Assessment of MRI criteria for MS in Japanese MS and HAM/TSP.

We evaluated the usefulness of the MRI criteria for multiple sclerosis (MS) proposed by Paty et al and Fazekas et al in 36 Japanese MS patients, using HTLV-I-associated myelopathy (HAM)/tropical spastic paraparesis (TSP) as the control. Although 30 of 36 HAM/TSP patients had multiple white matter lesions on T2-weighted cranial MRI, only two fulfilled the MRI criteria for MS. At the same time, 31 of the 36 MS patients fulfilled the primary MRI criterion, yielding 93% specificity and 86% sensitivity for the criterion. MS has disease-specific MRI abnormalities.

[Assessment of nucleolar organizer regions (Ag-NORs) in bronchial dysplasia and roentgenographically occult bronchogenic squamous cell carcinoma].

Mean argyrophil nucleolar organizer regions (Ag-NORs) per nucleus were determined on columnar epithelium, bronchial dysplasia, roentgenographically occult squamous cell carcinoma and squamous cell carcinoma with roentgenographically findings in 76 formalin-fixed, paraffin-embedded resected specimens. 1) Mean Ag-NOR counts per nucleus (mean +/- standard deviation) were 1.67 +/- 0.30 in 10 columnar epitheliums, 2.74 +/- 0.41 in 13 lesions with bronchial dysplasia, 3.75 +/- 0.59 in 57 lesions with roentgenographically occult squamous cell carcinoma and 4.44 +/- 0.51 in 14 lesions with squamous cell carcinoma with roentgenographically findings. There were significant differences between each two groups. 2) In lesions with squamous call carcinoma, mean Ag-NOR counts per nucleus of T2, T3 or T4 tumor were significantly higher than those of T1 tumor. Mean Ag-NOR counts per nucleus of N1 or N2 tumor were significantly higher than those of N0 tumor.

[Influence of endotoxin on sensitivity to oxytocin in pregnant rat myometrium].

In order to clarify the mechanism of preterm labor induced by intrauterine infection, we studied the effect of endotoxin on oxytocin sensitivity of the myometrium in the pregnant rat. We administrated bacterial endotoxin obtained from Escherichia coli to the pregnant rat at preterm. By using a myometrial strip, we determined the effect of oxytocin on contractility, production of prostaglandins (PGs) and phosphoinositide hydrolysis. 1. Endotoxin increased myometrial sensitivity to oxytocin in the pregnant rat at preterm. The pD2 value was equal to that of myometrium taken from term pregnant rat. This hypersensitivity was significantly inhibited by indomethacin. 2. In the myometrial strips from rats to which endotoxin was administered, the concentrations of PGF2 alpha and PGE2 had already developed a tendency to increase during spontaneous contractions, when compared to the controls. There was seen a significant additional increase due to oxytocin. PGF2 alpha production was also significantly inhibited by indomethacin. 3. Oxytocin significantly stimulated total inositol phosphate production in a dose-dependent manner in myometrium taken from preterm pregnant rat. The inositol 1.4.5-trisphosphate production stimulated by oxytocin significantly increased in comparison to the controls. These results suggest that not only the production of PGs but phosphoinositide hydrolysis of transmembrane signaling pathways would play an important role in the mechanism of myometrial contraction at preterm when intrauterine bacterial infection developed.

[Detection of human papillomavirus type 16, 18 and 33 DNA in stage I (pT1N0M0) squamous cell carcinoma of the lung by polymerase chain reaction].

Human papillomavirus (HPV) has been considered one of the important factors of malignant change. To investigate the occurrence of HPV DNA in primary squamous cell carcinoma of the lung, we tried to detect HPV DNA by polymerase chain reaction (PCR). Formalin-fixed paraffin-embedded tissue specimens from 8 cases of stage I (pT1N0M0) squamous cell carcinoma of the lung were examined for the presence of human papillomavirus DNA (type 16, 18 and 33) by polymerase chain reaction. DNA was extracted from double 5 microns sections of each specimen by deparaffinization and proteinase K digestion. Using the primers described previously by Shimada et al, target DNA sequences were selectively amplified through 40 cycles of 94 degrees C for 1 min, 55 degrees C for 2 min, and 72 degrees C for 2 min. When stained band was unclear, second PCR was conducted using product of first PCR as template of second PCR. Human papillomavirus type 18 was present in 1 case (12.5%) in contrast that type 16 and 33 were not detected in these cases.

[Results of surgical treatment of intrathoracic recurrence after complete resection of non-small cell lung cancer: clinical significance of subsequent lesion in lung parenchyma].

Results of surgical treatment for 33 intrathoracic recurrence after complete resection of non-small cell lung cancer were analyzed. Prognosis of the second surgical treatment were favorable in patients with subsequent cancer with in situ component and solitary lesion in lung parenchyma. Retrospective study of 53 patients who recurred and were thoroughly followed up their clinical course until lung cancer death revealed that the solitary one tends to be confined to the intrathoracic location, and the multiple one did not confined to the intrathoracic location but also extended to the extrathoracic distant metastasis or to the supraclavicular lymph nodes.

Tumor doubling time and prognostic assessment of patients with primary lung cancer.

BACKGROUND: Relationships between tumor doubling time (DT) and other prognostic factors and the risk of death related to these factors are not yet fully understood. METHODS: Tumor doubling time of primary lung carcinomas of 174 patients, detected in a limited number of local municipalities during a limited period, was calculated using the Schwartz formula. Survival rate of the 174 patients was compared with reference to categories of prognostic factors (univariate analyses) and significant factors affecting survival were identified by multivariate analyses using the Cox proportional hazard model. RESULTS: Tumor doubling time had a log normal distribution. There was a significant difference in mean DT in relation to sex, smoking history, presence of symptoms, cell type, primary tumor factor, and stage. Univariate analyses showed a significant difference in survival in relation to DT, age, sex, method of tumor detection, smoking history, symptoms, therapy, cell type, primary tumor (T) factor, regional lymph node (N) factor, distant metastasis (M) factor, and stage. Multivariate analyses using the Cox's proportional hazard model in a stepwise fashion identified a final set of five significant variables: N factor (P = 0.0001); therapy (P = 0.0016); M factor (P = 0.0017); T factor (P = 0.0018), and DT (P = 0.0152). CONCLUSIONS: Tumor doubling time was an independent and significant prognostic factor for lung cancer patients.

Methylcobalamin (methyl-B12) promotes regeneration of motor nerve terminals degenerating in anterior gracile muscle of gracile axonal dystrophy (GAD) mutant mouse.

We examined the effects of methylcobalamin (methyl-B12, mecobalamin) on degeneration of motor nerve terminals in the anterior gracile muscle of gracile axonal dystrophy (GAD) mutant mice. GAD mice received orally methyl-B12 (1 mg/kg body wt/day) from the 40th day after birth for 25 days. In the distal endplate zone of the muscle, although most terminals were degenerated in both the untreated and methyl-B12-treated GAD mice, sprouts were more frequently observed in the latter. In the proximal endplate zone, where few degenerated terminals were seen in both groups of the mice, the perimeter of the terminals was increased and the area of the terminals was decreased significantly in the methyl-B12-treated GAD mice. These findings indicate that methyl-B12 promotes regeneration of degenerating nerve terminals in GAD mice.

Transmission of hepatitis C virus from mothers to infants. The Vertical Transmission of Hepatitis C Virus Collaborative Study Group.

BACKGROUND: Although there are case reports of vertical transmission of hepatitis C virus (HCV), it remains uncertain to what extent infected mothers transmit this virus to their infants. METHODS: We investigated the transmission of HCV from infected mothers to their babies by analyzing HCV RNA in the blood. Three independent studies were performed. First, 7698 parturient women were tested for anti-HCV antibodies; 53 were positive. Their 54 infants (including one set of twins) were followed prospectively for at least six months and tested for HCV disease were prospectively studied. Third, the families of three HCV-infected infants were examined retrospectively. RESULTS: Of the 53 antibody-positive mothers, 31 were also positive for serum HCV RNA: Three of the 54 babies born to these mothers (5.6 percent) became positive for HCV RNA during the follow-up period. None of the babies of the 22 women who were antibody-positive but HCV RNA-negative became positive for HCV RNA: In the second study, HCV RNA was detected in one of the six infants of infected mothers. In the third study, HCV RNA was detected in the mothers of the three HCV-infected infants. In each of the seven infected infants we studied, the genomic sequence of HCV was almost identical to that from the mother. These seven mothers had significantly higher titers of HCV RNA than did the mothers of infants with no evidence of infection (mean [+/- SD], 10(6.4 +/- 0.5) vs. 10(4.4 +/- 1.5) per milliliter; P < 0.001). CONCLUSIONS: HCV is vertically transmitted from mother to infant, and the risk of transmission is correlated with the titer of HCV RNA in the mother.

Endocrinologic and biophysical responses to prolonged (24-hour) hypoxemia in fetal goats.

OBJECTIVE: The purpose of this study was to observe some fetal endocrinologic and biophysical responses to prolonged (24-hour) nonacidemic hypoxemia in fetal goats. STUDY DESIGN: Endocrinologic and biophysical changes during continuous infusion of nitrogen into the maternal trachea were examined in six chronically instrumented goat fetuses at 123 to 131 days' gestation. We measured arginine vasopressin, adrenocorticotropic hormone, cortisol, epinephrine, and norepinephrine as endocrinologic parameters and fetal heart rate, fetal blood pressure, and fetal breathing movements as biophysical parameters. RESULTS: Fetal arterial PO2 declined from 25.1 +/- 2.1 mm Hg at baseline to 16.8 +/- 0.9 mm Hg at 1 hour of hypoxemia and remained at this level (p < 0.05). Arginine vasopressin, adrenocorticotropic hormone, and epinephrine initially rose in response to hypoxic stress, gradually declined, and by 24 hours were not significantly different from control values. Cortisol and norepinephrine remained significantly elevated through the experiments. Fetal breathing movements as a percentage of total time were 34.9% +/- 1.2% at baseline, 12.9% +/- 3.7% at 1 hour, and 70% to 80% of control values from 4 hours. Fetal heart rate was significantly reduced for the first 10 minutes, then progressively became tachycardiac thereafter. Systolic fetal blood pressure was significantly elevated for only 30 minutes. CONCLUSION: Arginine vasopressin, adrenocorticotropic hormone, and epinephrine of the endocrinologic parameters and fetal heart rate, fetal blood pressure, and fetal breathing movements are normalized during prolonged nonacidemic hypoxemia in fetal goats.

Multicentricity in resected occult bronchogenic squamous cell carcinoma.

The frequency and the treatment of multicentricity in 127 patients with resected roentgenographically occult bronchogenic squamous cell carcinoma were studied. The cumulative rate and the incidence of postoperative metachronous multiple primary lung cancer were 0.11 at 5 years after initial operation and 0.022 per patient-year, respectively. The cumulative rate and the incidence of second primary lung cancer, which includes synchronous and subsequent metachronous cancer in patients with initial lung cancer, were 0.17 at 5 years after the initial operation and 0.041 per patient-year, respectively. The cumulative rate and the incidence of third primary lung cancer in patients with second primary lung cancer were 0.47 at 5 years, which was significantly higher (p = 0.05) than that of second primary lung cancer, and 0.11 per patient-year, respectively. In all 12 patients with synchronous multiple primary lung cancer, no recurrence was observed after treatment, but 3 had subsequent multiple primary lung cancer. Among the 13 patients with postoperative metachronous multiple primary lung cancer, recurrence was observed in 1 of the 6 patients who underwent resection and in 2 of the 4 patients treated with laser or radiation therapy or both. The overall survival rate at 5 years after initial operation in patients with solitary and those with multicentric occult bronchogenic squamous cell carcinomas was 0.90 and 0.59, respectively.

In situ demonstration of the HTLV-I genome in the spinal cord of a patient with HTLV-I-associated myelopathy.

Using polymerase chain reaction (PCR) and in situ hybridization, we investigated the HTLV-I genome in the CNS of an HTLV-I-associated myelopathy/tropical spastic paraparesis (HAM/TSP) patient with a 20-year disease duration. Neuropathologically, there was severe white matter degeneration throughout the spinal cord, but lymphocytic infiltrates were not evident in any lesion. PCR amplification of the pX region of HTLV-I DNA detected its sequence in the spinal cord and all extra-CNS tissue samples. In situ hybridization using probes complementary to the pX and gag regions detected the HTLV-I genome in the cytoplasm and nucleus of cells in the thoracic cord. The findings indicate a direct involvement of HTLV-I in the neurodegeneration of HAM/TSP.

[Significance of surgical treatment for bone metastasis from lung cancer].

To assess significance of surgical therapy for patients who had metastatic bone tumor from primary lung cancer, sites of recurrence in patients who underwent surgical resection for lung cancer lesions were analyzed. Clinical courses of patients who underwent surgical therapy for metastatic bone tumor were analyzed, too. 1) From 1985 to 1989, 88 patients died from lung cancer in our hospital who has underwent pulmonary resection for lung cancer lesions. Of these, 22 patients had metastatic bone tumor. Sixty patients died who had underwent complete resection for lung cancer lesions. In eight patients of these 60 cases, bone were first site of recurrence. Of these eight patients, length from pulmonary resection to detection of bone metastasis were 4-23 months. In the other hand, length from detection of bone metastasis of death were 3-24 months. There was no correlation between two length. All of these eight patients had another metastatic lesions at the time of death. 2) From 1985 to July 1993, twelve patients underwent surgical therapy for their bone metastasis of primary lung cancer. Of these, four had metastatic cancer in thoracic vertebral body, two had in clavicle, two had in rib, three had in femoral bone, another had in tibial bone. Objective of operation were improvement of paralysis or pain, prevention of fracture, and so on. Most cases succeeded to get improvement in a short time, but some cases got worse soon. Surgical treatment was effective to improve paralysis or pain, and to increase quality of life. But it can not be effective except appropriate management in a short time because another symptom raise soon.

[Assessment of nucleolar organizer regions (Ag-NORs) in primary lung cancer--correlation between Ag-NORs and tumor doubling time].

A total of 86 resected cases with primary lung cancer were examined on relationships between argyrophil nucleolar organizer regions (Ag-NORs) and other prognostic factors and correlation between Ag-NORs and tumor doubling time (DT). Survival rates were compared between patients with low Ag-NOR counts and patients with high Ag-NOR counts. 1) After logarithmic conversion of mean Ag-NOR counts in lung cancer, a small skewness (0.00486) and a small kurtosis (-0.7859) showed a normal distribution. Mean Ag-NOR counts was found to have a log-normal distribution. 2) There was a significantly inverse correlation between mean Ag-NOR counts and DT (correlation coefficient -0.705, p < 0.001). By plotting log (DT) on X axis and log (Ag-NORs) on Y axis, a formula representing a linear correlation was obtained: Y = 1.17-0.312 X, correlation coefficient -0.886. 3) The five-year survival rate (35%) of 46 patients with mean Ag-NOR counts which were more than or equal to 3.0 significantly lower than that (74%) of 40 patients with mean Ag-NOR counts of less than 3.0.

[A case of lung cancer with axillary nodal involvement].

A 57-year-old man with lung cancer was reported. Primary tumor was located at left S1+2, and directly invaded to chest wall (from 1st. rib to 4th rib). Hypercalcemia and delirium were observed. Whole body examination showed that no distant metastasis except for nodal swelling of left axillary region. Left upper lobectomy combined with chest wall resection was performed. Hilar, mediastinal and axillary nodes were also dissected. Histological examination revealed that nodal involvement was not present at neither hilar or mediastinal region, but was present in axillary node. It was thought that lymphatic extension had occurred from trough chest wall to axillary nodes but not through mediastinal rout. So systematic dissection of locally invaded region as well as hilar and mediastinal region was recommended in each cases.

[Incidence of nodal involvement of #12u nodes (lymph nodes around the upper lobe bronchus) in cases with right middle lobe, right lower lobe, and left lower lobe carcinoma].

#12u nodes are peribronchial lymph nodes around the upper lobe bronchus which should not be routinely dissected in cases with middle lobe carcinoma or lower lobe carcinoma. In this paper, #12u were examined histologically in 152 lung cancer cases. As a results, 14 of 152 (9.2%) had nodal diseases in #12 nodes. #12u nodes were involved in 3 of 30 N1 diseases (10%), and in 11 of 35 N2 diseases (31.4%). There was a case which had no nodal involvement except for #12u node. And in three cases, we found only one nodal metastasis except for #12u nodes. If we did not dissect #12u nodes, these cases will be relapse in near future. Thus, for curative operation, #12u nodes should be dissected as a standard operation.

The influence of prostaglandins on the contractile response to electrical field stimulation in rabbit myometrial and cervical smooth muscles.

Indomethacin (10 microM) significantly reduced the contractile responses to electrical field stimulation (EFS) in the non-pregnant myometrium (5-50 Hz by an average of 25.7%) and the pregnant myometrium (30-50 Hz by an average of 16.5%). On the other hand, it significantly increased the contractile responses of the cervix in non-pregnant (2.5-30 Hz by an average of 21.5%) and pregnant (2.5-10 Hz by an average of 12.5%) muscular strips. Prostaglandin (PG) E2 (0.1-1000 nM) strongly and PGI2 (0.1-1000 nM) slightly and dose-dependently inhibited the contractile response to EFS of the non-pregnant cervical muscle, but not of the myometrium. In the pregnant uterus, the contractile responses of the cervical muscle were similar to those of the non-pregnant cervix, while the response of the myometrium was increased significantly and dose-dependently by the application of PGE2 or PGI2.PGF2 alpha (1000 nM) significantly increased the contractile response of the myometrium but not cervical muscle to EFS in the pregnant rabbits. In non-pregnant muscles, the contractile responses of the myometrium and cervical smooth muscle to EFS were not influenced by the treatment with PGF2 alpha.PGE2 (0.1-1000 nM) also inhibited the contractile response to direct muscle stimulation, but the inhibition was significantly less than that of the response to indirect stimulation. PGI2 slightly inhibited the contractile responses to both indirect and direct stimulation. These findings indicate that PGE2, PGI2 and PGF2 alpha increase the contractile response of pregnant myometrium to EFS.(ABSTRACT TRUNCATED AT 250 WORDS)

Oxytocin, PGE2 and PGF2 alpha stimulate the production of inositol phosphates in the rabbit myometrium.

Phosphoinositide breakdown is thought to be important in regulating a variety of transmembrane signal transduction in the action of oxytocic agent during uterine smooth muscle contraction. We investigated the effects of oxytocin and prostaglandins (PGE2 and PGF2 alpha) on phosphoinositide hydrolysis in the myometrium taken from non-pregnant and pregnant rabbits by measuring the accumulation of total inositol phosphates (IP). Oxytocin strongly, and PGE2 and PGF2 alpha slightly but significantly, stimulated IP production in both the non-pregnant and pregnant myometria. Oxytocin more markedly accelerated the IP production in pregnant myometrium than in non-pregnant myometrium. However, IP production stimulated by PGE2 and PGF2 alpha was much the same in non-pregnant and pregnant myometria. The amount and time course of the increase in the production of the total IPs by oxytocin are quite different from those by PGs. It seems that the mechanism by which oxytocin stimulates phospholipase C is different from that of the PGs. It is suggested that transmembrane signalling pathways of phosphoinositide hydrolysis play an important role in the each mechanism.
