[Application of high frequency ultrasound to the diagnosis of varicocele in infertile males].

OBJECTIVE: To investigate the objective index of the diagnosis of varicocele (VC) in infertile males by ultrasonography and the testis volume changes resulting from varicocele. METHODS: Forty-six healthy male volunteers and 178 infertile men with left varicocele were detected by high frequency ultrasound. According to the clinical data and ultrasonographic results, the 178 VC patients were divided into 4 groups, SVC group (45 cases), VC I group (44 cases), VC II group (48 cases), and VC III group (41 cases). RESULTS: (1) The differences in DR, DV, Vmax, TR and testis volume between the right and the left sides were not statistically significant in the control group (P > 0.05). (2) The differences in DR, DV, Vmax and TR between the experimental and control groups as well as among the VC groups were statistically significant (P < 0.001). (3) The left testis volume was smaller than the right among the VC groups (P < 0.01). The right testis volume of the VC II and VC III groups was significantly smaller than that of the control group ( P < 0.05), and the left testis volume in the VC III group was significantly smaller than that of the SVC group (P < 0.05). CONCLUSION: (1) High frequency ultrasound can detect the accurate diameter of the internal spermatic vein, hemodynamic index and testis volume of infertile men with VC, and hence help to screen the causes of male infertility. (2) Unilateral varicocele can cause a volume decrease in both testes, especially in the left. Both unilateral SVC and VC can cause testicular atrophy, and the more serious VC, the higher testicular atrophy.

Effects of recombinant human growth hormone on intestinal translocation of bacteria and endotoxin in rats with obstructive jaundice.

BACKGROUND: Extrahepatic biliary obstruction promotes intestinal translocation of bacteria and endotoxin and this process is an important cause of morbidity and mortality in patients with jaundice. This study was undertaken to investigate the effect and mechanism of recombinant human growth hormone(rhGH) and to alleviate intestinal translocation of bacteria and endotoxin in murine obstructive jaundice. METHODS: A group of 42 Wistar rats were divided into 3 groups:sham operation(SO), bile duct ligation (BDL), and BDL and rhGH treatment(rhGH). By the end of the experiment, on day 7, the animals were killed, and their liver function and serum endotoxin were measured, bacterial cultures of the liver, kidney and mesenchymal lymph were made. Terminal ileum mucosa was observed under an electron microscope. RESULTS: Liver function was improved more significantly in the rhGH group than in the BDL group. The value of endotoxin in the rhGH group was 0.38+/-0.03 EU/ml, significantly lower than that in the BDL group(0.65+/-0.04 EU/ml, P < 0.01), and similar to that in the SO group (0.30+/-0.02 EU/ml, P > 0.05). The rate of bacteria translocation in the liver, kidney and mesenteric lymph was much higher in the BDL group than in other two groups. The rate of bacteria translocation in mesenteric lymph was 64.29%, significantly higher than that in the SO group and the rhGH group (P < 0.05). There was no significant difference in bacteria translocation rate between the SO group and the rhGH group (P > 0.05). Under an electron microscope, ileum mucosa epithelial cells in the BDL group were necrotic, and organelle were markedly metamorphic. In the rhGH group, ultrastructural changes were less evident or similar to those in the SO group. CONCLUSION: rhGH has significant protective effects on intestinal mucosa barrier in obstructive jaundice, and reduces intestinal translocation of bacteria and endotoxin.

[Molecular diversity of the gene encoding Fe(II)-oxidizing enzyme in Acidithiobacillus ferrooxidans].

Acidithiobacillus ferrooxidans (A.f) is a Gram-negative chemolithotrophic bacterium able to oxidize ferrous iron, elemental sulfur and inorganic sulfur compounds. Fe(II)-oxidizing enzyme plays key role in Fe(II)-oxidizing system. Different A.f strains, which were isolated and purified from various ecological niches, show obviously phenotypic diversity after comparison rates of growth and oxidation of Fe2+. The Fe(II)-oxidizing enzyme gene(iro) for five strains was identified by PCR reactions uitilising primers and genomic DNA, then sequenced and multialignment. The results show there is a putative high variable region in CDS between the 187th and 195th bp and the sequence similarities are from 97% to 99%. For strain YTW, P187-189 Thr-->Pro; For P193-195, Met-->Asn for strain TK; Met-->Ile for strain BY; For P219, all strains T-->C. For upstream of CDS, sequences of all strains are identical to the Genebank sequence (E03451), but downstream of CDS, the sequences exist some variable sites too.

[Study on the molecular epidemiology of Mycobacterium tuberculosis in Shanghai].

OBJECTIVE: To explore the molecular-epidemiology of Mycobacterium tuberculosis in Shanghai. METHODS: Drug-resistant and drug-susceptible strains of M. tuberculosis were randomly selected from the bank of M. tuberculosis of Shanghai Municipal Center for Disease Control and Prevention and were genotyped by mycobacterial interspersed repetitive units(MIRU) and Spoligotyping methods. The genotyping results were analyzed and combined with epidemiological data. RESULTS: The Spoligotyping results demonstrated that 89 % (81/91) of the strains belonged to the Beijing genotype. Of the patients who had received BCG-vaccination,88.5% (54/61) infected with strains of Beijing genotype and 90.0% (27/30) of the patients were not BCG-vaccinated. However, the difference was not statistically significant. Drug-resistant rate from those strains of Beijing genotype was 45.7 (37/81), lower than that of non-Beijing genotype (60.0% ,6/10). Again, the difference was not statistically significant. The MIRU results showed that 62.6 % (57/91) were strains of clusters. CONCLUSION: The Beijing genotype of M. tuberculosis were found to be the dominant strains in Shanghai. The associations between Beijing genotype strains and BCG vaccination or drug-resistant were not found. Results from cluster analysis suggested that some cases might belong to the newly developed cases.

Morphological evolution in the electrodeposition of the Pb-Sn binary system.

Morphological evolution in the electrodeposition of Pb-Sn binary system is studied. As the second component increases, the morphology of the codeposit changes from dendrite to ramification, to dense branch, and finally to fractal structure, respectively. The evolution arises from the influence of crystallographic texture, which leads to a splitting of dendritic tips and the formation of ramified morphology. This work provides direct evidence to explore the crystallographic influence on the morphological evolution in electrodeposition.

[Induction, purification and antifungal activity of beta-1, 3-glucanase from wheat leaves].

Treatment with mercuric chloride (0.01%), salicylic acid (10.0 mg/mL) or riboflavin (1 mmol/L) induced the beta-1, 3-glucanase activity in all the three wheat varieties i.e. 331, Kangdao 680 and Lumai 23 tested, with the strongest inductive effect on variety 331 by treatment with mercuric chloride (0.01%) for 24 h. From leaves of variety 331 treated with mercuric chloride (0.01%) for 24 h, a kind of beta-1, 3-glucanase was purified by fractional precipitation with ammonium sulphate, Phenyl-Sepharose chromatography (Phenyl-Sepharose Fast Flow), ion-exchange chromatography (DEAE-Sepharose Fast Flow) and gel-filtration chromatography (Sephacryl S-100). Through SDS-PAGE and gel filtration, the molecular weight of the purified beta-1, 3-glucanase was determined to be about 52.0-53.6 kD. The purified beta-1, 3-glucanase showed antifungal activity against both Alternaria longipes and Rhizoctonia cerealis on tested plates, and inhibited the germ tube elongation and spore germination of Verticillium dahliae and Fusarium omysporum f.sp cucumerinum.

Achieving differentiation of single-base mutations through hairpin oligonucleotide and electric potential control.

A novel assay for surface DNA hybridization, which is free of sample and probe labeling, convenient and of low cost, sensitive and capable of differentiation of single-base mutations, is reported. Hairpin oligonucleotides are carefully designed as probes and are covalently attached to Si chips. Segments of the human p53 gene are chosen to demonstrate the major features of the novel technique. Impedance measurement is used to detect the hybridization. To further optimize the performance, electric potential is applied on the chip. The apparently different responses of the chip to the complementary strand and the single-base mutant are shown under electric potential control. The criteria on the design of the hairpin oligonucleotides are discussed.

Monitoring DNA hybridization on alkyl modified silicon surface through capacitance measurement.

Single strand oligodeoxynucleotide is attached to the alkyl modified silicon surface through a peptide bond. The oligodeoxynucleotide-modified silicon substrate is used as a working electrode in an electrochemical cell system. After the electrode is treated by a solution containing strands of complementary oligodeoxynucleotide the Mott-Schottky measurements exhibit obvious negative shift in the flat band potential of the electrode, while in a control experiment treated with a solution of non-complementary oligodeoxynucleotide such a shift does not occur. The DNA hybridization is also manifested in a real time capacitance measurement. A DNA sensor based on the capacitance measurement could be more convenient than that based on a fluorescence detection.

Relation of ultrasonic tissue characterization with integrated backscatter to contractile reserve in patients with chronic coronary artery disease.

BACKGROUND: Previous studies have shown that viable but stunned myocardium displays contractile reserve and exhibits cardiac cycle-dependent variations of integrated backscatter (CVIB), whereas infarcted myocardium does not. HYPOTHESIS: This study was designed to clarify whether assessment of the acoustic properties of the myocardium can predict contractile reserve in patients with chronic coronary artery disease (CAD). METHODS: In all, 21 patients with chronic CAD and 19 normal control subjects were studied. The magnitude of CVIB of the myocardium was measured in the basal and mid segment of the anterior septum and posterior wall of the left ventricle, using a real-time, two-dimensional integrated backscatter imaging system. The results were compared with the percent systolic wall thickening and the wall motion before and after revascularization. The wall motion was graded as normal, hypokinetic, or akinetic, and contractile reserve was considered present when an akinetic or hypokinetic segment improved after revascularization. RESULTS: The average magnitude of CVIB was lower among dysfunctional segments of CAD than among normal segments of controls (3.73 +/- 1.71 vs. 6.35 +/- 0.69, p < 0.001). Of the 77 segments examined, 38 showed reversible dysfunction. Before revascularization, percent systolic wall thickening was similar among segments showing contractile reserve compared with those with persistent dysfunction myocardium (17.97 +/- 8.41 vs. 16.83 +/- 6.37%, p = 0.19), and the mean CVIB was significantly greater in segments with than in those without contractile reserve (4.73 +/- 1.47 vs. 2.75 +/- 1.31, p < 0.001). The CVIB above 3 dB before percutaneous transluminal coronary angioplasty predicted segments with contractile reserve with a sensitivity and specificity of 84.2 and 79.5%, respectively. CONCLUSIONS: Cardiac cycle-dependent variations of integrated backscatter reflected myocardial contractility and functional capacity of the myocardium. They predicted segmental contractile reserve in patients with CAD.

Molecular and phenotypic heterogeneity in mitochondrial trifunctional protein deficiency due to beta-subunit mutations.

The mitochondrial trifunctional protein (TFP) is a multienzyme complex of the fatty acid beta-oxidation cycle. It is composed of four alpha-subunits (HADHA) harboring long-chain enoyl-CoA hydratase and long-chain L-3-hydroxyacyl-CoA dehydrogenase (LCHAD) and four beta-subunits (HADHB) harboring long-chain 3-ketoacyl-CoA thiolase (LKAT). Mutations in either subunit can result in TFP deficiency with reduced activity of all three TFP enzymes. We characterize 15 patients from 13 families with beta-subunit mutations by clinical, biochemical, and molecular features. Three clinical phenotypes are apparent: a severe neonatal presentation with cardiomyopathy, Reye-like symptoms, and early death (n=4); a hepatic form with recurrent hypoketotic hypoglycemia (n=2); and a milder later-onset neuromyopathic phenotype with episodic myoglobinuria (n=9). Maternal HELLP syndrome occurred in two mothers independently of the fetal phenotype. Mutational analysis revealed 16 different mutations, the majority being missense mutations (n=12). The predominance of missense mutations and the milder myopathic phenotype are consistent. Based upon homology to yeast thiolase that has been characterized structurally, the mutation localization within the protein correlates with the clinical phenotype. Outer loop mutations that are expected to alter protein stability less were only present in milder forms. The degree of reduction in thiolase antigen also correlated with the severity of clinical presentation. Although TFP deficiency is highly heterogeneous, there is genotype-phenotype correlation.

Expression of a Streptomyces 3-hydroxysteroid oxidase gene in oilseeds for converting phytosterols to phytostanols.

Plant sterols and their hydrogenated forms, stanols, have attracted much attention because of their benefits to human health in reducing serum and LDL cholesterol levels, with vegetable oil processing being their major source in several food products currently sold. The predominant forms of plant sterol end products are sitosterol, stigmasterol, campesterol and brassicasterol (in brassica). In this study, 3-hydroxysteroid oxidase from Streptomyces hygroscopicus was utilized to engineer oilseeds from rapeseed (Brassica napus) and soybean (Glycine max), respectively, to modify the relative amounts of specific sterols to stanols. Each of the major phytosterols had its C-5 double bond selectively reduced to the corresponding phytostanol without affecting other functionalities, such as the C-22 double bond of stigmasterol in soybean seed and of brassicasterol in rapeseed. Additionally, several novel phytostanols were obtained that are not produced by chemical hydrogenation of phytosterols normally present in plants.

MS/MS-based newborn and family screening detects asymptomatic patients with very-long-chain acyl-CoA dehydrogenase deficiency.

OBJECTIVES: To determine whether asymptomatic persons with biochemical evidence of very long-chain acyl-CoA dehydrogenase (VLCAD) deficiency identified through expanded newborn screening with tandem mass spectometry have confirmed disease. STUDY DESIGN: We characterized 8 asymptomatic VLCAD-deficient individuals by enzyme and/or mutational analysis and compared them with clinically diagnosed, symptomatic patients with regard to mutations, enzyme activity, phenotype, and age of disease onset. RESULTS: VLCAD molecular analyses in 6 unrelated patients revealed the previously reported V243A mutation, associated with hepatic or myopathic phenotypes, on 7/12 alleles. All other mutations were also missense mutations. Residual VLCAD activities of 6% to 11% of normal were consistent with milder phenotypes. In these identified individuals treated prospectively with dietary modification as preventive measures, clinical symptoms did not develop during follow-up. CONCLUSIONS: MS/MS-based newborn screening correctly identifies VLCAD-deficient individuals. Based on mutational and enzymatic findings, these infants probably are at risk of future disease. Because life-threatening metabolic derangement can occur even in otherwise mild phenotypes, we advocate universal newborn screening programs for VLCAD deficiency to detect affected patients and prevent development of metabolic crises. Longer-term follow-up is essential to define outcomes, the definite risk of future disease, and appropriate treatment recommendations.
