Rapid and efficient immobilization of soluble and small particulate antigens for solid phase radioimmunoassays.

A rapid method of antigen immobilization (10 min.) was developed using soluble antigens (bovine serum albumin, epidermal growth factor, and goat IgG) and small particulate antigens (Keyhole limpet hemocynanine and E. coli) by drying them on filter paper discs. This technique results in a high % of the soluble antigen remaining firmly bound, goat IgG (89%), bovine serum albumin (73%). All the antigens we tested retained their antigenicity after drying as detected by [125I] labeled staphylococcal protein A radioimmunoassay. This method of antigen immobilization was compared to adsorption to plastic wells and was found to be much faster (10 min vs 18 hrs) and was 5 times more efficient than adsorption to plastic wells. Using this technique, we were able to detect as little as 40 ng of bovine serum albumin. These characteristics suggest that this technique of soluble antigen immobilization may be useful in rapid detection of antibodies to many different antigens, as well as detecting ng amounts of the antigens directly.

Dose response of intravenous mannitol on the human eye.

At a dose of 1 gm/kg body weight, intravenous mannitol caused a significant reduction in both intraocular pressure and anterior chamber depth in seven sitting, normal subjects. Intraocular pressure dropped from 15.3+/-4.6 to 11+/-3.4 mm Hg and anterior chamber depth decreased from 3.18+/-.34 to 3.13+/-.34 mm Hg. Intravenous mannitol may lower intraocular pressure without reducing vitreous volume.

Interim FDA report on intraocular lenses.

Intraocular lenses have been regulated by the Food and Drug Administration since February 9, 1978. Data reported through August 9, 1979 reveal 177,503 lenses of four different classes have been implanted. The anterior chamber intraocular lens appears to have a higher incidence of certain postoperative complications compared with both control operations and implantation of lenses in the other three classes. The anterior chamber intraocular lens appears to be associated with a lower level of visual acuity compared with both control cases and the other three lens classes during the first six months following implantation. Some complications occurred less frequently following implantation of intraocular lenses than in control cases.

An evaluation of iridology.

Iridology is an analysis of health based on examination of the iris of the eye. One hundred forty-three patients had photographs taken of both eyes. Nine-five patients were free of kidney disease, defined as a creatinine level of less than 1.2 mg/dL (mean, 0.8 mg/dL), and 48 had kidney disease severe enough to raise the plasma creatinine level to 1.5 mg/dL or greater (mean, 6.5 mg/dL). Three ophthalmologists and three iridologists viewed the slides in a randomized sequence without knowledge of the number of patients in the two categories or any information about patient history. Iridology had no clinical or statistically significant ability to detect the presence of kidney disease. Iridology was neither selective nor specific, and the likelihood of correct detection was statistically no better than chance.

Immobilization of viral antigens on filter paper for a [125I]staphylococcal protein A immunoassay: a rapid and sensitive technique for detection of herpes simplex virus antigens and antiviral antibodies.

A new technique is described for the rapid detection and quantitation of herpes simplex virus (HSV) antigens and antiviral antibodies. It involves immobilization of HSV antigens on filter paper discs and subsequent analysis by 125I-labeled staphylococcal protein A (SPA) radioimmunoassay. A specially designed 96-well filtration device is employed which serves both as an incubation chamber and as a filtration manifold. It is rapid, simple, sensitive and specific, and requires only small volumes of antiserum and few target cells. The results may be readily and objectively quantitated. This technique permits the simultaneous assay of a large number of specimens in less than 1 h. Its sensitivity is considerably greater than that of other currently used immunologic techniques, and it is amenable to automation. These characteristics suggest that this [125I]SPA immunofiltration technique may be applicable to the rapid diagnosis of viral infections.

Argon laser trabeculotomy:long-term follow-up.

A five-year follow-up of argon laser trabeculectomy in a group of 28 male patients with bilateral patients with bilateral primary open-angle glaucoma showed an overall mean pressure drop of 10.3 mm Hg in treated eyes. Twenty percent of the treated eyes had a pressure less than 20 mm Hg up to two years after treatment. No specific laser parameters were associated with pressure lowering. The pressure drop appears more patient dependent than treatment dependent. A randomized clinical trial is suggested.

Future impact of manpower needs on ophthalmology training programs.

Current projections for ophthalmology manpower needs in the year 2000 suggest that the number of new ophthalmologists trained should be frozen at 500 a year. Factors that would cause a need to train more ophthalmologists would be a shorter workweek or workyear, an increased length of time for each patient visit, universal comprehensive health insurance, an earlier age of retirement, and an increased incidence of eye disease. The factor that would most likely reduce the need for new ophthalmologists would be the transfer of primary eye care delivery to nonophthalmologists. Modulation of resident numbers should be based on peer-judged program quality, not on geography or government edict.

Clinical evaluation of continuous-wear hydrophilic lenses.

Twenty patients who had previously not worn contact lenses were fitted with a thin, continuous-wear hydrophilic contact lens in the right eye for 12 weeks. Seventeen patients completed the study and tolerated the lenses well. Some patients developed changes in corneal thickness, corneal curvature, and refraction, and some changes in visual acuity were reversed when the lens was removed. Many developed lens opacities that did not interfere with vision. We encountered no infections or corneal neovascularization in this study.

A continuous-wear hydrophilic lens. Prophylactic topical antibiotics.

Seventeen patients, who were fitted with a continuous-wear hydrophilic lens for 12 weeks, received topical neomycin sulfate-polymyxin B sulfate-gramicidin (Neosporin) eyedrops in one eye only at a frequency of 3.7 times per day. The continuous wear of a hydrophilic lens in and of itself did not after normal bacterial flora, and the use of the antibiotic drops did not alter normal conjunctival flora. In addition, the eyedrops were not more effective when used in conjunction with the hydrophilic lens and did not appear to produce resistant organisms during the 12-week study. In normal eyes, a continuously worn hydrophilic soft contact lens that is well tolerated does not adversely affect the bacterial flora and the addition of neomycin sulfate-polymixin B-gramicidin drops at a frequency of 3.7 times per day does not appear to be beneficial or necessary.
