combination of plate lag-screw and prebending offers stability up to high loads.

[Preoperative immunosuppression in the kidney-transplanted dog in connection with specific antigenic pre-treatment].

Postoperative, nonspecific routine immunosuppressive therapy fails in up to 50% in human cadaver renal transplantation. Donorspecific preoperative immunological conditioning of the host is harder to induce in dogs than in some rodents. In a pilot study 2 i.v. donorblood injections on days -18 and -11 followed by a 6-day preoperative pulse without any postoperative therapy induce a significant albeit limited prolongation of graft function.

[Blood transfusion and kidney transplantation].

The non-transfusion policy for potential recipients of kidney allografts that has been followed in recent years has not proven successful. There is now ample clinical evidence of improved transplant function in the transfused recipients category. This has also been confirmed in recipients of a first cadaver kidney in Basel and Geneva. If the conditions valid for human kidney transplantation are matched as closely as possible in animal experiments, prolongation of graft function rather than accelerated rejection is usually observed after blood transfusions. Potential kidney graft recipients should be more liberally transfused and experimental evidence ought to be accumulated in order to establish a rationale for optimal host conditioning, while avoiding the hazards of blood transfusions to the greatest extent possible.

[The technic of tracheotomy: experience with 250 tracheotomies].

The results of 250 tracheostomies, primarily or secondarily performed at the intensive care unit of the Surgical Department of the Basle University, are presented. The standardized technique of tracheotomy is described, details for the choice and management of large prestretched double-cuff tracheostomy tubes are given, a new method of low-pressure cuff-blocking is outlined. The follow-up studies --autopsy, clinical examination and tracheoscopy--demonstrate a very low complication rate, due to the technique of tracheostomy, the preferred tracheostomy tubes and the method of alternate low-pressure cuff-blocking.
