[The clinical picture of Lyme disease in neurology].

Neuroborreliosis of central and peripheral nervous system was found in 24 patients with typical and atypical clinical syndromes. Information about tick bite was obtained in only 1/3 of our patients. Specific antibodies were found in 3/4 by flourescent antibody test, oligoclonal bands in 1/2, and signs of inflammation in cerebrospinal fluid in 1/3 of patients. The majority had favourable reaction to antibiotic therapy. It is necessary to investigate every case of suspected neuroborreliosis because it can imitate many neurological diseases. One must insist on information about the tick bite and/or erythema migrans and search for specific antibody production, both in blood and cerebrospinal fluid.

[Chronic encephalomyelitis caused by Borrelia burgdorferi. Case report].

We present a female patient with typical third stage neuroborreliosis with progressive chronic encephalomyelitis. One month after a tick bite, in the first stage of Lyme disease, she had myalgias during ten days and after one year polyarthralgias and polyarthritis. Neurological problems occurred 15 years after the tick bite with headache, nystagmus, intentional tremor and spastic paraparesis with sphincter disturbances. Etiological diagnosis was established after three years. Cytobiochemical findings in cerebrospinal liquor were normal but oligoclonal IgG bands were found. Fluorescent antibody test was positive in serum (1:75) as well as ELISA (1:447). The patient reacted favourably to intravenous crystal penicillin 20 x 10(6) units daily during 18 days. Till now, she is in remission and has only mild paresis of the left leg.

[Rhombencephalitis as a manifestation of neuroborreliosis].

Rhombencephalitis is a rare neurological manifestation of stage II of Lyme disease. We presented two cases with no recollection of tick bite nor erythema migrans and with cranial nerve nuclei, pyramidal tract and cerebellar tracts involvement of sudden onset, inflammatory syndrome in cerebrospinal liquor, positive oligoclonal bands and elevated IgG index. A spontaneous remission occurred in both cases, but antibiotic was given intravenously for prevention of relapses and disease progression.

The role of sympathetic dysfunction in blood pressure regulation in uremic patients treated by continuous ambulatory peritoneal dialysis.

The role of autonomic nervous system dysfunction in the regulation of arterial blood pressure in 4 male and 10 female uremic patients, age 35-65 years (57 +/- 9 years), treated by continuous ambulatory peritoneal dialysis (CAPD) for 6-40 months (20 +/- 12 months), was studied. The integrity of the entire autonomic reflex arc was checked using Valsalva's test and the orthostasis challenge test. In parallel, efferent sympathetic function was tested with the handgrip test. A significantly decreased Valsalva ratio was found in 7 patients (50%), and it was normal in only 2 patients (14%). The handgrip test was subnormal in 5 patients (36%) and abnormal in 7 patients (50%). On the contrary, the orthostatic challenge test showed normal compensatory reaction in 10 patients (72%). The evaluation of three autonomic tests showed combined (afferent + efferent) lesion in 6 patients (43%), predominant afferent lesion in 3 patients (21%), and in 3 patients predominant efferent lesion. In 2 patients (14%) there was no end-organ response. Despite significant autonomic nervous system dysfunction in uremics on CAPD, as demonstrated by these results, blood pressure regulation in the supine and upright position was adequate in most patients (72%), indicating a relatively greater role of other blood pressure-regulating factors.

Erythrocyte delta-aminolevulinate dehydratase measurements in Balkan endemic nephropathy.

Balkan endemic nephropathy (BEN) is a chronic progressive kidney disease leading to renal insufficiency. The possible etiological role of some toxic factors was considered in this study by investigating the activity of erythrocyte delta-aminolevulinate dehydratase (ALA-D), an enzyme influenced by various environmental factors. We observed that ALA-D activity was markedly decreased in patients with BEN and in 32% of their healthy family members. Glutathione concentration was found normal in all the groups studied, however, it was possible to reactivate the enzyme in vitro by addition of exogenous glutathione. Percent activation was significantly higher in the groups with decreased ALA-D activity. Blood lead levels were within normal range. The results suggest a normal synthesis of ALA-D apoenzyme, and also the presence of some factors, environmental, metabolic, or genetic, which may affect the enzyme activity through binding to the reactive groups in the active center of this enzyme or by oxidation of the reactive groups. Additional studies are necessary to further elucidate the significance of decreased ALA-D activity in BEN and their healthy relatives.

Dexamethasone suppression test in patients with Parkinson's disease.

The Dexamethasone Suppression Test (DST), supposed to effectively distinguish between endogenous and nonendogenous depression, was performed in a group of 34 patients with Parkinson's disease. Abnormal DST results were observed in 50% of the patients. The patients were clinically divided into subgroups of depressed and nondepressed parkinsonians. Abnormal DST results were significantly more frequent in depressed (75%) than in nondepressed parkinsonians (27.7%).

Parathyroid sonography in secondary hyperparathyroidism: correlation with clinical findings.

We present the results of ultrasonic neck examination in 72 patients (47 males, 25 females) on chronic haemodialysis, with clinical and biochemical signs of increased function of the parathyroid glands. The findings of the ultrasonic examination performed using linear transducers of 5 and 7.5 MHz for the surface tissue are compared with the duration of haemodialysis treatment, biochemical parameters (calcium, phosphate, parathormone, alkaline phosphatase) and skeleton X-ray findings. In 18 patients parathyroid glands were not visible, in 30 patients there were one or two enlarged (i.e. abnormal) parathyroid glands, and in the remaining 24 patients three or four of the glands were enlarged. Abnormal parathyroid glands were more often found in patients who had been dialysed for a longer period of time, as well as in those with a higher parathormone level in the blood. It is concluded that ultrasonic examination of the neck is very useful in the diagnosis of enlarged parathyroid glands, and helpful in the management of patients on chronic haemodialysis.

[Sclerotization of the parathyroid glands--description of a method].

The method of ultrasonically guided percutaneous 95% ethanol injection of enlarged parathyroid gland is presented. Treatment was performed on the patient who has been treated with chronic hemodialysis for eight years and developed the symptoms of secondary hyperparathyroidism and who has had one enlarged parathyroid gland detected with ultrasound and confirmed by cytology. One month later the symptoms subsided, the laboratory findings improved and ultrasound examination revealed the decreased volume of the parathyroid. The paper points out the value of this method in the treatment of hyperparathyroidism.
